The Prevalence of Scapholunate Signal Abnormalities on Magnetic Resonance Imaging.

BACKGROUND: Although trauma is a major cause of symptomatic scapholunate interosseous ligament (SLIL) pathology, many patients do not recall a specific injury or repetitive trauma. We report on: (1) the prevalence of SLIL signal changes in patients who underwent wrist magnetic resonance imaging (MRI) for various indications; and (2) the prevalence of SLIL signal changes on MRI in patients without prior wrist trauma. METHODS: This is a retrospective study evaluating 1021 patients who underwent wrist MRI or magnetic resonance arthrogram. We collected data on SLIL signal changes on MRI. Patients

Weight-Bearing Computed Tomography Analysis of Scapholunate Ligament Injury.

PURPOSE: Weight-bearing computed tomography allows for assessment of carpal alignment and stability under physiologic load conditions, which may allow for detection of subtle dynamic instabilities that may not be apparent in nonweight-bearing images. The purpose of this study was to evaluate carpal position under physiologic load in healthy and scapholunate interosseous ligament (SLIL)-injured wrists using weight-bearing computed tomography. METHODS: Ten healthy controls and 10 participants with arthroscopy (Geisler III or IV) or magnetic resonance imaging-proven SLIL injury underwent compute

Reconstruction of an Early-Stage Scapholunate Advanced Collapse Wrist with the 3-Ligament Tenodesis Procedure: A Controversial Reappraisal.

BACKGROUND: The authors hypothesized that the 3-ligament tenodesis (3-LT) procedure is still sufficient-even in scapholunate advanced collapse (SLAC) cases-to reduce pain and improve wrist function. The authors compared patient-reported outcomes of scapholunate interosseus ligament (SLIL) injury patients with SLAC, to SLIL injury patients treated with 3-LT, and then to patients who underwent proximal row carpectomy (PRC), as a control group. METHODS: The authors included all patients with a traumatic SLIL injury and associated SLAC components treated with 3-LT and completed patient-rated wris

Scapholunate Ligament Reconstruction With Internal Brace Augmentation Techniques for Chronic Scapholunate Dissociation: A Clinical Follow-up Study.

INTRODUCTION: The scapholunate ligament is the most important stabilizer of the scapholunate articulation. The management of chronic irreversible injuries of this ligament in the absence of preexisting arthritis of the wrist joint remains controversial. Recently, surgeons introduced a novel surgical technique using an internal brace (IB). Several biomechanical studies on this technique have been conducted using cadavers; however, very few studies have discussed the results in detail in actual clinical practice. Therefore, herein, we investigated the radiological and functional results of patie

Mid-term outcomes of three commonly used surgical reconstructions for scapholunate instability.

This retrospective analysis reports the mid-term (>2 years) outcomes of capsulodesis, (modified) Brunelli tenodesis and bone-ligament-bone reconstruction for scapholunate ligament instability. In total, 60 patients (64 wrists) returned the Patient-Rated Wrist Evaluation and Disabilities of the Arm, Shoulder, and Hand questionnaires. Of these participants, 42 (46 wrists) returned for a wrist examination and radiographs. In the questionnaire group, the mean follow-up for capsulodesis was 11.8 years, tenodesis 5.9 years and bone-ligament-bone 8.9 years. In the objective outcomes group, the mean f

Association of scapholunate dissociation and two-part articular fractures of the distal radius.

Scapholunate dissociation may occur in association with distal radial fractures and is easily missed at initial presentation. The aim of this study was to examine variances in the scapholunate distance with respect to subtypes of two-part partial articular distal radial fractures. Axial computed tomography (CT) scans of acute two-part intra-articular radial fractures were assessed retrospectively from 80 patients and compared to 20 controls. From each scan, two images were analysed to identify the scaphoid, lunate and articular fracture line in the distal radius for fracture type categorizatio

Scapholunate kinematics of asymptomatic wrists in comparison with symptomatic contralateral wrists using four-dimensional CT examinations: initial clinical experience.

OBJECTIVES: Using four-dimensional CT scan (4DCT), we aimed at showing the kinematics of scapholunate (SL) interval in asymptomatic wrists in comparison with symptomatic contralateral wrists with inconclusive radiographic findings. METHODS: This is an IRB approved, HIPPA compliant, retrospective study. Patients suspected of SL interosseous ligament (SLIL) injuries were referred for further evaluation of chronic wrist pain (>3 months). Twelve wrists (11 subjects) with chronic symptoms and inconclusive plain radiographs and 10 asymptomatic wrists (in 10 different subjects) were scanned using 4D

Arthroscopic ligamentoplasty of the dorsal and volar portions of the scapholunate ligament.

Classical arthroscopic techniques for scapholunate instability consist of debridement, thermal shrinkage, and percutaneous pinning. Good results are obtained in acute lesions or in chronic partial tears, but they are less predictable when the lesion is complete, because of the poor healing capacity of the scapholunate ligament and because it is not possible to perform an anatomic ligamentous reconstruction with these techniques. Open techniques are thus required for reconstruction, but they damage the soft tissues. We recently published a description and cadaver study of an arthroscopic ligame

Management of suspected scaphoid fractures in accident and emergency departments--time for new guidelines.

