Reverse total shoulder arthroplasty for massive irreparable rotator cuff tears in patients younger than 65 years old: results after five to fifteen years.

BACKGROUND: The role of reverse shoulder arthroplasty (RTSA) in the relatively young individual is currently unclear. Our study evaluates the midterm to long-term results of RTSA for patients aged younger than 65 years with pseudoparalysis secondary to massive irreparable rotator cuff tears, with or without arthritis. METHODS: Between 1997 and 2006, 46 RTSAs (41 patients) were performed. Mean age was 60 years (range, 46-64 years). At the latest follow-up, 5 patients had died and 1 was lost, leaving 35 patients (40 shoulders) with a mean follow-up of 93 months (range, 60-171 months). RESULTS:

Rotator cuff tears: association with acromion angulation on MRI.

OBJECTIVE: Using magnetic resonance imaging (MRI), evaluate the correlation of acromion angulation with thickening of the coracoacromial ligament (CAL) and narrowing of the subacromial space resulting in impingement upon the rotator cuff tendons. MATERIALS AND METHODS: Eighty-nine shoulder MRI studies performed on a 3T scanner were retrospectively analyzed by two blinded independent reviewers. Measurements of the acromion angle (delta angle), CAL thickness and distance between the CAL and humeral head were obtained. The data were categorized into two groups, delta angle less that and greater

Postural dependence of passive tension in the supraspinatus following rotator cuff repair: a simulation analysis.

BACKGROUND: Despite surgical advances, repair of rotator cuff tears is associated with 20-70% incidence of recurrent tearing. The tension required to repair the torn tendon influences surgical outcomes and may be dependent on the gap length from torn tendon that must be spanned by the repair. Detailed understanding of forces throughout the range of motion (ROM) may allow surgeons to make evidence-based recommendations for post-operative care. METHODS: We used a computational shoulder model to assess passive tension and total moment-generating capacity in supraspinatus for repairs of gaps up t

Open versus two forms of arthroscopic rotator cuff repair.

UNLABELLED: There have been technologic advances in the methods for repairing torn rotator cuffs. We compared the clinical and structural outcomes of three different forms of rotator cuff repair with up to 24 months' followup. We wished to assess how surgical technique affected clinical outcomes and see how these correlated to repair integrity. Three cohorts of patients had repair of a symptomatic rotator cuff tear using (1) an open technique (n = 49); (2) arthroscopic knotted (n = 53); or (3) arthroscopic knotless (n = 57) by one surgeon. Standardized patient- and examiner-determined outcomes

The level of vitamin D in the serum correlates with fatty degeneration of the muscles of the rotator cuff.

This study examined the role of vitamin D as a factor accounting for fatty degeneration and muscle function in the rotator cuff. There were 366 patients with disorders of the shoulder. A total of 228 patients had a full-thickness tear (group 1) and 138 patients had no tear (group 2). All underwent magnetic resonance arthrography and an isokinetic muscle performance test. The serum concentrations of vitamin D (25(OH)D(3)) were measured. In general, a lower serum level of vitamin D was related to higher fatty degeneration in the muscles of the cuff. Spearman's correlation coefficients were 0.173

A comparison of prospective and retrospective assessment of functional outcome after rotator cuff repair.

Prospective outcome studies are generally considered to be better than retrospective studies. The purpose of this study was to assess correlations between prospective and retrospective outcome assessment after rotator cuff repair. One-hundred and twelve patients (118 shoulders) with chronic rotator cuff tears were evaluated at a mean of 54 months (34-85) after rotator cuff repair, using several outcome measures including a retrospective assessment of improvement. The retrospective assessment of post-operative pain, function, and quality of life had fair correlations with the prospectively dete

Total shoulder arthroplasty using the superior approach: influence on glenoid loosening and superior migration in the long-term follow-up after Neer II prosthesis installation.

Glenoid component loosening and superior humeral translation are common after Neer II total shoulder arthroplasty using the anterior approach. To determine whether the superior approach reduced these complications, we retrospectively reviewed 20 shoulders in 16 patients. Both components were cemented. Patient satisfaction, unweighted Constant score, and imaging studies were evaluated at a mean of 3.5 years and at a mean of 11.1 years. Fourteen patients were satisfied or very satisfied. The mean unweighted Constant score improved from 25/100 preoperatively to 57/100 after 3.5 years and to 51/10

Release of growth factors after arthroscopic acromioplasty.

