Out-of-Hospital Perimortem Cesarean Section as Resuscitative Hysterotomy in Maternal Posttraumatic Cardiac Arrest.

The optimal treatment of a severe hemodynamic instability from shock to cardiac arrest in late term pregnant women is subject to ongoing studies. However, there is an increasing evidence that early "separation" between the mother and the foetus may increase the restoration of the hemodynamic status and, in the cardiac arrest setting, it may raise the likelihood of a return of spontaneous circulation (ROSC) in the mother. This treatment, called Perimortem Cesarean Section (PMCS), is now termed as Resuscitative Hysterotomy (RH) to better address the issue of an early Cesarean section (C-section)

Children with lethal streptococcal fasciitis after a minor contusion injury.

Necrotizing fasciitis is a severe life-threatening soft tissue infection characterized by rapidly spreading necrosis of the fascia and the subcutaneous tissue. Mortality as a result of streptococcal necrotizing fasciitis has been associated with the presence of hypotension, streptococcal toxic shock syndrome, or bacteremia in the literature. These infections are rare in children, and the diagnosis should be considered in the presence of any soft tissue infection presenting with signs of toxicity and marked wound edema. In addition, traumatic compartment syndrome has also gained public attentio

Use of blood and blood products in trauma.

According to the global study of the burden of disease, violence and accidental injury account for 12% of deaths worldwide; 30-40% of trauma mortality is attributable to haemorrhage. The highly complex haemostatic system is severely impaired as a result of haemorrhagic shock, acidosis, hypothermia, haemodilution, hyperfibrinolysis, and consumption of clotting factors. Thus it is important to prioritize the prevention of the development of coagulopathy. Timely transfusion of red blood cells and plasma products becomes essential to restore tissue oxygenation, support perfusion, and maintain the

Pulmonary embolization by chorionic villi causing maternal death after a car crash.

Throughout the last century, there has been a marked decline in obstetric maternal deaths, resulting in an increase in the proportion of nonobstetric deaths among pregnant women. Trauma, in particular, has become a leading cause of maternal death. We report the case of a 20-year-old primigravid woman who was involved in a motor vehicle crash at 36 weeks gestation. The woman developed abruptio placentae, followed by disseminated intravascular coagulation, adult respiratory distress syndrome, and shock, and died the day after the crash. Widespread pulmonary embolization by chorionic villi was id

Induction of heme oxygenase-1 (HO-1) after traumatic brain injury in the rat.

The induction of heme oxygenase-1 (HO-1), the 32 kDa heat shock protein, was examined in the traumatized rat brain. At 24 h after either mild or severe brain injury or sham surgery, HO-1 was immunolocalized in fixed sections of brain. After mild brain injury, hemorrhage was apparent in the subarachnoid space, external capsule and cerebellum. HO-1 was induced in similar areas in macrophages in the subarachnoid space and in glia in the cortex adjacent to the site of impact, the ipsilateral hippocampus, external capsule and cerebellum. After severe brain injury, extensive hemorrhage occurred in t

Determinants of early adult respiratory distress syndrome. A retrospective study of 220 patients with major fractures.

The records of 220 consecutive trauma patients admitted to intensive care in the period 1974 through 1982 were reviewed in an attempt to find determinants of early adult respiratory distress syndrome (ARDS). All the patients were considered to be at risk of ARDS and had major fractures without concomitant severe injuries to brain, chest or abdomen. No patient died. ARDS developed in 27 patients (12.3%), on average in the second day post-trauma. The clinical determinants of post-traumatic ARDS were high fracture index, implying severe tissue trauma, and shock on admission. Fluid overload was no

The effects of observed sudden infant death syndrome (SIDS) on hospital staff.

We have described an observed case of SIDS in a hospital setting. The victim was "well baby" hospitalized for psychosocial reasons. Death occurred on the fifth hospital day, during sleep and two hours after feeding. The death was silent and was preceded by cyanosis and respiratory arrest. Hospital staff who attended the baby during his admission experienced the same traumatic reactions as families of SIDS victims, ie, shock, disbelief, anger, guilt, fear, blaming, sadness, and behavioral manifestations. Information on SIDS, communication about feelings and continued group support were of utmos

Development and Characterization of a Rat Model of Blast Polytrauma and Hemorrhagic Shock for Evaluating Innate Immunotherapies During Prolonged Damage Control Resuscitation.

BACKGROUND: A major challenge in developing effective immunological damage-control therapies for traumatic hemorrhage (TH) is the lack of animal models that accurately reproduce the immune and pathophysiological responses observed in humans. In this study, we established a clinically relevant rat model that combines blast injury with hemorrhagic shock in a simulated prolonged damage control resuscitation environment. METHODS: Male Sprague Dawley rats were anesthetized and subjected to moderate blast overpressure, followed by controlled hemorrhage equivalent to 40% of the estimated total blood

Primary biomarkers of hemorrhagic shock, traumatic brain injury, lung and kidney pathology were also negative for aeromedical evacuation effects. In summary, the medical benefits associated with immediate (i.e., within a few hours of injury) helicopter evacuation of severe polytrauma patients likely …

Total pancreas-preserving pancreatic injury-site jejunostomy for traumatic pancreatic head injury with main pancreatic duct transection: feasibility and decision algorithm.