INTRODUCTION: The objectives of this work were to assess the clinical knowledge of clinicians in the accident and emergency (A&E) departments in England & Wales and evaluate the current trend for the acute management of radiologically normal, but clinically suspected, fractures of the scaphoid. SUBJECTS AND METHODS: We conducted a telephone survey on 146 A&E senior house officers (SHOs) in 50 different hospitals. This survey assessed the clinicians' experience, their clinical and radiological diagnostic methods, and their initial treatment of suspected scaphoid fractures. RESULTS: The majori

[Translated article] Subtalar dislocations: Analysis of a case series. Our urgent treatment algorithm.

INTRODUCTION AND OBJECTIVES: Subtalar dislocations are a rare entity (<1%) in traumatic foot injuries. There is a loss of anatomical relationship between the talus, calcaneus and scaphoid. There are only small-published series. MATERIAL AND METHODS: We present a series of patients (N=13) with subtalar dislocations in which a descriptive analysis of the main epidemiological, clinical and radiological variables was carried out, from which an urgent treatment algorithm is proposed. Cases with fractures of the neck of the talus, body of the calcaneus, or isolated Chopart fracture-dislocations wer

Evaluating metallic artefact of biodegradable magnesium-based implants in magnetic resonance imaging.

Magnesium (Mg) implants have shown to cause image artefacts or distortions in magnetic resonance imaging (MRI). Yet, there is a lack of information on how the degradation of Mg-based implants influences the image quality of MRI examinations. In this study, Mg-based implants are analysed in vitro, ex vivo, and in the clinical setting for various magnetic field strengths with the aim to quantify metallic artefact behaviour. In vitro corroded Mg-based screws and a titanium (Ti) equivalent were imaged according to the ASTM F2119. Mg-based and Ti pins were also implanted into rat femurs for differe

Comparison of scapholunate distance measurements on plain radiography and computed tomography for the diagnosis of scapholunate instability associated with distal radius fracture.

PURPOSE: To investigate radiographic criteria for scapholunate instability (SLI) in the setting of distal radius fracture (DRF) confirmed by arthroscopy. METHODS: Eighty-eight wrists with DRF treated by open reduction and internal fixation and assessed for SLI arthroscopically were evaluated. The scapholunate distance (SLD) was measured by preoperative posteroanterior wrist radiography and computed tomography (CT). SLD on radiographs was measured as the distance between the scaphoid cortex and the lunate cortex at the center of the scapholunate joint. SLDs were measured at the volar end (A1),

Proximal row carpectomy: long-term results.

OBJECTIVES: To evaluate long-term clinical outcome of proximal carpectomy. Our assumption was that this intervention should result in long-term benefit, making a wrist painless, mobile, and functional, compatible with social and professional life, whatever the initial etiology of the degenerative wrist. METHODS: We report a continuous single centre retrospective series of 24 patients. Objective (mobility, strength, radiographic evaluation) and subjective (pain, subjective wrist value, functional scores) functions were assessed by an independent observer. Surgery was performed mainly for Kienb

Physeal-Sparing Medial Femoral Condyle Flap for Treatment of Scaphoid Nonunion With Avascular Necrosis in Skeletally Immature Patients: A Case Series With Long-Term Follow-Up.

BACKGROUND: Scaphoid nonunion is a rare but challenging condition in the pediatric population. While acute scaphoid fractures in children typically heal well with cast immobilization, nonunion can result in persistent pain and functional impairment. The medial femoral condyle (MFC) flap offers potential advantages for reconstruction by providing a robust blood supply and structural support. However, its application in skeletally immature patients is limited by concerns over damaging the open distal femoral physis. METHODS: We present 2 cases of skeletally immature patients with scaphoid nonun

Dorsal intercalated segmental instability associated with malunion of a reconstructed scaphoid.

UNLABELLED: We analysed scaphoid deformity as a result of surgical treatment of scaphoid fracture nonunion and assessed the deformity associated with a dorsal intercalated segmental instability pattern of carpal malalignment. A total of 45 patients who were treated for scaphoid fracture nonunion were included in the study. The height-to-length ratio of the scaphoid was measured on computed tomographic images and used to assess scaphoid deformity. Carpal malalignment was quantified based on the radio-lunate angle. A correlation analysis between the height-to-length ratio and the radio-lunate an

Wrist position affects loading of the dorsal scaphoid: possible effect on extrinsic scaphoid blood flow.

Cadaver studies using radial artery injection techniques were used to study the vascular supply along the dorsal ridge of the scaphoid. These revealed an intraarticular membrane between the wrist capsule and the dorsal ridge of the scaphoid through which arteriolar vessels (25-100 microm internal diameter) passed. Biomechanical tests revealed that the extensor carpi radialis brevis may apply significant pressure to the dorsal ridge of the scaphoid when the wrist is flexed. The highest pressures occurred with the wrist flexed at 60 degrees or 90 degrees and in slight (15 degrees ) ulnar deviati
