The possible causes of wrist pain in a patient with normal roentgenograms are many, and the above discussion of differential diagnosis is by no means intended to be all inclusive. Rather, my intent has been to provide a checklist of some of the more likely causes as well as a systematic approach to the identification of the cause. Many of the tests described are somewhat costly (Table 31-3), and they should not be ordered indiscriminately. Rather, the need for each test is determined by a reasonable presumptive diagnosis, depending on the history, clinical examination, and routine roentgenogra

Long-standing lateral ligament instability of the ankle results in unbalanced loading of the medial joint space and the possible development of degenerative arthritis. Thirty-six patients who had had lateral ankle instability for at least ten years and complaints of increasing ankle pain evidence degenerative changes of the articular cartilage over the medial half of the talar and tibial surfaces of the ankle joint. These changes were minimally apparent on roentgenograms unless weight-bearing roentgenograms were made, but arthroscopy of the joint accurately revealed the extent of degeneration.

CONTEXT: Limited longitudinal data exist on patient-reported outcomes (PROs) after acute lateral ankle sprain (LAS). The impact of prospective hop testing on PROs at return to play (RTP) and 6 months post-RTP is unclear.
OBJECTIVE: To determine if high school athletes with an LAS who return to baseline physical function as measured by a single-leg hop for distance (SLHOP) have better PROs relative to individuals who return to symmetry.
DESIGN: Cohort study.
SETTING: Ten high schools over 2 years.
PATIENTS OR OTHER PARTICIPANTS: Two hundred six high school athletes who sustained an LAS were

CONTEXT: Approximately 72% of patients with an ankle sprain report residual symptoms 6 to 18 months later. Although 44% of patients return to activity in less than 24 hours after experiencing a sprain, residual symptoms should be evaluated in the long term to determine if deficits exist. These residual symptoms may be due to the quality of ligament tissue and motion after injury.
OBJECTIVE: To compare mechanical laxity of the talocrural joint and dorsiflexion range of motion (DFROM) over time (24 to 72 hours, 2 to 4 weeks, and 6 months) after an acute lateral ankle sprain (LAS).
DESIGN: Cros

PURPOSE: The open "Broström-Gould" procedure has become the gold standard technique for the treatment of chronic ankle instability. Although arthroscopic techniques treating ankle instability have significantly evolved in the last years, no all arthroscopic Broström-Gould has been described. The aim of the study was to describe the all-arthroscopic Broström-Gould technique [anterior talofibular ligament (ATFL) repair with biological augmentation using the inferior extensor retinaculum (IER)], and to evaluate the clinical results in a group of patients.

Introduction: Due to the continual increase in the number of children engaging in sports today, physicians encounter finger injuries at an increasing frequency. This study sought to investigate the effectiveness of the method of Kinesio taping versus classic finger splint technique on pediatric patients with PIP (proximal interphalangeal) joint sprains of the fingers. Method: This is a retrospective cohort study. Forty-nine pediatric patients with PIP joint sprains were included in the study. The patients were divided into two groups, Group 1 being those treated with Kinesio taping and Group 2

Although landing in a plantarflexion and inversion position is a well-known characteristic of lateral ankle sprains, the associated kinematics of the knee and hip is largely unknown. Therefore, the purpose of this study was to examine the changes in knee and hip kinematics during landings on an altered landing surface of combined plantarflexion and inversion. Participants performed five drop landings from 30 cm onto a trapdoor platform in three different conditions: flat landing surface, 25° inversion, or a combined 25° plantarflexion and 25° inversion. Kinematic data were collected using a se

CONTEXT: The mechanisms that contribute to the development of chronic ankle instability are not understood. Investigators have developed a hypothetical model in which neuromuscular alterations that stem from damaged ankle ligaments are thought to affect periarticular and proximal muscle activity. However, the retrospective nature of these studies does not allow a causal link to be established.
OBJECTIVE: To assess temporal alterations in the activity of 2 periarticular muscles of the rat ankle and 2 proximal muscles of the rat hind limb after an ankle sprain.

