Endoscopic bile duct stenting as preliminary treatment of complicated choledochal malformation before definitive surgery.

OBJECTIVES: Acute pancreatitis or obstruction is considered an indication for urgent surgery in patients with choledochal malformation. However, in this inflammatory stage, surgery can be challenging. Our approach is to treat the acute obstruction by endoscopic retrograde cholangiopancreatography (ERCP), papillotomy, and placement of a bile duct stent. Subsequently, surgical resection of choledochal malformation is performed after the inflammation has subsided. METHODS: In this retrospective study, we included all patients who underwent surgical resection in our department for choledochal mal

Clinical outcomes of endoscopic papillectomy of ampullary adenoma: A multi-center study.

BACKGROUND: Ampullary adenoma is a rare premalignant lesion, but its incidence is increasing. Endoscopic papillectomy has become the first treatment of choice for ampullary adenomas due to its safety and effectiveness, thereby replacing surgical resection. However, recurrence rates and adverse events after endoscopic papillectomy were reported in up to 30% of cases. AIM: To review the long-term outcomes of endoscopic papillectomy and investigate the factors that affect these outcomes. METHODS: We retrospectively analyzed the data of patients who underwent endoscopic papillectomy for ampullar

Novel method for evaluating the indication for endoscopic papillectomy in patients with ampullary adenocarcinoma.

This study aimed to determine the clinicopathological features of the subtypes of ampullary carcinoma (AC) to explore the indications for endoscopic papillectomy (EP) in early AC. Fifty-seven patients with AC who underwent curative resection were retrospectively reviewed. The 0/IA stages were significantly more common in the intestinal type (I-type) than in the mixed and pancreatobiliary type (M&PB-type) (90.7% vs 35.7%, P < 0.001). Tis/T1a tumors limited to the ampulla [Tis/T1a(ampulla)] were significantly more likely to be I-type than M&PB-type (74.4% vs 14.3%, P = 0.002). The tub1 rate was

Predictive factors for extraction of common bile duct stones during endoscopic retrograde cholangiopancreatography in Billroth II anatomy patients.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) for extraction of common bile duct (CBD) stones in Billroth II anatomy patients is still a technical challenge and factors affecting stone extraction have not yet been clarified. This study aimed to analyze our experience and evaluate potential factors affecting CBD stone extraction. METHODS: A retrospective analysis of CBD stones patients with a history of Billroth II gastrectomy, who underwent therapeutic ERCP for stone extraction at our center from August 1999 to December 2017, was conducted. The outcomes of ERCP and potenti

Self-expandable metal stents for choledocholithiasis in Billroth II gastrectomy patients.

BACKGROUND: Efficient ampullary intervention is essential for endoscopic retrograde cholangiopancreatography (ERCP) in patients with a prior Billroth II gastrectomy. We retrospectively evaluated the safety and effectiveness of ampullary intervention using fully covered self-expandable metal stents (FCSEMSs) for the management of common bile duct (CBD) stones in a subset of patients with a history of Billroth II gastrectomy. METHODS: This retrospective analysis involved patients with a prior Billroth II gastrectomy who underwent ampullary intervention with FCSEMSs for the management of CBD sto

A case of plummer-vinson syndrome associated with Crohn's disease.

Plummer-Vinson syndrome manifests as cervical dysphagia, iron deficiency anemia, an upper esophageal web, and atrophic glossitis. The cause of the esophageal web is thought to be iron deficiency anemia; however, the cause of Plummer-Vinson syndrome has not been established. Crohn's disease is usually accompanied by malnutrition and iron deficiency anemia; however, no case of concomitant Crohn's disease and Plummer-Vinson syndrome with aggravated malnutrition and anemia has been previously reported. Here, we report on a rare case of Plummer-Vinson syndrome in a Crohn's disease patient, which ca

Meta-analysis comparison of endoscopic papillary balloon dilatation and endoscopic sphincteropapillotomy.

AIM: To assess endoscopic papillary balloon dilatation (EPBD) and endoscopic sphincteropapillotomy (EST) for common bile duct (CBD) stone removal using a meta-analysis. METHODS: Randomized controlled trials published from 1990 to 2012 comparing EPBD with EST for CBD stone removal were evaluated. This meta-analysis was performed to estimate short-term and long-term complications of these two treatments. The fixed random effect model or random effect model was established to analysis the data. Results were obtained by analyzing the relative risk, odds ratio, and 95%CI for a given comparison usi

A new guidewire cannulation technique in ERCP: successful deep biliary access with triple-lumen sphincterotome and guidewire controlled by the endoscopist.

BACKGROUND: Current studies have addressed ways to improve the success of selective biliary cannulation in endoscopic retrograde cholangiopancreatography (ERCP). The objective of this study was to assess the efficacy of deep bile duct access using a short-wire system with sphincterotome and guidewire controlled only by the endoscopist. METHODS: This was a prospective study of 70 patients with biliary diseases subjected to ERCP. Biliary cannulation was performed by the endoscopist without direct cooperation of the assistant in two centers with different experience in ERCP. The RX Biliary Syste

[Carcinoma of the major duodenal papilla].

An experience with treatment of 62 patients with carcinoma of the major duodenal papilla during the period of 1989-1999 is generalized. Mean age of the patients was 62.4. Most of the patients admitted (85%) had jaundice. The most informative method of examination was FGDS, its sensitivity was 84%. All the patients were operated on. Transduodenal papillectomies (TDPE) were performed in 10 patients, pancreatoduodenal resection (PDR) in different modifications--in 42 patients, in 2 of them for recurrent carcinoma after TDPE, biliodigestive anastomoses--in 13 patients. Resectability was 80%, posto

Long-term follow-up of a prospective randomized study of endoscopic versus surgical treatment of bile duct calculi in patients with gallbladder in situ.

Eighty-three patients with bile duct calculi were entered in a prospective randomized study of endoscopic sphincterotomy (ES) and stone removal (group 1) versus surgery alone (group 2), and were followed for more than 5 years. In group 1 endoscopic stone clearance was successful in 35 of 39 patients. Thirteen patients subsequently had cholecystectomy with (n = 7) or without (n = 6) biliary symptoms and one had a cholecystostomy for acute cholecystitis. Two patients have had mild biliary colic or pancreatitis. Two patients died from gallbladder carcinoma after 9 days and 18 months. In group 2 b…

Pancreas divisum. Diagnosis, clinical significance, and management alternatives.

Pancreas divisum patients make up a small but problematic portion of ERCP cases. Minor papilla cannulation techniques have been refined. Recurrent pancreatitis patients generally will benefit from minor papilla therapy. Methods to select patients who are likely to respond to invasive therapy and further study need validation. Clinicians and endoscopists are strongly encouraged to be cautious and conservative with this patient group until stronger data indicate optimal management schemes.

[Wernicke-Korsakoff syndrome caused by prolonged infusion therapy during the postoperative period].

Wernicke-Korsakoff's syndrome (W.K.S.) is a complication of alcoholism and malnutrition and usually presents acutely and is characterized by disturbances of consciousness, paralysis of the external ocular muscles, ataxia and disorder of retentive memory. The disease results from deficiency of vitamin B1, or thiamine, an essential coenzyme in intermediate carbohydrate metabolism. We report a seriously ill, nonalcoholic surgical patient, who developed W.K.S. in the postoperative period as a result of thiamine deficiency, during prolonged intravenous therapy. It is recommended that malnourished p
