[SILICONE BREAST IMPLANTS - HISTORICAL MEDICAL ERROR].

Silicone is a foreign material to our body and therefore, has been found to stimulate the immune system. Silicone breast implants (SBIs), made of silicone polymer, have been used for aesthetic and medical purposes since the 1960s, and were found to trigger acute/chronic inflammation, eventually leading to the formation of fibrotic capsules on the surface of the implant. Silicone implants have been found to be associated with the development of severe and sometimes unexplained clinical manifestations such as: chronic fatigue, sleep and memory problems, widespread pain, dry mouth and eye, depres

Potential of EPR spin-trapping to investigate in situ free radicals generation from skin allergens in reconstructed human epidermis: cumene hydroperoxide as proof of concept.

The first step in the development of skin sensitisation to a chemical, and in the elicitation of further allergic contact dermatitis (ACD), is the binding of the allergen to skin proteins after penetrating into the epidermis. The so-formed antigenic adduct is then recognised by the immune system as foreign to the body. Sensitising organic hydroperoxides derived from autoxidation of natural terpenes are believed to form antigens through radical-mediated mechanisms, although this has not yet been established. So far, in vitro investigations on reactive radical intermediates derived from these sk

Platelet rich plasma as a minimally invasive approach to uterine prolapse.

Pelvic organ prolapse (POP) is a major health problem that affects many women with potentially severe physical and psychological impact as well as impact on their daily activities, and quality of life. Several surgical techniques have been proposed for the treatment of POP. The FDA has published documents that refer to concerns about the use of synthetic meshes for the treatment of prolapse, in view of the severe complications that may occur. These led to hesitancy in use of these meshes and partial increase in use of other biological grafts such as allografts and xenografts. Although there se

Bundles of spider silk, braided into sutures, resist basic cyclic tests: potential use for flexor tendon repair.

Repair success for injuries to the flexor tendon in the hand is often limited by the in vivo behaviour of the suture used for repair. Common problems associated with the choice of suture material include increased risk of infection, foreign body reactions, and inappropriate mechanical responses, particularly decreases in mechanical properties over time. Improved suture materials are therefore needed. As high-performance materials with excellent tensile strength, spider silk fibres are an extremely promising candidate for use in surgical sutures. However, the mechanical behaviour of sutures com

Gingival recession defects and guided tissue regeneration: a review.

The last decade has seen an increasing number of clinical reports on guided tissue regeneration (GTR) for reconstruction of gingival recession defects. This article reviews the value of GTR in the management of gingival recession defects based on records from such reports. Studies and case-series using nonresorbable and bioresorbable membranes, studies comparing GTR to the subepithelial connective tissue graft (CTG) procedure, and histologic reports of healing following GTR, published in the English language from 1985 to 2000, were identified using a Medline search and were included in the dat

Effects of polyglycolic acid and polypropylene meshes on postoperative adhesion formation in mice.

The purpose of this study was to investigate the effects of polyglycolic acid (PGA), an absorbable (ABS) mesh, and polypropylene (PP), a nonabsorbable (NA) mesh, on intestinal adhesion formation. Altogether 72 mice were divided into a control group of 24, an ABS mesh group of 23, and an NA mesh group of 25. All three groups were divided into two subgroups for evaluation of adhesion severity at postoperative (po) days 5 and 90. Adhesion severity was measured with adhesion grading and tissue hydroxyproline (OHP) levels. Adhesion degree was minimal (1) in all subjects on day 5. Also there was no

Use of collagen film as a dural substitute: preliminary animal studies.

Cadaver grafts, laminated metallic materials, and synthetic fabrics have been evaluated as dural substitutes. Use of cadaver tissues is limited by fear of transmission of infectious disease while use of synthetic materials is associated with implant encapsulation and foreign body reactions. The purpose of this study is to evaluate the use of collagen film as a dural substitute. Collagen films prepared from bovine skin were used to replace the dura of rabbits and histological observations were made at 16, 28, 42, and 56 days postimplantation. Controls consisted of dura that was removed and then

Release of peptides from sustained delivery systems (microcapsules and microparticles) in vivo. A histological and immunohistochemical study.