BACKGROUND: Traumatic pancreatic head injury with complete main pancreatic duct (MPD) transection is rare and associated with high morbidity. For the American Association for the Surgery of Trauma Organ Injury Scale (AAST-OIS) Grade IV injuries, pancreaticoduodenectomy is often considered the only definitive option; however, its invasiveness and risk of long-term endocrine and exocrine dysfunction create a major dilemma when pancreaticoduodenectomy may be disproportionate to the anatomic injury. Practical pancreas-preserving strategies remain poorly defined. CASE PRESENTATION: A 25-year-old m

Trauma-associated extracellular histones mediate inflammation via a MYD88-IRAK1-ERK signaling axis and induce lytic cell death in human adipocytes.

Despite advances in the treatment and care of severe physical injuries, trauma remains one of the main reasons for disability-adjusted life years worldwide. Trauma patients often suffer from disturbances in energy utilization and metabolic dysfunction, including hyperglycemia and increased insulin resistance. White adipose tissue plays an essential role in the regulation of energy homeostasis and is frequently implicated in traumatic injury due to its ubiquitous body distribution but remains poorly studied. Initial triggers of the trauma response are mainly damage-associated molecular patterns

Determining the top research priorities in UK prehospital critical care: a modified Delphi study.

BACKGROUND: Prehospital critical care is a rapidly evolving field. There is a paucity of evidence relating to its practice, with limited progress in answering those research questions identified over a decade ago. It is vital that evidence gaps are identified and addressed. This study aimed to define the current research priorities in UK prehospital critical care. METHODS: This modified national Delphi study was coordinated by the Pre-HOspital Trainee Operated research Network and conducted in four rounds between October 2021 and April 2022. Rounds 1 and 2 were conducted online with clinician

Enhanced fear memories and brain glucose metabolism (18F-FDG-PET) following sub-anesthetic intravenous ketamine infusion in Sprague-Dawley rats.

Ketamine is a multimodal dissociative anesthetic, which provides powerful analgesia for victims with traumatic injury. However, the impact of ketamine administration in the peri-trauma period on the development of post-traumatic stress disorder (PTSD) remains controversial. Moreover, there is a major gap between preclinical and clinical studies because they utilize different doses and routes of ketamine administration. Here, we investigated the effects of sub-anesthetic doses of intravenous (IV) ketamine infusion on fear memory and brain glucose metabolism (BGluM) in rats. Male Sprague-Dawley

Chronic traumatic encephalopathy in blast-exposed military veterans and a blast neurotrauma mouse model.

Blast exposure is associated with traumatic brain injury (TBI), neuropsychiatric symptoms, and long-term cognitive disability. We examined a case series of postmortem brains from U.S. military veterans exposed to blast and/or concussive injury. We found evidence of chronic traumatic encephalopathy (CTE), a tau protein-linked neurodegenerative disease, that was similar to the CTE neuropathology observed in young amateur American football players and a professional wrestler with histories of concussive injuries. We developed a blast neurotrauma mouse model that recapitulated CTE-linked neuropath

Non-traumatic Tension Faecopneumothorax: A Rare Complication of Intrathoracic Colonic Perforation.

Tension faecopneumothorax is an exceptionally rare and life-threatening condition, typically resulting from bowel herniation through the diaphragm with intrathoracic perforation. Although diaphragmatic hernias can be congenital, traumatic, or iatrogenic, colonic herniation and subsequent perforation into the thoracic cavity are exceedingly uncommon and diagnostically challenging. We report the case of a 53-year-old woman with a history of type 2 diabetes mellitus and a recent laparoscopic cholecystectomy, admitted to the emergency department (ED) with severe respiratory distress. Despite multi

Decision-making regarding primary prevention implantable cardioverter-defibrillators among older adults.

Most implantable cardioverter defibrillators (ICDs) are implanted for the purpose of primary prevention of sudden cardiac death among older patients with heart failure with reduced ejection fraction. Shared decision-making prior to device implantation is guideline-recommended and payer-mandated. This article summarizes patient and provider attitudes toward device placement, device efficacy and effectiveness, potential periprocedural complications, long-term events such as shocks, quality of life, costs, and shared decision-making principles and recommendations. Most patients eligible for an IC

Long-lasting marked inhibition of periaqueductal gray-evoked defensive behaviors in inescapably-shocked rats.

Clinical evidence suggests that depression and trauma predispose the subject to panic. Accordingly, here we examined the late effects of uncontrollable stress, a presumptive model of depression and/or traumatic disorder, on panic-like behaviors evoked by electrical stimulation of the dorsal periaqueductal gray (DPAG). Changes in anxiety and depression were also assessed in the elevated plus-maze (EPM) and forced-swimming test (FST), respectively. Rats with electrodes in the DPAG were subjected to a 7-day shuttle-box one-way escape yoked training with foot-shocks either escapable (ES) or inesca
