Clinical application of single-shot fast spin-echo sequence for cerebrospinal fluid flow MR imaging.

In normal-pressure hydrocephalus, disturbances in cerebrospinal fluid (CSF) circulation occur; therefore, understanding CSF dynamics is crucial. The two-dimensional phase-contrast (2D-PC) method, a common approach for visualizing CSF flow on MRI, often presents challenges owing to prominent vein signals and excessively high contrast, hindering the interpretation of morphological information. Therefore, we devised a new imaging method that utilizes T2-weighted high-signal intensification of the CSF and saturation pulses, without requiring specialized imaging sequences. This sequence utilized a

Stereotactic Navigation and Intracranial Hemorrhage Risk Following Ventricular Catheter Placement: A Single-Center Cohort Study.

BACKGROUND AND OBJECTIVES: Cerebrospinal fluid shunting is a common treatment for hydrocephalus. Several techniques for ventricular catheter placement exist, yet comparative safety data remain limited. This study evaluated whether stereotactic navigation-assisted ventricular catheter placement is associated with reduced 30-day postoperative intracranial hemorrhage (ICH) rates. METHODS: This was a retrospective analysis of adults who underwent frontal or occipital shunt placement for hydrocephalus between 2014 and 2023. The primary exposure variable was the use of stereotactic navigation for v

Primitive megalencephaly in children: natural history, medium term prognosis with special reference to external hydrocephalus.

We studied 74 children with primitive megalencephaly retrospectively with attention directed to familial megalencephaly, birth history, enlarged pericerebral subarachnoid space (SAS) (idiopathic external hydrocephalus), head and statural growth dynamics, developmental and school prognosis, morphological findings and development of subdural haematoma. In the megalencephalic children, no significant differences were found between those with normal or those with enlarged pericerebral SAS. Out of 62, 31 children (50%) were already megalencephalic at birth. Of 74, 37 children (50%) showed variable

Clinical utility of CSF Aβ38 in Japanese research and clinical cohorts.

INTRODUCTION: Previous studies have reported that cerebrospinal fluid (CSF) amyloid beta (Aβ42/Aβ38) performs comparably to Aβ42/Aβ40 in predicting amyloid positron emission tomography (PET) positivity in White cohorts. However, this finding has not been validated in diverse populations. Moreover, the utility of CSF Aβ38 in diagnosing various neurological diseases has not been fully understood. METHODS: We analyzed CSF Aβ38, Aβ40, Aβ42, phosphorylated tau181, and neurofilament light chain in Japanese research and clinical cohorts with Alzheimer's clinical syndrome (ACS) or non-ACS. RESULTS:

Reversible dementia and gait disturbance after prolonged use of valproic acid.

Valproic acid (VPA) has been reported to cause cognitive decline and parkinsonism that are reversed with cessation of medication. Common imaging findings among these reports demonstrate reversible cortical pseudoatrophy and enlargement of the lateral ventricles. This case exemplifies these adverse effects of VPA which can cause reversible neurological symptoms even in long-term treated patients and can present as dementia syndromes such as normal pressure hydrocephalus.

Ventriculo-peritoneal shunt surgery in an infant with double aortic arch, patent ductus arteriosus and atrial septal defect.

Double aortic arch with patent ductus arteriosus and atrial septal defect is an uncommon association. Such complex cardiac lesions may complicate an otherwise normal anesthetic course. We came across a case with aqueductal stenosis and hydrocephalus, scheduled for ventriculo-peritoneal shunt surgery, on an emergent basis. The child was managed successfully. The anesthetic implications of resultant left-to-right shunt with increased intracranial pressure have been described.

Tracheal tube cuff perforation during ventriculoatrial shunt procedure.

We report a case of tracheal and endotracheal tube cuff perforation that occurred intraoperatively during a ventriculoatrial shunting procedure for a patient with normal pressure hydrocephalus. Unusual tracheal anatomy and technique were contributory factors. Use of noninvasive imaging devices to guide the intraoperative placement of catheters may avoid many of the complications related to anatomy, skill, and technique. The method of diagnosis, prevention, and management is discussed.

