Laryngeal Adenoid Cystic Carcinoma-Two Case Reports and Literature Review.

Laryngeal adenoid cystic carcinoma (LACC) is a rare malignant tumor, representing less than 1% of all laryngeal cancers. This tumor arises from the minor salivary or mucous glands of the upper respiratory tract and is characterized by slow growth, local invasiveness, and a high propensity for distant metastases. The submucosal extension and lack of symptoms often delay diagnosis. Such features present notable challenges in clinical practice. We report two cases of LACC in elderly male patients with clinically advanced laryngeal tumors. The first case involved a 73-year-old patient with bilatera

Biomarkers in ALS trials: from discovery to clinical utility.

INTRODUCTION: Motor neuron disease (MND), also known as amyotrophic lateral sclerosis (ALS), is a progressive neurodegenerative disorder characterized by motor neuron degeneration, leading to muscle weakness, paralysis, and eventual respiratory failure. Despite advances in understanding its pathology, effective therapies remain limited, underscoring the need for reliable biomarkers to aid early diagnosis, monitor disease progression, and optimize clinical trials. This systematic review explores the role of biomarkers in ALS, focusing on their application in clinical trials to accelerate therap

An Atypical Descending Variant of Guillain-Barré Syndrome With Bulbar Palsy, Autonomic Instability, and Delayed Colonic Pseudo-Obstruction: A Case Report.

Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy that is the most common cause of acute flaccid paralysis worldwide. The classical form manifests as an ascending, symmetrical weakness, but atypical variants, including descending presentations, have been increasingly recognized. Such variants often pose diagnostic challenges and are associated with more severe disease and prolonged recovery. We describe a 70-year-old woman with well-controlled hypertension who presented with progressive dysphagia, pooling of saliva, and upper limb weakness. Clinical examination

Advance directives in amyotrophic lateral sclerosis - a systematic review and meta-analysis.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease of the upper and lower motoneuron. It is associated with a life expectancy of 2-4 years after diagnosis. Individuals experience paralysis, dysphagia, respiratory failure and loss of communicative function, rendering advance care planning (ACP) critically important. This systematic review primarily aimed to internationally compare the application of advance directives (AD) and ACP in ALS. Its secondary aim was to identify ACP preferences, identify fields for future research and to generate recommendations for improvi

Thyroid surgery outcomes in octogenarians: A national analysis.

BACKGROUND: The incidence of thyroid pathology increases with age. Yet octogenarians may face increased rates of complications after thyroid surgery. Using a nationally representative cohort, we evaluated the outcomes of thyroidectomy among octogenarians. METHODS: All patients ≥55 years who underwent inpatient thyroidectomy were identified using the 2010 to 2020 National Readmissions Database. Patients ≥80 years were classified as octogenarians (others: nonoctogenarians). Multivariable models were built to evaluate independent associations between octogenarians and key clinical and financial

Motor neuron replacement therapy for amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis is a motor neuron degenerative disease that is also known as Lou Gehrig's disease in the United States, Charcot's disease in France, and motor neuron disease in the UK. The loss of motor neurons causes muscle wasting, paralysis, and eventually death, which is commonly related to respiratory failure, within 3-5 years after onset of the disease. Although there are a limited number of drugs approved for amyotrophic lateral sclerosis, they have had little success at treating the associated symptoms, and they cannot reverse the course of motor neuron degeneration. Thus

The impact of co-morbidity on the disease burden of VTE.

Venous thromboembolism (VTE) is often accompanied by co-morbidities, which complicate and confound data interpretation concerning VTE-related mortality, costs and quality of life. We aimed to assess the contribution of co-morbidities to the burden of VTE. The PREFER in VTE registry, across seven European countries, documented and followed acute VTE patients over 12 months. Patients with co-morbidities were grouped in major co-morbidity groups: cancer, cardiovascular (CV) comorbidity (other than VTE), CV risks, venous, renal, liver, respiratory, bone and joint diseases, and lower extremity para

The RECITE Study: A Canadian Prospective, Multicenter Study of the Incidence and Severity of Residual Neuromuscular Blockade.

