SMARCB1/INI1-deficient medullary-like renal cell carcinoma (MLRCC), also used to be known as unclassified renal cell carcinoma with medullary phenotype (RCCU-MP), is an extremely rare and highly aggressive kidney tumor. The 2022 World Health Organization (WHO) classification proposes that high-grade renal cell carcinomas exhibiting morphological and immunophenotypic features identical to renal medullary carcinoma, but without evidence of sickle cell trait or disease, should be diagnosed as SMARCB1/INI1-deficient medullary-like renal cell carcinoma(MLRCC) and classified as a subtype of SMARCB1-…

Corticosteroid therapy is a known risk factor for osteonecrosis, more commonly with chronic use and high cumulative dose. Osteonecrosis (avascular necrosis) has been described in pregnancy involving primarily the femoral head. To our knowledge, only rare cases of femoral meta diaphysis or knee osteonecrosis in pregnancy have been documented in the literature. We report a 28-year-old woman with sickle cell trait and beta-thalassemia trait who developed severe bilateral knee pain shortly after corticosteroid therapy. She was 34-weeks pregnant when she presented with the signs of preterm labor an…

OBJECTIVES: Significantly low serum levels of complement components have been reported in patients with Sickle cell anaemia (SCA) which may result in a high susceptibility to infections. However some studies have reported a normal or high complement level. The aim of our study was to assess some haematological parameters and Complement levels in adult patients with steady state SCA in Zaria, North western Nigeria. This will serve as a guide towards infection surveillance in the quest to reduce significant morbidity and mortality in our setting.
METHODS: A cross sectional study of 40 patients

Prevention of sickle cell anemia, a severe and painful disease, relies on carrier detection and information, and on prenatal diagnosis with the possibility of medical abortion. Here we report the first results of a medical and welfare program launched in Paris 20 months ago and dedicated to sickle cell carrier screening and information.

Disease and mortality rates for Streptococcus pneumoniae infections are much higher in patients with sickle cell disease (SCD) than in age-matched patients without SCD. Pneumococcal surface protein A (PspA) has been proposed as a component in human vaccines against S. pneumoniae to provide greater breadth of coverage than can be obtained with the 7-valent conjugate vaccine. The cross-reactivity of PspA is associated with the 'PspA family' structure. In this study we examined strains of S. pneumoniae from patients with and without SCD to determine whether the strains infecting the hypersuscepti

Pituitary gland dysfunction and its contribution to menarcheal delay in sickle cell anaemia patients was investigated. Ten SS patients mean age 17.5 years who had not achieved menarche were recruited and 10 each of AS and AA controls, mean ages 17.4 and 17.7 years were used as controls to study the effect of the heterozygous state. Dynamic studies with LHRH and TRH were performed for 60 minutes and LH, FSH, PRL and TSH assays were done. Median basal values were significantly lower in the SS patients compared with the AS and AA controls for LH, FSH and PRL. LH: 3.0; 7.1; 7.7 U/L, FSH: 2.1: 4.3:

OBJECTIVE: Despite penicillin prophylaxis and vaccination, infection with encapsulated organisms remains a leading cause of morbidity and death in children with sickle cell disease. The role of Fc receptors in the clearance of encapsulated organisms is well documented. The His(H)-Arg(R) polymorphism at amino acid 131 of the Fc gamma RIIA receptor alters binding affinity for human IgG2 and influences infection with encapsulated organisms in children without sickle cell disease. We hypothesized that the genotype for high-affinity human IgG2 binding (H/H131) is underrepresented in children with s

Renal medullary carcinoma (RMC) is a rare, aggressive disease that predominantly afflicts individuals of African or Mediterranean descent with sickle cell trait. RMC comprises 1% of all renal cell carcinoma diagnoses with a median overall survival of 13 months. Patients are typically young (median age-22) and male (male:female ratio of 2:1) and tumors are characterized by complete loss of expression of the SMARCB1 tumor suppressor protein. Due to the low incidence of RMC and the disease's aggressiveness, treatment decisions are often based on case reports. Thus, it is critical to develop precl…

The evolution of the preparticipation physical examination (PPE) in the United States continues to advance. In May 2010, the fourth edition of the Preparticipation Physical Examination Evaluation (PPE-4) monograph was published. The monograph is a product reflecting the collaborative efforts of 6 author societies. This article provides a brief historical review of the PPE and then highlights the recent changes and updates contained in the PPE-4 monograph, including cardiovascular screening in athletes. New recommendations to include the PPE in all well-child care visits and the need to develop

In patients with sickle cell anemia, fetal hemoglobin (HbF) concentrations vary by 2 orders of magnitude. This variance may be a result of heterogeneity in gene regulatory elements; accordingly, we searched for single nucleotide polymorphisms (SNPs) that might identify this variation. More than 180 SNPs were studied in 38 genes in 280 sickle cell anemia patients. The strongest association with HbF was found with SNPs near a QTL previously localized on chromosome 6q22.3-q23.2. Initially, two SNPs were identified in intergenic portions of this QTL and were associated with about a 20% difference

The mechanisms by which the hemoglobin genotype AS protect against severe malaria are not fully understood. We have investigated the possibility that protection might be achieved through an inability of red blood cells (RBC) with the AS genotype to form rosettes with RBC infected by Plasmodium falciparum. No evidence was obtained to support this hypothesis because RBC with the AS genotype formed rosettes with wild isolates of P. falciparum as readily as RBC with the AA genotype. However, the previous finding that parasitized RBC form rosettes more readily with RBC belonging to group A or B tha

Several nutritional surveys have shown that African-American children have lower mean hematocrit and hemoglobin values than white children. While the high prevalence of both alpha and beta thalassemias in African-American children can partly account for the lower hematologic values, the remaining differences cannot be explained. African-American individuals have a high prevalence of traits for hemoglobin S (HbAS) and hemoglobin C (HbAC). The mean hematocrit of children with normal hemoglobin AA (HbAA) (n = 15637) was compared with that of children with traits for HbAS (n = 1197) and HbAC (n =

OBJECTIVE: To assess compliance with oral penicillin prophylaxis in children with sickle cell disease and identify possible reasons for poor compliance.
DESIGN: Closed questionnaires given to parents of children with sickle cell disease and general practitioners in Brent. Urine samples from 23 children were tested for penicillin.
SETTING: Paediatric haematology clinic, Central Middlesex Hospital, and general practices in Brent.
SUBJECTS: 50 children (aged less than or equal to 16) attending clinic with sickle cell disease over six months (33 HbSS, 12 HbSC, five HbS beta thalassaemia). 30 ge

Sickle-cell disease is a well-recognized clinical entity. The pathophysiology of this hemoglobinopathy has been described in detail by numerous investigators since the first case report appeared in 1910. Orthopaedic manifestations of sickle-cell disease account for much of the morbidity associated with this disorder, including pain, osteonecrosis, arthritis, and sepsis. Effective management of these bone and joint sequelae reflect accurate diagnosis, understanding of this disorder's pathophysiology, and knowledge of available medical and surgical treatment alternatives. In this review, the aut

Massive nontraumatic hematuria threatens the life of the patient and presents a complex management problem for the clinician. A rational approach to the condition demands rapid initial assessment followed by specific diagnosis of the cause of bleeding and appropriate corrective steps.