Acute infarction of corpus callosum due to transient obstructive hydrocephalus.

Acute ischemia of the corpus callosum (CC) is not a well-known feature in patients with acute hydrocephalus. Herein, we describe a case with acute CC infarction due to another rare entity; transient obstructive hydrocephalus. A 66-year-old male was admitted with sudden onset right-sided hemiparesia. CT demonstrated a hematoma on the left basal ganglia with extension to all ventricles. The following day, the patient's neurological status progressed to coma and developed bilateral pyramidal signs. MRI demonstrated obstructive hydrocephalus and acute diffuse infarction accompanied by elevation of

Hydrocephalus and achondroplasia. A study of 25 observations.

Two series of achondroplastic patients were studied. The first series included 5 patients referred to our neurosurgical department for macrocranium and/or mental retardation. The second series was composed of 20 patients arbitrarily chosen from a medical group. Macrocranium was observed in 60% of the patients and was related to hydrocephalus in all but 2 cases. Radiological studies demonstrated that the posterior fossa was deformed and narrowed in its 3 planes, while its volume was conserved because of an abnormal ascension of the tentorium. The conflict between normal brain development and th

Preventing Distal Catheter Obstruction in Laparoscopic Ventriculoperitoneal Shunt Placement in Adults: The "Falciform Technique".

BACKGROUND: Distal obstruction of ventriculoperitoneal (VP) shunts is a major problem in the treatment of hydrocephalus. To avoid this complication, we describe a simple method using the falciform ligament to place and hold the distal catheter in the right subdiaphragmatic space, preventing catheter migration and distal obstruction. MATERIALS AND METHODS: We retrospectively collected data of all VP shunt insertion and revision for adults with idiopathic normal-pressure hydrocephalus between November 2011 and September 2013. All of these were done using the "falciform technique," with the dist

Contribution of ethical approach and effective communication to the correct diagnosis of acute appendicitis - a single center experience from a pragmatic perspective.

PURPOSE: Chronic venous hypertension due to jugular foramen stenosis has been proposed as an etiology for the hydrocephalus and tonsillar herniation seen in some patients with complex craniosynostosis. We report the use of MR venography (MRV) to evaluate venous outflow obstruction in this clinical setting. MATERIALS AND METHODS: We studied 17 patients, (ages 4 months to 34 years; mean 7.3 years) with complex craniosynostosis; 8 patients with Crouzon's syndrome, 2 with Apert's, 1 with Pfeiffer's and 6 patients without an eponymous classification. MR imaging included routine imaging sequences a

Transcutaneous pressure adjustable valve for the treatment of hydrocephalus and arachnoid cysts in adults. Experiences with 75 cases.

The authors report a series of 75 adults treated over the last four years for hydrocephalus (69 cases) or arachnoid cysts (6 cases) by using a transcutaneous pressure adjustable valve (Sophy SU 8), the mechanism of which is recalled. The shunt was ventriculo-atrial 46 times, ventriculo-peritoneal 23 times and cysto-peritoneal 6 times. The opening pressure of the valve was initially adjusted 56 times to the medium, 9 times to the high, and 10 times to the low position, according to each particular patient's needs. Radionuclide shuntogram demonstrating migration of distal ventriculoperitoneal shunt tubing out of the peritoneal cavity.

An 81-year-old man with a history of ventriculoperitoneal (V-P) shunt placement for symptomatic normal pressure hydrocephalus presented with recurrence of confusion and gait disturbance. Radionuclide cisternography demonstrated loculation of In-111 DTPA in the abdominal wall. A soft tissue mass palpated at the location of tracer accumulation was confirmed to be extraperitoneal pooling of fluid at the site of coiling of the distal shunt tip. After surgical revision, with repositioning of the distal tube in the peritoneal cavity, the patient's symptoms promptly improved. When symptoms of hydroce

A Rare Case of Delayed Ventriculoperitoneal Shunt Catheter Migration Into the Heart: A Case Report.