BACKGROUND: Postoperative residual neuromuscular blockade (NMB), defined as a train-of-four (TOF) ratio of <0.9, is an established risk factor for critical postoperative respiratory events and increased morbidity. At present, little is known about the occurrence of residual NMB in Canada. The RECITE (Residual Curarization and its Incidence at Tracheal Extubation) study was a prospective observational study at 8 hospitals in Canada investigating the incidence and severity of residual NMB. METHODS: Adult patients undergoing open or laparoscopic abdominal surgery expected to last <4 hours, ASA p

RYR1 mutations as a cause of ophthalmoplegia, facial weakness, and malignant hyperthermia.

IMPORTANCE: Total ophthalmoplegia can result from ryanodine receptor 1 (RYR1) mutations without overt associated skeletal myopathy. Patients carrying RYR1 mutations are at high risk of developing malignant hyperthermia. Ophthalmologists should be familiar with these important clinical associations. OBJECTIVE: To determine the genetic cause of congenital ptosis, ophthalmoplegia, facial paralysis, and mild hypotonia segregating in 2 pedigrees diagnosed with atypical Moebius syndrome or congenital fibrosis of the extraocular muscles. Liver transplantation in infants weighing under 7 kilograms: management and outcome of PICU.

Liver transplantation (LT) is an established treatment for children with acute and chronic liver failure. Some reports suggest that infants under the age of 1 yr and children weighing under 13 kg are high-risk groups associated with less satisfactory results. This report describes our experience during the pediatric intensive care unit stay of 16 infants weighing <7 kg who received LT. We reviewed the records of 16 infants with median age 7.4 months and median weight 5.8 kg, who received 18 liver allografts, nine whole and nine reduced. We also reviewed the use of adrenergic agonist agents, an

Cyanidin-3-O-glucoside (C3G): A natural small-molecule compound for alleviating envenomation symptoms Induced by Bungarus multicinctus.

Bungarus multicinctus (many-banded krait) ranks among the world's most medically significant venomous snakes. Its venom, predominantly composed of α-bungarotoxin neurotoxins in a complex mixture, induces life-threatening respiratory paralysis, pulmonary failure, and often multi-organ dysfunction following envenomation. Building upon our discovery that the chemical dye Cy7-SE attenuates the toxicity of Bungarus multicinctus venom, this study employed network pharmacology to analyze molecular docking parameters between Cy7-SE and α-bungarotoxin. We subsequently applied computational virtual scre…

Case Report: Diagnosis and treatment strategy for a case of multiple chest wall implantations and neck lymph node metastasis following total areola endoscopic surgery for papillary thyroid carcinoma.

BACKGROUND: Endoscopic thyroid surgery has gained increasing prominence in the field of thyroid surgery. However, special attention should be paid to its limitations in total thyroidectomy, surgical tract implantation metastasis, and suboptimal cervical lymph node dissection. This case report describes the occurrence of chest wall implantations and neck lymph node metastases in an elderly patient following total areola endoscopic surgery for papillary thyroid carcinoma (PTC), emphasizing the need for appropriate diagnostic and therapeutic interventions in such cases. METHODS: The patient unde

Maternal and neonatal outcomes following emergency cesarean section or operative vaginal delivery for fetal distress in the second stage of labor.

BACKGROUND: Operative vaginal delivery and emergency cesarean section are critical interventions used in the second stage of labor when fetal distress threatens maternal or neonatal well-being. Prompt recognition of fetal compromise and timely decision-making are essential to ensure safe delivery outcomes while minimizing complications for both mother and baby. The aim of this study is to compare maternal and neonatal outcomes between operative vaginal delivery and emergency cesarean section in cases of fetal distress occurring during the second stage of labor. METHODS: This is a retrospectiv

Development of abnormalities at the neuromuscular junction in the SOD1-G93A mouse model of ALS: dysfunction then disruption of postsynaptic structure precede overt motor symptoms.

INTRODUCTION: The ultimate deficit in amyotrophic lateral sclerosis (ALS) is neuromuscular junction (NMJ) loss, producing permanent paralysis, ultimately in respiratory muscles. However, understanding the functional and structural deficits at NMJs prior to this loss is crucial for therapeutic strategy design. Should early interventions focus on reversing denervation, or supporting largely intact NMJs that are functionally impaired? We therefore determined when functional and structural deficits appeared in diaphragmatic NMJs relative to the onset of hindlimb tremor (the first overt motor sympt
