OBJECTIVE: To investigate the medial pathways of the right retromesenteric plane by reviewing multidetector CT (MDCT) findings in patients with acute pancreatitis and pyelonephritis.

We report 2 cases of complicated spontaneous dissection of the celiac artery, which were successfully treated by a stent graft. The first patient was a 47-year-old man who presented with acute abdominal pain. CT scan showed ruptured saccular aneurysm with surrounding retroperitoneal hematoma. The second patient was a 57-year-old man with progressive dissecting aneurysm. Endovascular stent graft was placed in the celiac trunk to control bleeding, and to prevent rupture in each patient. Follow-up CT scans showed complete obliteration of a dissecting aneurysm.

Castleman's disease is presented in the following article in more detail using the case report of a 55-year-old patient with abdominal pain without any significant pre-existing medical conditions. A computer tomography of the abdomen demonstrated a retroperitoneal tumor in the lower abdomen below the aortic bifurcation. Because malignancy of the tumor could not be excluded, surgical extirpation was performed without complications. Histopathological examination revealed the diagnosis of a localized castleman's tumor. Castleman's disease is a rare disease of the lymph nodes of unknown etiology.

We report a case of a patient with a fistula between the left ureter and abdominal aorta. The patient was a 44-year-old male who had undergone radiation therapy (intraoperative: 25 Gy, external beam: 50 Gy) and chemotherapy (CDDP: 250 mg) for retroperitoneal lymph node metastasis from seminoma. We report a novel double wire technique for ipsilateral access of the external iliac/common femoral artery transition side-branches in a patient with active extravasation. The case is noteworthy in that asymptomatic extravasation was noted in both the inferior epigastric and deep circumflex iliac branches on femoral angiography prior to intraaortic balloon pump insertion following emergent diagnostic left heart cathetrization. We speculate the side branch injury was related to the J-wire and describe how this complication may be best treated by the ipsilateral technique reported herein.