The treatment of hydrocephalus with a ventriculoperitoneal (VP) shunt can lead to complications such as shunt migration. A 67-year-old male, who had previously undergone VP shunt surgery for normal-pressure hydrocephalus, presented approximately five years later with symptoms of general weakness and abdominal pain. Imaging revealed shunt malpositioning, with the catheter passing through an abnormal route to the heart. The catheter was successfully removed under fluoroscopic guidance while monitoring patient's electrocardiogram to prevent potential secondary complications. Although rare, cardia

Evidence of an application of a variable MEMS capacitive sensor for detecting shunt occlusions.

A sensor was tested subdural and in vitro, simulating a supine infant with a ventricular-peritoneal shunt and controlled occlusions. The variable MEMS capacitive device is able to detect and forecast blockages, similar to early detection procedures in cancer patients. For example, with gradual occlusion development over a year, the method forecasts a danger over one month ahead of blockage. The method also distinguishes between ventricular and peritoneal occlusions. Because the sensor provides quantitative data on the dynamics of the cerebrospinal fluid, it can help test new therapies and work

Racialized economic segregation and disparities in non-small cell lung cancer care and outcomes.

BACKGROUND AND PURPOSE: Idiopathic normal pressure hydrocephalus (iNPH) is a cerebrospinal fluid (CSF) dynamics disorder as evidenced by the delayed ascent of radiotracers over the cerebral convexity on radionuclide cisternography. However, the exact mechanism causing this disruption remains unclear. Elucidating the pathophysiology of iNPH is crucial, as it is a treatable cause of dementia. Improving the diagnosis and treatment prognosis rely on the better understanding of this disease. In this study, we calculated the pulsatile transmantle pressure and investigated the phase lag between this

Craniectomy-associated Progressive Extra-Axial Collections with Treated Hydrocephalus (CAPECTH): redefining a common complication of decompressive craniectomy.

Extra-axial fluid collections following decompressive craniectomy have been observed in a variety of patient populations. These collections have traditionally been thought to represent extra-axial signs of hydrocephalus, but they often occur even in settings where hydrocephalus has been optimally treated. This study aims to elucidate the phenomenon of extra-axial fluid collections after decompressive craniectomy in patients with treated hydrocephalus, in order to improve identification, classification, prevention and treatment. We retrospectively reviewed all patients at a single institution u

Computed tomography findings in 15 cases of Creutzfeldt-Jakob disease with histological verification.

Computed tomography (CT) was normal in twelve of fifteen patients with definite Creutzfeldt-Jakob disease. In two patients CT showed mild sulcal widening, while marked ventricular enlargement and moderate cortical atrophy were seen in a patient who had both Creutzfeldt-Jakob disease and normal pressure hydrocephalus. No correspondence was observed between CT findings, severity of the clinical picture and postmorten gross brain examination. According to these results, a normal CT in a demented patient should suggest Creutzfeldt-Jakob disease. Conversely, CT is of value for the diagnosis of othe

Disproportionately Enlarged Subarachnoid-Space Hydrocephalus on MRI in Pathologically Confirmed Progressive Supranuclear Palsy.

BACKGROUND AND OBJECTIVE: Several studies have shown that idiopathic normal-pressure hydrocephalus (iNPH) can mimic other neurodegenerative disorders, particularly progressive supranuclear palsy (PSP). In this study, we investigated iNPH clinical and neuroimaging features in patients with autopsy-confirmed PSP or Lewy body disease (LBD) by assessing the normal pressure hydrocephalus (NPH) triad of symptoms and imaging features of disproportionately enlarged subarachnoid-space hydrocephalus (DESH) and Evans index (EI) on antemortem MRI scans. METHODS: Among our study participants (N = 190), th
