The paper provides the results of using original procedures for revascularization of the osseous bed and free bony grafts by transplanting osteovascular complexes in patients with tuberculosis of the hip joint and spine. A total of 20 patients with tuberculous coxitis and 14 with spondylitis were operated on. These interventions were found to have advantages over plastic surgery with bony grafts and mobilizing operations without plasty in similar skeletal lesions, appeared as a great scope of movements in the operated joints is achieved, less time for grafts to adhere with the bed and bony blo

STUDY DESIGN: Retrospective case-control study.
OBJECTIVES: To analyze the results of two surgical treatments for lower lumbar tuberculous spondylitis with neurological deficits in the aged.
METHODS: We studied 33 cases of lower lumbar spinal tuberculous spondylitis treated in our center from January 2006 to October 2010. The cases were divided into two groups: 16 cases (group A) underwent single- or two-stage anterior debridement, bone grafting and posterior instrumentation, and 17 cases (group B) underwent single-stage posterior debridement, decompression, interbody fusion and instrumentat

INTRODUCTION: Tuberculous spondylitis (TBS) is the most common form of extra-pulmonary tuberculosis. The mainstay of TBS management is anti-tuberculous chemotherapy. Most of the patients with TBS are treated conservatively; however in some patients surgery is indicated. Most common indications for surgery include neurological deficit, deformity, instability, large abscesses and necrotic tissue mass or inadequate response to anti-tuberculous chemotherapy. The most common form of TBS involves a single motion segment of spine (two adjoining vertebrae and their intervening disc). Sometimes TBS inv

Lateral extracavitary and costotransversectomy approaches have been well described, and they are useful for posterior thoracic corpectomies. However, these approaches require pleural dissection and are associated with welldocumented morbidities, including hemothorax, pneumothorax, and pneumonia. But without removing the rib head, the window through which an expandable cage can be placed from a posterior approach is narrow. Thus, smaller nonexpandable mesh cages or methylmethacrylate constructs are commonly used for anterior column reconstruction. The authors describe a technique of using a "tr

Vertebral osteomyelitis or spondylodiscitis is an uncommon, mainly hematogenous, disease that usually affects adults. The incidence of this condition has steadily risen in recent years because of increases in spine surgery and nosocomial bacteremia, aging of the population, and intravenous drug addiction. Pyogenic infection due to Staphylococcus aureus is the most frequent form of the disease, but tuberculosis and brucellosis are still common causes of spondylitis in Spain. The clinical presentation is nonspecific and the diagnosis is often delayed. Magnetic resonance imaging is the most sensi

BACKGROUND: Intramedullary tuberculomas are rare. With the widespread availability of MRI and the increasing incidence of HIV and HIV-related tuberculous infections, the incidence of these lesions is likely to increase worldwide. The role of medical and surgical treatment of these relatively rare lesions remains to be defined. We report 2 patients who presented with intramedullary tuberculomas and discuss the importance of early surgery in this condition.
CASE DESCRIPTION: Two female patients presented with insidious onset of myelopathy. The first patient had seizures due to imaging-documente

Classic procedure in the treatment of vertebral tuberculosis is drainage of the abscess, curettage of the devitalized vertebra and application of antituberculous chemotherapy regimen. Posterior instrumentation results are encouraging in the prevention or treatment of late kyphosis; however, a second stage operation is needed. Recently, posterolateral or transpedicular drainage without anterior drainage or posterior instrumentation following anterior drainage in the same session is preferred to avoid kyphotic deformity. Seventy-six patients with spinal tuberculosis were operated in the 1st Depa

BACKGROUND: The cervical spine is injured in approximately 3% of major trauma patients, and 10% of patients with serious head injury. Therefore, clearance of the cervical spine in multitrauma patients is a critically important task. This is particularly important, considering that there is a positive correlation between a Glasgow Coma Scale of <14 and cervical spine injury. Radiography is not sensitive enough to rule out cervical spine injury, especially as radiography done in the trauma setting is usually technically unsatisfactory.
OBJECTIVE: The current study aims to assess the diagnostic

INTRODUCTION: The thoracic spine is stable because of kyphotic alignment, rib cage, and costovertebral joints. Any compression or kyphosis in the thoracic spine always causes spinal cord injury.
CLINICAL CASE: A 47-year-old male with complaints of back pain 1 day prior to admission, after he sustained a motorbike crush and landed on his back. The pain, radiates to both limbs, associated with severe spasms, numbness, and weakness in his lower extremities, however no incontinence. No other associated injuries were reported. 25 years ago he had a history of tuberculosis of the spine with progres

INTRODUCTION: Spinal epidural abscess (SEA) refers to collection of infection of the epidural space due to various etiologies. Tuberculosis (TB) of the spine is one of the important causes of SEA. Patient with SEA usually presents with history of fever, back pain, difficulty in walking, and neurological weakness. Magnetic resonance imaging (MRI) is the initial modality for diagnosis and it can be confirmed by examination of abscess for microorganism growth. It can be treated by laminectomy and decompression which helps to drain out the pus and relive the compression on the cord.

OBJECT: Access to the upper thoracic vertebrae has been hampered by numerous anatomical structures and is further impaired by the transition from cervical lordosis to thoracic kyphosis. Therefore, the authors endeavored to study an anterior transsternal approach for upper thoracic disease (T1–4).
METHODS: Fifty-four patients with upper thoracic disease underwent anterior decompression and fusion with sternotomy. Ages in the 33 men and 21 women ranged from 37 to 69 years (average 49 years). Before surgery, there were 7 patients with Frankel Grade B function, 17 with Grade C, 21 with Grade D, a

A 10-year old girl presenting with an insidious-onset low-grade fever for one month was found to have mediastinal widening with bilateral perihilar opacities. MRI scanning further revealed dorsal spondylitis with collapse of T2-4 vertebral bodies with prevertebral and epidural tubercular collections of 3 × 6.5 × 8 cm. This case emphasises the importance of considering TB among differential diagnoses of a mediastinal mass, especially among children from endemic countries.

Tuberculosis of the cervical spine differs from other vertebral localizations by its extreme rarity, the clinical images are very diversified, the radiological measurements allow a good diagnostic orientation and specifically the MRI which allows a multi-planar study of the various lesions. Only bacteriological evidence can confirm the diagnosis. The treatment is based on a 12-month antituberculosis multidrug therapy and much debate upon the surgical indication. In our case, the patient presented with bilateral cervicobrachialgia with pain on examination at the mobilization of the cervical spi

A cold abscess causing tracheo-oesophageal compression is quite unusual. A case of a 5-year-old boy with caries spine of D3-D4 vertebrea with respiratory distress due to mediastinal cold abscess pressing over trachea is being reported. Urgent costotransversectomy was done to decompress the cold abscess. There was immediate relief of airway compression which was evident from fall in airway pressure from 39 cm H2O to 29 cm H2O. The child was extubated on the 5th postoperative day and discharged on the 15th postoperative day in good condition.

Pott's disease, or tuberculosis of the spine, is the most common osteoarticular tuberculosis. Among them, dorsolumbar impairment is predominant. The authors report the case of a patient with a huge cold lumbar abscess associated with Pott's disease. The patient is a 32-year-old man presenting dorsolumbar tumefaction associated with an alteration in his general condition and fever for three months. Treatment by "traditional healers" did not provide any improvement. He consulted for mild lumbar pain triggered by fatigue appearing one week before and after the failure of the traditional practitio
