Prevalence, determinants, and management of chronic kidney disease in Karachi, Pakistan - a community based cross-sectional study.

BACKGROUND: Chronic kidney disease (CKD) is increasing being recognized as a global public health problem. However, there is dearth of information on the prevalence, determinants, and management of CKD from low- and middle-income countries. The objectives of the study were to determine the 1) prevalence of CKD; 2) socio-demographic and clinical factors associated with CKD; and 3) the existing management of these patients with regards to blood pressure control, and use of antihypertensive medications. METHODS: We conducted a cross-sectional study on 2873 participants aged ≥ 40 years in 12 repr

[The burden of diabetes in Africa: a major public health problem].

W.H.O. predicts that there will be some 438 million diabetic patients in 2030, most of them living in developing countries. The IFD estimates that the prevalence of diabetes will rise by 98% in Africa during the next 20 years, with dramatic implications for public health and national budgets of the poorest countries. Type 2 diabetes is the most common form in Africa; type 1 is rarer than in western countries and tends to occur later. Two other forms seem specific to black Africans: ketosis-prone atypical diabetes, and tropical malnutrition-related diabetes. An increasing prevalence of obesity,

The safety and efficacy of benzodiazepine-modified treatments as a special form of unmodified ECT.

BACKGROUND: Muscle relaxants reduce musculoskeletal morbidity with electroconvulsive therapy (ECT) but need to be administered under general anesthesia. The administration of anesthesia is not always possible for patients prescribed ECT. Consequently, unmodified ECT is still widely practiced, especially in developing countries. METHODS: We prospectively assessed musculoskeletal morbidity in consecutive patients who received unmodified bitemporal ECT during a part or the whole of their ECT course. All patients were pretreated with an intravenous benzodiazepine (usually diazepam, 10 mg) to effe

BERNA: a century of immunobiological innovation.

At the time the Swiss Serum and Vaccine Institute Berne (BERNA) was found in 1898, few vaccines or immune globulins were available. This short list included vaccines against cholera, typhoid fever, plague, smallpox and rabies and equine anti-tetanus and diphtheria immune globulins. Furthermore, their use was restricted due to limited production capacity, uncertainty regarding safety and no public health infrastructure to promote their utilization. Today, safe and effective vaccines exist for more than 30 infectious diseases while human hyperimmune globulins exist to treat or prevent rabies, te…

Building research capacity and capability to enhance the quality of living and dying addressing advancing frailty through integrated care: the ALLIANCE partnership.

BACKGROUND: Frailty affects around 10% of people aged over 65 years, increasing to 65% of those over 90 years. This number is increasing. Older people with frailty are projected to become the largest future users of care services as they near end of life. Living with frailty increases vulnerability to sudden deterioration, fluctuating capacity and mortality risk. This leads to complex needs, requiring integrated care, and an approach orientated towards living with, as well as dying from, advancing frailty. However, accessing care in a timely manner can be difficult. AIMS: To develop a sustain

Comparing outcomes of COVID-19 and influenza among hospitalized adults in the European severe acute respiratory infection vaccine effectiveness (EuroSAVE) network, 2021-2024: a retrospective cohort analysis.

BACKGROUND: Understanding changes in the severity of respiratory virus infections over time is critical for guiding public health interventions. Few studies have been conducted in middle-income countries (MICs), where healthcare infrastructure, COVID-19 and influenza vaccine coverage, and antiviral availability and use differ from high-income countries. METHODS: We compared the odds of severe outcome and death by virus test result among patients ≥18 years old hospitalized with severe acute respiratory infection (SARI) in five MICs and areas (Albania, Georgia, Kyrgyzstan, North Macedonia, and

Extended-spectrum β-lactamase and carbapenemase-producing bacteria urinary tract infections in adult HIV-positive outpatients at Embu County Referral Hospital, Kenya.

BACKGROUND: Antimicrobial resistance (AMR) poses a significant threat to public health, with the Global Action Plan advocating, among others, for strengthening AMR knowledge and evidence base through surveillance and research and optimizing the use of antimicrobial medicines in human and animal health. However, due to inadequate healthcare infrastructure in many resource-constrained countries, epidemiological data to inform infection prevention interventions and guide antimicrobial policy guidelines in people living with HIV is limited. Here, we assessed the prevalence, antimicrobial resistanc

Structural challenges in accessing and using big data in health care: A case study based on palliative care research.

BACKGROUND: Big data holds transformative potential for health research, yet accessing and utilizing it remains challenging. Based on firsthand experience, this study identifies structural challenges and proposes ways to improve data accessibility and usability. METHODS: Using a case study within the PhD project DepEnD (Deprescribing at the End of Life), we analysed two data requests at the national level: Cohort 1, linking Belgian health insurance data with data from Statistics Belgium, and Cohort 2, with the Belgian Resident Assessment Instrument (BelRAI). We conducted document analysis inc

The time is now to use the tools we have to end AIDS in children.

INTRODUCTION: Progress in reducing and eliminating vertical transmission of HIV in children has stagnated over recent years. Unfortunately, recent decisions in the global donor space are likely to lead to considerably lower funding available for HIV and other disease programmes in high-burden low- and middle-income settings. Understanding and implementing the most effective strategies to prevent, diagnose, treat and monitor HIV acquisition in children are critical to maximize available resources and provide the best care for children. DISCUSSION: A set of tools to support the elimination of v

Effect of Tai Chi for the prevention or treatment of osteoporosis in elderly adults: protocol for a systematic review and meta-analysis.

INTRODUCTION: Osteoporosis (OP) has been defined as a degenerative bone disease characterised by low bone mass and microstructural deterioration of bone tissue, leading to fragility and an increased risk of fractures, especially of the hip, spine and wrist. Exercise has been shown to benefit the maintenance of bone health and improvement of muscle strength, balance and coordination, thereby reducing the risk of falls and fractures. However, prior findings regarding the optimal types and regimens of exercise for treating low bone mineral density (BMD) in elderly people are not consistent. As an

Reconstruction of a resilient and secure community and medical care system in the coronavirus era - English translation of the Japanese opinion released from the Science Council of Japan.

BACKGROUND: Over 3 years have passed since the outbreak of novel coronavirus disease 2019 (COVID-19), a disease associated with a high risk of severe illness and death among older individuals. This period has brought to light regional and social issues, including issues in overall and regional healthcare, that existed before the epidemic. "COVID-19-related frailty" is defined as secondary damage to health caused by inactivity and disconnection from human interaction owing to prolonged isolation among older individuals. Now in its fourth year, COVID-19 cannot be taken lightly, even though it is

Improving surveillance of infectious diseases in child daycare settings: insights from a pilot study in the Netherlands.

INTRODUCTION: Child daycare centres (DCCs) are high-risk settings for infectious disease transmission. Although Dutch DCCs are legally required to report unusually high numbers of infections to Public Health Services (PHS), underreporting is common. Our pilot study aims to enhance DCC surveillance using a simple paper-based syndromic tool and wastewater monitoring. METHODS: Large DCCs in the Dutch PHS-region Zuid-Holland Zuid were invited. During 16 weeks in summer and winter, DCCs recorded weekly counts of children (0-4 years) and staff with respiratory, gastrointestinal and skin infections