PURPOSE: To investigate grade II syndesmosis injuries in athletes and identify factors important in differentiating stable from dynamically unstable ankle sprains and those associated with a longer time to return to sports.
METHODS: Sixty-four athletes with an isolated syndesmosis injury (without fracture) were prospectively assessed, with a mean follow-up period of 37 months (range, 24 to 66 months). Those with an associated deltoid ligament injury or osteochondral lesion were included. Those whose injuries were considered stable (grade IIa) were treated conservatively with a boot and rehabi

INTRODUCTION: Patients presenting to emergency departments (ED) for injuries resulting from recreational activities represent a unique source of information on important directions for injury prevention efforts. We describe the epidemiology of non-motorized wheeled activity-related injury in pediatric patients presenting to Canadian EDs as well as patients' helmet use.
METHODS: Data for the years 2004 to 2009 were abstracted from the Canadian Hospitals Injury Reporting and Prevention Program (CHIRPP), a national ED injury surveillance program in fifteen hospitals.
RESULTS: Most of the 28 618

BACKGROUND: Ankle joint injuries are extremely common sports injuries, with the anterior talofibular ligament involved in the majority of ankle sprains. There have been only a few large magnetic resonance imaging (MRI) studies on associated structural injuries after ankle sprains.
PURPOSE: To describe the injury pattern in athletes who were referred to MRI for the assessment of an acute ankle sprain and to assess the risk of associated traumatic tissue damage including lateral and syndesmotic ligament involvement.
STUDY DESIGN: Cross-sectional study; Level of evidence, 3.

BACKGROUND: Adequate hip range of motion is required for the transfer of energy from the lower to the upper extremity along the kinetic chain. Repetitive rotational stresses in the lower extremities during tennis may lead to sport-specific range of motion adaptations, which may increase the risk of injury to other joints along the kinetic chain.
PURPOSE: To assess whether such range of motion adaptations occur in the hip, and if so, to identify whether they are associated with injury.
STUDY DESIGN: Cross-sectional study; Level of evidence, 3.
METHODS: A total of 125 female professional tenn

Excessive abnormal strain or tension on the posterior tibial nerve in feet exhibiting talotarsal instability has been considered one of the possible etiologic factors of tarsal tunnel syndrome. The suggested treatment options in such cases include stabilization of the talotarsal joint complex in a corrected position, which might help minimize the abnormal forces placed on the posterior tibial nerve due to over stretching. The primary goal of this study was to quantify strain on the posterior tibial nerve in feet exhibiting hyperpronation caused by talotarsal instability, before and after an ex

BACKGROUND: The most common ankle injury is the lateral ankle sprain. Dutch annual sports-related ankle sprain costs can roughly be estimated at €187,200,000. Research has shown that proprioceptive training accounts for an approximated overall 50% reduction in ankle sprain recurrence rate.
HYPOTHESIS: An unsupervised proprioceptive training program to reduce the recurrence of lateral ankle sprains will reduce overall health care costs.
DESIGN: Cohort study (economic analysis); Level of evidence, 2.
METHODS: The study included 522 male and female athletes: 256 athletes (120 female and 136 ma

The time evolution of the postural behavior of 23 lateral ankle sprain patients (degrees I and II) were evaluated 14 h and 10 and 30 days on average after their injury and compared with those of 30 age-matched healthy individuals. The patients were tested with separate measurements of the reaction forces under each limb to highlight the possible compensatory mechanisms between the sound and the injured legs. Their postural behavior in bipedal stance was characterised by a weight-bearing asymmetry with more weight on the sound leg and an asymmetry of the postural stabilisation mechanisms, which

The authors report a case of bilateral C4-C5 facet fracture dislocation associated with a severe sprain underlying C5-C6, which had occurred during an traffic accident. The diagnosis of severe sprain was raised on the 55 th day. The injury mechanism is studied. Contiguous multilevel injuries of the lower cervical spine should be suspected in case of high-energy trauma. MRI can provide an exhaust if diagnosis of possible multilevel injuries. After fixation of the obvious lesion, intraoperative dynamic fluoroscopy must be performed to demonstrate any instability in another area.
