Strongyloides hyperinfection syndrome in an immunocompetent host resulting in bandemia and death.

Strongyloides stercoralis infection is usually asymptomatic but can result in a hyperinfection syndrome, most commonly triggered by acquired or iatrogenic immunosuppression. Here, we present a case of a man aged 60 years originally from a strongyloides endemic area with a medical history of alcohol abuse who presents with strongyloides hyperinfection syndrome (SHS) complicated by partial small bowel obstruction, pulmonary haemorrhage, large bandemia without eosinophilia and cardiac arrest resulting in death. This case is notable for the presence of bandemia and absence of eosinophilia, lack of

[Chronic infections in Latin American immigrants in Switzerland].

Recently arrived immigrants from Latin American countries represent a growing population in Switzerland. This article reviews the evidences on the most frequent chronic infections affecting this population group in Switzerland and presents screening recommendations adapted to primary care practice. Efforts should particularly focus on screening for Trypanosoma cruzi and Strongyloides stercoralis, especially in groups at higher risk of complication or transmission.

Strongyloides stercoralis hyperinfection syndrome in patients on prolonged steroid treatment: two case reports.

Two cases of Strongyloides hyperinfection have been reported who were on prolonged steroid therapy for frequent exacerbations of respiratory problems. One patient presenting with acute respiratory distress, rapidly deteriorated, did not give any time for management and died whereas the second patient survived because of early diagnosis of hyperinfection due to Strongyloides stercoralis.

Nonresponsive celiac disease due to Strongyloides stercoralis infestation.

Celiac disease (CD) is treated by eliminating all gluten from the diet. A 49-year-old man with CD was admitted to our clinic with complaints of recurrent diarrhea and abdominal pain despite strict adherence to a gluten-free diet. The duodenum was seen to be edematous on gastroduodenoscopic examination. Histological examination of the biopsy specimen taken from the duodenum showed multiple round shaped Strongyloides stercoralis (S. stercoralis) larvae within the crypts. He was successfully treated with albendazole. This case emphasizes the importance of duodenal biopsy in CD. To our knowledge,

Strongyloides stercoralis excretory/secretory protein strongylastacin specifically recognized by IgE antibodies in infected human sera.

The infective, microscopic Strongyloides stercoralis larvae in contaminated soil can penetrate human skin with the help of excretory/secretory proteases. These proteases play a critical role in infection and transmigration of the parasite to the intestines. Strongylastacin is similar to astacin (from the digestive gland of the crayfish Astacus astacus), a multi-domain protein with a signal peptide, a pro-enzyme, a catalytic domain containing the zinc binding consensus astacin family signature sequence HEXXHXXGFXHEXXRXDR, and a second conserved zinc binding motif SIMHY at N- terminal region. Strongyloides hyperinfection: a treatment dilemma.

Strongyloides stercoralis infection is a common cause of abdominal pain and diarrhea worldwide. Usually a chronic and limited disease, it can present a therapeutic dilemma when infection is overwhelming, such as what might occur in an immunosuppressed patient. Here we present a case of strongyloides hyperinfection treated successfully with a veterinary formulation of parenteral ivermectin.

Strongyloides stercoralis infection in a Finnish kennel.

BACKGROUND: Intestinal threadworm Strongyloides stercoralis is a parasite of dog, cat and primates that occurs worldwide being most prevalent in tropical and subtropical countries. The adult parasitic worm is about 2 mm long and slender. It possesses both parasitic and free-living lifecycles. The parasitic worms are females. Strongyloides stercoralis infects the host via percutaneous, peroral or transmammary transmission in addition to autoinfection. Clinical disease varies from inapparent to severe enteritis and pneumonia. The diagnosis is based on demonstration of larvae in fresh faeces, whi

Larval cyathostominosis in horses in Ontario: an emerging disease?

From 1991 to 2003, 24 cases of larval cyathostominosis were diagnosed at postmortem in equids, 15 (63%) from 2001 to 2003. Cases occurred from September to May, the majority from October to December. Median age was 12 mo. Diarrhea, colic, and weight loss were common clinical signs. Hypoalbuminemia and microcytosis were consistent findings.

A 34-year-old man with chronic itching and peripheral and submucosal eosinophilia.

A 34-year-old man with peripheral eosinophilia, chronic pruritus, and colonic eosinophilic infiltration is presented as a patient-oriented problem-solving case report to show the important aspects of differential and specific diagnosis, treatment, prognosis, and caveats in the approach to the workup of the patient with eosinophilia. Allergic rhinitis, asthma, atopy, and drug-induced eosinophilia should come to mind in the initial differential diagnosis of any patient with peripheral eosinophilia. Also included in the differential after allergic disease processes would be the general categories

In vitro antiparasitic activity of extracts of Cardiospermum halicacabum against third-stage larvae of Strongyloides stercoralis.

Extracts of Cardiospermum halicacabum, medicinal plant, were tested in vitro for their effectiveness against third-stage larvae of Strongyloidesstercoralis. Third-stage larvae of S. stercoralis were isolated from cultures of dog's feces using agar plate culture method. The larvae (1,000 larvae/ml), suspended in phosphate buffer saline solution, pH 7.4, were exposed to aqueous and alcohol extracts (2,000 microg/ml) of C. halicacabum at 37 degrees C with 5% CO2. Ivermectin (250 microg/ml) and piperazine (2,000 microg/ml) were also used as the reference drugs. The survival of Strongyloides larvae

Liquid chromatographic assay of ivermectin in human plasma for application to clinical pharmacokinetic studies.

There is a need for an accurate, sensitive and selective high-performance liquid chromatography (HPLC) method for the quantitation of iverme … ectin in healthy volunteers and to the analysis of plasma specimens from patients with disseminated strongyloidiasis.

Host immune responses are necessary for density dependence in nematode infections.

Nematode infections are subject to density-dependent effects on their establishment, survivorship and fecundity within a host. These effects act to regulate and stabilize the size of nematode populations. Understanding how these density-dependent effects occur is important to guide the development of control strategies against parasitic nematodes and the diseases that they cause. These density-dependent effects have been hypothesized to result from intraspecific competition between parasites for limited resources or from the action of host immune responses. However, no specific evidence exists

A case of Strongyloides stercoralis and mesenteric tuberculous infection with acute abdominal pain in an HIV-positive patient.

We describe an HIV-positive female patient who had acute abdominal pain as the initial presentation of Strongyloides stercoralis infection. The diagnosis was established by identifying rhabditiform larvae in stool. She also had intra-abdominal tuberculosis without intestinal perforation. To our knowledge, this is the first reported case of such a presentation.

Experimental studies with Stronglyloides papillosus in goats.

Unusual clinical and pathological observations in the field in goats and sheep suffering from Strongyloides papillosus infection prompted experimental work on this parasite. Goats were infected percutaneously with either single or multiple, low or high levels of S. papillosus. Young goats up to 12 months of age were found to be the most susceptible. Some animals, however, showed substantial resistance to infective doses. Clinical signs included transient diarrhoea, misshapen, elongated faecal pellets terminally, dehydration, anorexia, cachexia, gnashing of teeth, foaming at the mouth, anaemia
