Value of anterosuperior rotator cuff and labral tears at MRI for predicting long head of biceps tearing at arthroscopy.

AIM: To determine whether magnetic resonance imaging (MRI) signs of subscapularis (SBT), supraspinatus (SST), or superior labral tearing predict long head of biceps (LHB) tendon tearing at arthroscopy. MATERIALS AND METHODS: Subjects with MRI studies followed by arthroscopy were collected. Radiologists graded rotator cuff (RC) tendons and the superior labrum using three grade classifications and blinded to arthroscopy. Correlation between imaging variables and surgical outcome was expressed in terms of odds ratios and determined using a stepwise logistic regression model. RESULTS: Selection

[Use of cultured human autologous bone marrow stem cells in repair of a rotator cuff tear: preliminary results of a safety study].

PURPOSE OF THE STUDY Rotator cuff tears are one of the most frequent shoulder disorders which are often associated with pain and interfere with proper arm function. In order to evaluate the safety and effectiveness of using cultured human autologous mesenchymal stem cells (MSC) applied to the suture site during arthroscopic repair of a rotator cuff tear, a prospective clinical study was designed and started recently at the authors' department. Its primary goal was to evaluate the safety of using cultured human MSCs, the secondary goal then was to study a therapeutic effect of their application

Pseudoparalysis From a Massive Rotator Cuff Tear Is Reliably Reversed With an Arthroscopic Rotator Cuff Repair in Patients Without Preoperative Glenohumeral Arthritis.

BACKGROUND: Pseudoparalysis is defined as active forward flexion less than 90° with full passive motion. There is controversy about the ideal surgical management of a massive rotator cuff tear with pseudoparalysis. PURPOSE/HYPOTHESIS: The purpose of this study was to prospectively analyze the ability to reverse pseudoparalysis with an arthroscopic rotator cuff repair (ARCR). The hypothesis was that in the absence of substantial glenohumeral arthritis, preoperative fatty infiltration of grade 3 or higher and an acromiohumeral interval (AHI) of less than 7 mm would not prevent reversal of pseud

Is systematic long-term sonographic follow-up after arthroscopic rotator cuff repair useful in asymptomatic patients?

OBJECTIVE: To establish whether ultrasonographic (US) appearance is correlated to functional outcome in asymptomatic patients 12 months after rotator cuff repair. MATERIAL AND METHODS: 26 asymptomatic patients at twelve months after arthroscopic cuff repair and 26 controls were retrospectively included. A clinical score (Constant's Shoulder Score, CSS) was compared with a US score (the modified Sugaya Score, mSS). The minimal thickness of the anterior and posterior parts of the cuff was also measured. RESULTS: CSS improved significantly from preoperatively to postoperatively (mean increased

Reconstruction of rotator cuff tears in wheelchair-bound paraplegic patients.

BACKGROUND: Rotator cuff surgery in wheelchair-bound patients is challenging, and clinical data on this condition are limited. We hypothesized that rotator cuff repair in these patients might improve functional outcomes. METHODS: In a retrospective study, data on 13 paraplegic patients (8 men and 5 women; 16 shoulders) who underwent rotator cuff repair were analyzed. The average age at the time of surgery was 48.7 years. The causes of paraplegia were poliomyelitis in 9 patients, spinal fracture in 4, and cerebral infarction in 2. Open rotator cuff repair was performed in 11 patients and arthr

Effects of selective paralysis of the supraspinatus muscle using botulinum neurotoxin a in rotator cuff healing in rats.

We hypothesized that a temporary rotator cuff paralysis using botulinum-neurotoxin A (BoNtA) would lead to an improved tendon-to-bone healing after repair of supraspinatus lesions. One hundred sixty Sprague-Dawley rats were randomly assigned to either the BoNtA or the control (saline) group. BoNtA/saline-solution was injected into the supraspinatus muscle 1 week prior to surgery. A supraspinatus defect was made; we distinguished between a lesion with normal and increased repair load. Furthermore, one subgroup had the operated shoulder immobilized in a cast. Histologic analysis and biomechanica

MR imaging of the rotator cuff tendon: interobserver agreement and analysis of interpretive errors.

PURPOSE: To evaluate accuracy in interpretation of shoulder magnetic resonance (MR) images and interobserver agreement and to characterize the types of errors. MATERIALS AND METHODS: Five radiologists with varying experience independently and retrospectively twice interpreted the MR images of 222 symptomatic patients who underwent both MR imaging and shoulder arthroscopy. The first interpretation was a blind review; the second was with knowledge of the arthroscopic findings. RESULTS: For full-thickness tears, the sensitivity, specificity, and accuracy were 84%-96%, 94%-98%, and 92%-97%, resp

Arthroscopically enhanced "miniapproach" to rotator cuff repair.

The results of rotator cuff repair through a lateral deltoid splitting approach combined with arthroscopic subacromial decompression are presented in 18 patients with an average followup of 46 months. Sixteen patients (88%) scored good or excellent on the University of California at Los Angeles shoulder rating scale. Pain scores improved from an average of 1.6 to 7.6. Function scores improved from 2.5 to 8.4. Two patients had poor results; both had workers' compensation cases pending. One patient with a poor result had 2 complications: superficial infection and failure of repair that required

[Functional results of type A botulinum toxin versus oral anti-inflammatory agents in the rehabilitation of painful shoulder syndrome caused by rotator cuff lesion].

