Spinal low-grade neoplasm with leptomeningeal dissemination mimicking tuberculous meningitis in a child.

Spinal seeding of primary malignant intracranial tumors via CSF is common. However, this is rare in low-grade glial tumors. Cranial leptomeningeal metastasis of primary spinal cord low-grade gliomas at diagnosis or relapse is extremely rare. Leptomeningeal metastasis of spinal cord low-grade tumors may mimic tuberculous meningitis in children. A patient with primary spinal cord low-grade neoplasm mimicked tuberculous meningitis is presented. The patient successfully treated with chemoradiotherapy. At the end of 19-month follow-up, diffuse leptomeningeal infiltration and a dural mass compatible

Tuberculous spondylitis--analysis of 22 cases.

UNLABELLED: Tuberculous spondylitis (TS) frequently poses both diagnostic and therapeutic challenges. The clinical symptoms, radiological imaging studies and laboratory tests are quite often inconclusive in the early stages of the disease. GOAL: To identify early clinical symptoms, review results from radiological imaging studies and laboratory tests to establish their diagnostic value and determine the effect of conservative therapy in patients with early TS. RESULTS: Twenty two (22) patients with TS subjected to conservative therapy were studied. Medullary compression syndrome was found in

[New non-invasive Doppler ultrasound study of blood flow in segmental arteries].

The authors have proposed and tested a new noninvasive technique of Doppler ultrasound study of impaired blood flow in the segmental arteries, which indirectly evaluates spinal cord hemodynamics in vertebral column diseases. Clinical trials of the technique proposed have provided evidence for the association of blood flow in the segmental arteries with the degree of vertebral column canal and kyphotic deformities. A combined impact of spinal deformity and impaired blood flow in the segmental arteries at the level of lesion on the severity of neurological disorders and on deterioration of life

Patterns of disease on MRI in 53 children with tuberculous spondylitis and the role of gadolinium.

BACKGROUND: Tuberculosis (TB) of the spine is the most common site of osseous involvement and has a higher prevalence in developing nations with an increasing incidence in developed nations. There are few paediatric reports of TB spondylitis (TBS) that include MRI findings. OBJECTIVE: To determine the MRI characteristics of TBS in children with special reference to gadolinium enhancement and findings on follow-up MRI. MATERIALS AND METHODS: A retrospective review of patient records and MRI scans by three readers using a consensus method of 53 patients below 13 years of age. RESULTS: Seventy

Syringomyelia--as a late complication of tuberculous meningitis.

The aim of this paper is to demonstrate the unusual MR features of thoracic syringomyelia following TB meningitis and to discuss the neurosurgical aspect of the treatment of this rare entity. Four years after a TB meningitis episode, a 30 year-old female patient developed a progressive spastic paraparesis. MR studies revealed multiloculated syrinxes throughout the thoracic cord. She had a syringo-subarachnoid shunt with a silastic "T" tube inserted. On the first postoperative day, she showed a dramatic neurological improvement, but unfortunately her paraparesis progressed to the preoperative l

[Predicting the degree of surgical risk in patients with disseminated forms of tuberculous spondylitis].

It was shown that the basic parameters of respiratory and circulatory disorders are of the most informative value for the prognosis of the severity of the postoperative condition of patients with tuberculous spondylitis. A two-dimensional variant of the classification of patients into three groups with a different degree of surgical risk is given.

[Tuberculosis of the spine and ribs].

Tuberculosis of the spine or ribs is uncommon, occurring in less than 1% of patients with tuberculosis (TB). 2 women are presented who recently immigrated from Ethiopia and India, respectively. One, aged 55, presented with chest pain, fatigue and several masses under the skin of the scalp. Chest X-ray and CT scan suggested Pancoast tumor, and the lateral parts of the first 2 right ribs were absent. The origin of the patient, clinical findings and positive PPD suggested TB of the ribs and anti-TB therapy resulted in cure. The diagnosis was later confirmed by a positive culture. The other woman,

Use of intrathecal hyaluronidase in the management of tuberculous meningitis with hydrocephalus.

A preliminary study to evaluate the efficacy of intrathecal hyaluronidase was carried out in nine children suffering from tuberculous meningitis with communicating hydrocephalus. This was followed by a randomized trial in which five cases were treated with intrathecal hyaluronidase, while six cases were treated by the insertion of a ventriculoperitoneal shunt. No untoward reaction of any significance was noted. The results were judged in terms of improvement in the sensorium and mentation, in specific neurological deficit (e.g., visual impairment and hemiparesis), and in overall functional per

Non-traumatic paraplegia in northern Tanzania.