Sustained delivery systems (microcapsules, microparticles, or implants) developed for once a month administration of peptides are efficacious and convenient. Long acting formulations of several bioactive peptides are based on microcapasules of a biodegradable polymer poly(DL-lactide-co-glycolide) (PLG), but a better understanding is required of the mechanism of the peptide release from the microcapsules, which is assumed to be primarily by diffusion through pores. In order to clarify this mechanism, microcapsules and microparticles of the agonist [D-Trp6]-LHRH and microcapsules of the LHRH ant

Computed tomography of orbital lesions. A radiological study of 144 cases.

During the first two years with the 160 x 160 matrix EMI scanner 144 consecutive patients referred with suspicion of orbital disease were subjected to 190 computed tomography (CT) examinations. The X-ray attenuation and enhancement patterns of the various lesions were analyzed. In general it was difficult to correlate these parameters with the histopathological features. Hemangiomas, however, showed little enhancement and meningiomas a large degree of enhancement, while malignancies, lymphomas and pseudotumors were mainly intermediate. With orbital Graves' disease a low preinjection attenuatio

Mechanisms underlying biomaterial-regulated macrophage polarization in bone tissue engineering: From interface sensing to immune microenvironment remodeling.

Traditional biomaterials commonly trigger a foreign body reaction after implantation, leading to fibrous encapsulation and poor osseointegration. Advances in osteoimmunology have enabled contemporary tissue engineering to redefine biomaterials as active modulators of the osteoimmune microenvironment rather than as passive scaffolds. At the core of this reconceptualization is the targeted control of macrophage polarization to establish an immune environment conducive to bone repair. The plasticity of macrophages reflects the dynamic balance between the M1 pro-inflammatory and M2 pro-regenerativ

Biomechanical Evaluation of a Novel Sutureless Hydrogel Device for Peripheral Nerve Repair.

Following traumatic injury, nerve repair is essential to the restoration of muscle function and sensation. The current gold standard of nerve repair is microsuture repair, which requires trained microsurgeons to perform time-intensive and technically demanding procedures under high magnification. Microsuture repair suffers from inconsistent repair quality among surgeons and variable clinical outcomes. Neurorrhaphy sutures are non-resorbable and prone to fibrous tissue ingrowth and attendant foreign body reaction, both of which are believed to contribute to the observed shortfall in clinical ou

The Role of Positron Emission Tomography Imaging in Breast Implant Illness.

BACKGROUND: Explantation often alleviates symptoms in women with breast implant illness. However, persistent complaints in some cases may be linked to persistent silicone-induced inflammation from residual silicone particles. Positron emission tomography (PET) imaging could potentially detect this inflammation. This case series describes the PET findings in women with ongoing symptoms after explantation. METHODS: A retrospective review was performed of cases from the silicone outpatient clinic at the Amsterdam University Medical Centers, the Netherlands. All women underwent PET imaging due to

Adaptation of the living therapeutic materials concept to the immune sensing of neutrophil granulocytes.

Neutrophils are innate immune cells that perpetually patrol the circulation and tissues. They sense and migrate toward invading microbes to initiate and orchestrate a robust immune response. Their highly reactive nature, driven by multiple and redundant receptor families recognizing bacterial components, makes them particularly sensitive to contaminants or nonsterile implants. This often leads to a neutrophil-driven foreign body reaction that shields the implant and triggers inflammation, collateral tissue damage, or even sepsis. This presents a significant challenge for living therapeutic mat

Engineering macrophage responses through 3D scaffold microarchitecture.

Biomaterial implantation in living tissues triggers a physiological response known as foreign body reaction, leading to the recruitment of macrophages, that can polarize either into a pro-inflammatory (M1) or an anti-inflammatory (M2) phenotype. Currently, there is growing interest in tailoring the physical properties of biomaterials to promote efficient tissue regeneration. Tridimensionality can profoundly influence macrophage behaviour; however, there is no clear consensus on the underlying mechanisms. 3D microstructures may play a crucial role in modulating immune cells, promoting anti-infl

Development of a novel small diameter vascular graft based on an electrospun blend PET/PU scaffold: from fabrication to structural, mechanical, and in vitro evaluation.

The development of small-diameter vascular grafts (SDVGs) remains a significant challenge and unsolved problem due to issues with compliance mismatch, thrombosis, and graft failure. This study explores electrospun blended scaffolds made from polyethylene terephthalate (PET) and polyurethane (PU), both Food and Drug Administration (FDA)-approved polymers, as potential candidates for small-diameter vascular applications. Nanofibrous scaffolds composed of blended PET and PU were fabricated using the electrospinning method. The morphological and chemical properties of the scaffolds were characteri