It has recently been postulated that a variety of growth factors may be released from cancellous bone after an acromioplasty. The aim of this study was to demonstrate the presence of growth factors in the subacromial space after acromioplasty. Between October 2006 and March 2007, 23 patients underwent arthroscopic acromioplasty. A sample of at least 3 ml of fluid from the shoulder was obtained 15 min after the end of the procedure. At the same time another sample of 3 ml of the patient's venous blood was obtained as a control. The concentrations of growth factors in the fluids collected were d

Influence of tendon healing after arthroscopic rotator cuff repair on clinical outcome using single-row Mason-Allen suture technique: a prospective, MRI controlled study.

The reported functional results of rotator cuff repair performed arthroscopically have been good. Only little is known about the cuff integrity after arthroscopic repair and how it influences the outcome. The aim of the study is to set a baseline of what rate of healing response respectively re-tears to expect and how cuff integrity alters the outcome. Fifty-three consecutive patients with an isolated supraspinatus tendon tear were arthroscopically operated and their tendons repaired. All pre- and postoperative data were prospectively collected. At a minimum follow-up after 24 months (average

Reconstruction of large rotator cuff tendon defects with porcine small intestinal submucosa in an animal model.

Large rotator cuff tears represent a challenging problem. The purpose of our study was to evaluate the small intestinal submucosa (SIS) extracellular matrix in reconstruction of such defects in an animal model. Forty rats were equally divided into an SIS group (reconstruction of a large supraspinatus tendon defect by use of the SIS) and a defect group (no repair). The operative and contralateral normal shoulders underwent histologic evaluation and biomechanical testing at 6 and 16 weeks. Neovascularization and fibroblastic ingrowth were present in SIS-regenerated tendons, which had an ultimate

Proximal migration can be measured accurately on standardized anteroposterior shoulder radiographs.

UNLABELLED: During the last decade, investigators of several studies have stressed the importance of early diagnosis and treatment of rotator cuff tears to improve outcome of surgical repair. Proximal migration of the humeral head is thought to be indicative for rotator cuff disorders. We wanted to assess the accuracy of proximal migration measurement on anteroposterior radiographs. Computed tomography scans and anteroposterior radiographs of 43 shoulders in 26 patients were taken using the same protocol. Proximal migration was measured as the acromiohumeral interval and the upward migration i

Open rotator cuff repair without acromioplasty.

BACKGROUND: In most clinical reports on rotator cuff repair, acromioplasty was done as part of the procedure. In this prospective study, we evaluated the hypothesis that rotator cuff repair without acromioplasty would result in a substantial improvement in shoulder comfort and function. METHODS: Ninety-six consecutive primary repairs of full-thickness tears of the rotator cuff were performed through a deltoid-muscle-splitting incision that preserved the integrity of the coracoacromial arch and the deltoid insertion. All patients were invited to participate in a prospective study involving per

Subscapularis sparing approach vs. tenotomy of the subscapularis tendon in reverse shoulder arthroplasty: a prospective, randomized, double-blinded clinical trial.

PURPOSE: The management of the subscapularis tendon in reverse shoulder arthroplasty (RSA) is still discussed controversially. We hypothesized, that (1) patients treated with the subscapularis sparing approach would present with a higher internal rotational strength, while (2) the external rotational motion is not reduced. METHODS: A prospective, randomized, double blinded clinical trial was conducted. Patients were randomized into two groups: subscapularis sparing approach (group 1) or tenotomy without repair of the subscapularis tendon (group 2). Clinical follow-up examinations were perform

Early results of reverse shoulder arthroplasty in patients with rheumatoid arthritis.

BACKGROUND: Rheumatoid arthritis affecting the shoulder is typically associated with rotator cuff compromise and can also result in severe glenoid erosion. Since reverse shoulder arthroplasty is capable of addressing both rotator cuff disorders and glenoid bone deficiencies, our aim was to evaluate the outcome of reverse shoulder arthroplasty in patients with rheumatoid arthritis and either or both of these associated conditions. METHODS: We performed eighteen primary reverse total shoulder arthroplasties in sixteen patients with rheumatoid arthritis involving the shoulder as well as associat

[Ultrasonic diagnosis of tendon and ligament injuries in anterior dislocations of the shoulder joint].

The usefulness of ultrasonography in diagnosis of frequently occurring traumatic injuries of the ligament-tendon apparatus of the shoulder has been evaluated and compared with intra-operative nd contrast roentgenography examinations. An analysis of 102 cases, including 85 fresh anterior dislocation, 17 habitual anterior dislocation, has been made. 306 ultrasonographic examinations have been performed. A frequent coexistence of injuries of the tendon of the long head of the biceps muscle with an injury of the rotators cuff, mainly the supraspinous muscle, has been pointed out. The purposefulnes