A retrospective study of all 100 cases of non-traumatic (medical) paraplegia admitted to a large hospital in northern Tanzania over an eight-year period was undertaken; 15 of the patients were examined. Patients' ages ranged from 2 to 80 years (mean 31), and 67 were male. Seventy-one lived under 85 km (53 miles) from the hospital, and the average period from onset of symptoms of paraplegia to admission to the referral hospital was ten weeks. Tuberculosis was the most frequent cause of paraplegia (54%), followed by neoplasia (13%) and schistosomiasis, (6%). No cases of nutritional myelopathy we

Pott's paraplegia.

Tuberculosis of the spine associated with neurologic deficit has fortunately become an uncommon disease. Of 48 patients with Pott's paraplegia seen over a 20-year period, 28 were adequately followed up. Ten patients in this series died, most of causes not directly related to the tuberculosis, and ten patients were lost to follow-up. Most of the patients in this study were in the older age group which, in part, accounts for the mortality. Of those who did survive, the outlook for recovery from the paraplegia was very good with or without surgical intervention.

Research progress of metagenomic next-generation sequencing in infectious diseases of the spine: a systematic review.

BACKGROUND: Infectious diseases of the spine (IDS) cause structural destruction and abscess formation, requiring precise early diagnosis. While conventional culture methods show limited sensitivity and slow turnaround, metagenomic next-generation sequencing (mNGS) offers a promising alternative with its broader pathogen spectrum, rapid turnaround time, high detection rate, and sensitivity, showing significant advantages in the diagnosis of IDS. OBJECTIVES: This systematic review aims to synthesize the current evidence on the advantages and clinical utility of mNGS in diagnosing and managing I

Orthopaedic portrayals in The Seven Works of Mercy painted by a Dutch master in the year 1504.

PURPOSE: We examined The Seven Works of Mercy, painted by the Master of Alkmaar in 1504, through the lens of orthopaedic pathology. This study approaches the panels from a medical perspective, aiming to uncover visual indicators of disease and disability. The findings offer insight into how physical abnormalities were observed and depicted in the early sixteenth century. To our knowledge, this is the first study to explore orthopaedic pathology in the Seven Works of Mercy. METHODS: An interdisciplinary analysis of The Seven Works of Mercy was undertaken, with a focus on the visual representat

Evidence of Cushing's syndrome in a pre-Columbian Mexican statue?

INTRODUCTION: In the absence of skeletons or written narrations, information about diseases in past societies may be acquired from icono-diagnosis. From the observation of a masterpiece presenting pathological features, we tried to make retrospective diagnosis. MATERIAL AND METHOD: A pre-Columbian Mexican statuette originating from the Chupicuaro culture and dated 600 BC to 200 AD - conserved in the Louvre Museum in Paris (Section of the quai Branly - Jacques Chirac museum) was examined; it was found to display a huge spinal curvature with excessive dorsal kyphosis and obesity. RESULTS: The

Percutaneous wide bore needle biopsy for spinal lesions using the kyphoplasty cannula-a technical note.

PURPOSE: Management of vertebral and discovertebral lesions depends on highly specific modalities like biopsy and histopathology. The transpedicular and transforaminal routes have gained popularity for obtaining samples for diagnosing these lesions. Sample inadequacy plays an important part in lowering the diagnostic accuracy. Present study introduces a technical modification by using a kyphoplasty cannula and pituitary forceps through it thus improving sample adequacy and diagnostic accuracy. METHODS: Seventy-one patients with radiological evidence of spinal lesions from T1 to S1 were includ

Preclinical models of vertebral osteomyelitis and associated infections: Current models and recommendations for study design.

Spine-related infections, such as vertebral osteomyelitis, discitis, or spondylitis, are rare diseases that mostly affect adults, and are usually of hematogenous origin. The incidence of this condition has gradually risen in recent years because of increases in spine-related surgery and hospital-acquired infections, an aging population, and intravenous (IV) drug use. Spine infections are most commonly caused by Staphylococcus aureus, while other systemic infections such as tuberculosis and brucellosis can also cause spondylitis. Various animal models of vertebral osteomyelitis and associated i

A pilot study of inflammatory mediators in brain extracellular fluid in paediatric TBM.

Tuberculous meningitis (TBM) is the most fatal form of tuberculosis and frequently occurs in children. The inflammatory process initiates secondary brain injury processes that lead to death and disability. Much remains unknown about this cerebral inflammatory process, largely because of the difficulty in studying the brain. To date, studies have typically examined samples from sites distal to the site of disease, such as spinal cerebrospinal fluid (CSF) and blood. In this pilot study, we examined the feasibility of using direct brain microdialysis (MD) to detect inflammatory mediators in brain
