Ophthalmic outcomes five years after lensectomy among children with Down syndrome.

We report demographics and post-lensectomy outcomes in 30 children (39 eyes) with Down syndrome and cataract. Mean age at first lensectomy was 3.0 years (range, 0.04-12.9); 16 of 30 patients (53%) had lensectomy before 12 months of age. The 5-year cumulative incidence of glaucoma-related adverse events was 38% (95% CI, 0%-69%); of retinal detachment, 3% (95% CI, 0%-9%). The median myopic shift in children with refractive data at 5 years was -10.00 D (IQR, -12.50 to -8.38 D) in 11 aphakic eyes and -1.50 D (IQR, -3.00 D to -0.25 D) in 7 pseudophakic eyes. The period prevalence (combining cases a

A novel PAX6 mutation in Chinese patients with severe congenital aniridia.

PURPOSE: We identified a novel mutation in Paired Box gene 6 (PAX6) and characterized its associated clinical features of severe ocular malformation in a Chinese family with congenital aniridia. METHODS: We studied two patients with aniridia from a Chinese family. All patients and noncarriers in this family underwent full ophthalmologic, general and urinary examinations. Total genomic DNA was isolated from peripheral blood of two aniridia patients. PAX6 levels were determined by PCR and its mutational status was determined by sequencing. Direct sequencing detected variations in PAX6. RESULTS

Central anticholinergic syndrome: a case report.

Postoperative central anticholinergic syndrome (CAS) is caused by anticholinergic medications that cross the blood-brain barrier. Medications with central anticholinergic effects block muscarinic cholinergic receptors, resulting in a wide array of symptoms. Symptoms may range from coma to a highly agitated state. CAS may be underdiagnosed because of its varying presentation and lack of awareness. Differential diagnosis for the patient presenting with abnormal neurological signs and symptoms should include CAS after the exclusion of other potential causes. This case report details the occurrenc

Vestibular evoked myogenic potentials in neurofibromatosis 2.

Neurofibromatosis 2 (NF2) is characterized by bilateral vestibular neurofibromas. Although the facial nerve, the cochlear nerve, and the superior division of the vestibular nerve can be evaluated before surgery, whether the inferior division of the vestibular nerve is affected remains undetermined without an operation. A total of 7 patients with NF2 (2 men and 5 women) underwent pure tone audiometry, caloric testing, vestibular evoked myogenic potential (VEMP) testing, and magnetic resonance imaging. Audiometry revealed mean hearing levels of less than 26 dB for 1 ear, 26 to 70 dB for 8 ears,

Diagnostic value of eye movement and vestibular function tests in patients with posterior circulation infarction.

OBJECTIVE: To evaluate the diagnostic value of eye movements and vestibular function tests in PCI patients. METHODS: Thirty-eight PCI patients and 31 patients with unilateral vestibular neuritis (UVN) were enrolled and underwent eye movement tests and vestibular function tests (spontaneous nystagmus [SN], head-shaking nystagmus [HSN]). RESULTS: The rates of eye movement abnormality were 78.9% and 41.9% in PCI and UVN patients, respectively. The positive rate of SN and HSN were, respectively, 41.2% and 43.8% in cerebellar infarction patients, 33.3% and 66.7% in medullary infarction patients,

Exposures and health effects at sea: report on the NIVA course: maritime occupational medicine, exposures and health effects at Sea Elsinore, Denmark, May 2014.

The presentations and discussions summarised provide an overview on the current state of knowledge on a wide range of occupational health risks to which seafarers are exposed. The definition of an occupational risk for a seafarer poses problems as their ship provides both their working and their living environment and, because of its mobility, can expose them to diverse climatic and infectious risks. Knowledge about levels of exposure to potential health risks in seafarers is limited when compared to those working ashore while, because of a pattern of working that is often temporary and insecu

Autopsy confirmed multiple system atrophy cases: Mayo experience and role of autonomic function tests.

BACKGROUND: Multiple system atrophy (MSA) is a sporadic progressive neurodegenerative disorder characterised by autonomic failure, manifested as orthostatic hypotension or urogenital dysfunction, with combinations of parkinsonism that is poorly responsive to levodopa, cerebellar ataxia and corticospinal dysfunction. Published autopsy confirmed cases have provided reasonable neurological characterisation but have lacked adequate autonomic function testing. OBJECTIVES: To retrospectively evaluate if the autonomic characterisation of MSA is accurate in autopsy confirmed MSA and if consensus crit

Head-shaking aids in the diagnosis of acute audiovestibular loss due to anterior inferior cerebellar artery infarction.

OBJECTIVE: To determine the patterns and diagnostic value of head-shaking nystagmus (HSN) in patients with acute audiovestibular loss. METHOD: Eighteen patients underwent evaluation of spontaneous nystagmus, gaze-evoked nystagmus, HSN, head impulse test, ocular tilt reaction, subjective visual vertical, bithermal caloric tests, and pure-tone audiogram. The findings were compared with those of 21 patients with labyrinthitis. RESULTS: Fifteen patients (83%) exhibited HSN, and the horizontal HSN usually beat contralesionally (10/14, 71%). However, 9 (50%) patients also showed patterns of centra

Ocular characteristics in a variant microcephalic primordial dwarfism type II.

BACKGROUND: Microcephalic osteodysplastic primordial dwarfism, type II (MOPD II) is a rare disease that is assumed to be caused by a pericentrin (PCNT) gene mutation. Clinical manifestations have been reported in pediatrics and neurology; however, only a few ocular findings have been documented. CASE PRESENTATION: We present three unrelated cases of MOPD II with similar facial features and short stature. Unlike the cases described in the literature, all subjects had normal birth weight and height but their growth was retarded thereafter. In addition to delayed milestones, they have a broad fo