UNLABELLED: Rotator cuff conditions are characterized by unspecific signs, as well as anatomic alterations and symptoms. They have a multifactorial etiology and may include everything from tendinitis to massive, full thickness tears of the rotator cuff tendon that compromise the normal biomechanics of the involved shoulder. They usually occur in people over 40 years of age but lesions resulting from trauma may vary according to the mechanism of injury and are not directly related with the age at onset of symptoms. Vascular factors have been described as related with rotator cuff tendon damage

Case report and rehabilitative management of a complete musculocutaneous nerve injury caused by shoulder dislocation.

Anterior shoulder dislocation can secondarily cause nerve injury and/or rotator cuff tear. An elderly male with a recent shoulder dislocation and multiple medical comorbidities was transferred to a rehabilitation inpatient service in order to maximize his functional status. Physical exam suggested a brachial plexus injury. Electrodiagnostic testing confirmed a complete musculocutaneous and a partial axillary nerve lesion. This specific combination of nerve lesions is a previously unreported complication of anterior shoulder dislocation. Prognosis and treatment are discussed, particularly the u

Mechanically Preconditioned Biomimetic Gradient Scaffold for Tendon-to-Bone Regeneration in a Rabbit Rotator Cuff Tear Model.

BACKGROUND: Healing of the tendon-bone interface (TBI) after rotator cuff injury is limited, often leading to scar-mediated repair. Decellularized tendon fibrocartilage-bone composite (dTFBC) and bone marrow mesenchymal stem cell sheets (BMSCS) improve repair in preclinical models, but full scaffold encapsulation with mechanical preconditioning remains unexplored. PURPOSE: To evaluate TBI regeneration using mechanically preconditioned, BMSCS-encapsulated dTFBC scaffolds in a rabbit model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 48 male New Zealand White rabbits were r

The effectiveness of arthroscopically inserted onlay bioinductive implant on revision rotator cuff repair in workers' compensation patients: a case-controlled cohort study with minimum 2-year follow-up.

BACKGROUND: Retear following rotator cuff repair is relatively common. Onlay biological grafts offer a potential solution, as a source of additional healing. The purpose of this study was to determine whether the addition of an onlay bioinductive implant would improve repair integrity and examiner-measured and/or patient-rated shoulder function at ≥2 years after revision arthroscopic rotator cuff repair in workers' compensation patients. We hypothesized that the addition of the onlay bioinductive implant in arthroscopic revision rotator cuff repairs would improve repair integrity and functiona

Developing a three-dimensional convolutional neural network for automated full-volume multi-tissue segmentation of the shoulder with comparisons to Goutallier classification and partial volume muscle quality analysis.

BACKGROUND: Preoperative intramuscular fat (IMF) is a strong predictor of tendon failure after a rotator cuff repair. Due to the contemporary labor intensive and time-dependent manual segmentation required for quantitative assessment of IMF, clinical implementation remains a challenge. The emergence of accurate three-dimensional evaluation of the rotator cuff may permit implementation with greater inter-rater reliability than common subjective scales (eg, Goutallier classification (GC)). Here, we developed and validated a convolutional neural network (CNN) model for auto-segmentation of the sh

Protocol for the economic evaluation of individualised (early) patient-directed rehabilitation versus standard rehabilitation after surgical repair of the rotator cuff of the shoulder (RaCeR 2).

INTRODUCTION: RaCeR 2 is a pragmatic multicentre, open-label, randomised controlled trial, with full economic evaluation. The primary aim is to assess whether individualised (early) patient-directed rehabilitation (EPDR) results in less shoulder pain and disability at 12 weeks postrandomisation following surgical repair of full-thickness tears of the rotator cuff of the shoulder compared with the current standard (delayed) rehabilitation. This paper provides the protocol for the RaCeR 2 health economic evaluation. METHODS AND ANALYSIS: The health economic analysis of RaCeR 2 is made up of thr

Subcoracoid impingement exacerbates pain and scapular dysfunction in full-thickness rotator cuff tears.

BACKGROUND: Subcoracoid impingement co-occurs with subacromial impingement in 19% of the patients with rotator cuff tears. The influence of subcoracoid impingement in patients with full-thickness rotator cuff tear (FT-RCT) remains unknown. Therefore, it is worthwhile to investigate potential differences between FT-RCT patients with and without subcoracoid impingement. METHODS: Twenty-two participants were categorized as having an FT-RCT combined with subcoracoid impingement, while 25 others were diagnosed with an FT-RCT without subcoracoid impingement. Characteristics and tear sizes were reco

Human dermal allograft augmentation in primary and revision arthroscopic rotator cuff repair: a retrospective controlled study including patient outcomes and ultrasound evaluation of tendon healing.

PURPOSE: Rotator cuff (RC) tears are common in older adults, often leading to muscle atrophy. Standard arthroscopic repair has high re-tear rates, prompting the use of biological patches for augmentation. This study assessed differences in range of motion, strength, and tendon healing using ultrasound in primary and revision RC repairs using acellular dermal allograft augmentation. METHODS: Forty-eight patients undergoing arthroscopic RC repair with dermal allograft augmentation were assessed, with 42 completing a median follow-up of 32.4 months. Twenty patients had primary repairs and 22 had
