Bilateral vestibulopathy (BVP), which is due to peripheral lesions, may selectively involve certain semicircular canal (SCC). Recent eye movement recordings with search coil and video head impulse test (HIT) have provided insight in central lesions that can cause bilateral and selective SCC deficit mimicking BVP. Since neurological signs or ocular motor deficits maybe subtle or absent, it is critical to recognize central lesions correctly since there is prognostic and treatment implication. Acute floccular lesions cause bilateral horizontal SCC (HC) impairment while leaving vertical SCC functi

OBJECTIVES: The Dizziness Handicap Inventory (DHI) questionnaire is used to assess the severity of vertigo. In clinical practice, it is a good indicator for understanding the patient's feelings and can be used as a quantitative measure for monitoring ongoing treatment and rehabilitation. This study involved the translation, cultural adaptation, and validation of the Polish DHI questionnaire (DHI-POL).
METHODS: We recruited 127 subjects (mean age 55.1 years) who experienced vertigo, dizziness, and imbalance resulting from a disturbance to their vestibular system that had lasted longer than 1 m

OBJECTIVES: Cerebellar ataxia with neuropathy and bilateral vestibular areflexia syndrome (CANVAS) results from biallelic intronic pentanucleotide repeats in RFC1. We describe an adult male proband with progressive imbalance, cerebellar atrophy, somatosensory neuronopathy, and absence of peripheral vestibular function for whom clinical testing demonstrated a heterozygous RFC1 expansion consistent with an unaffected carrier.
METHODS: We performed whole-genome sequencing (WGS) on peripheral blood DNA samples from the proband and his unaffected mother. We performed DNA long-read sequencing and s

Neurofibromatosis type 2 (NF2) is an autosomal dominant genetic disorder resulting from germline mutations in the NF2 gene. Bilateral vestibular schwannomas, tumors on cranial nerve VIII, are pathognomonic for NF2 disease. Furthermore, schwannomas also commonly develop in other cranial nerves, dorsal root ganglia and peripheral nerves. These tumors are a major cause of morbidity and mortality, and medical therapies to treat them are limited. Animal models that accurately recapitulate the full anatomical spectrum of human NF2-related schwannomas, including the characteristic functional deficits

Otoferlin, a C2-domain-containing Ca(2+) binding protein, is required for synaptic exocytosis in auditory hair cells. However, its exact role remains essentially unknown. Intriguingly enough, no balance defect has been observed in otoferlin-deficient (Otof(-/-)) mice. Here, we show that the vestibular nerve compound action potentials evoked during transient linear acceleration ramps in Otof(-/-) mice display higher threshold, lower amplitude, and increased latency compared with wild-type mice. Using patch-clamp capacitance measurement in intact utricles, we show that type I and type II hair ce

INTRODUCTION: Bilateral and unilateral vestibulopathies mainly cause chronic imbalance/unsteadiness and oscillopsia, significantly impacting quality of life. Traditional clinical tests fail to assess functional impact on daily activities. This study aimed to characterize head movement patterns in bilateral vestibulopathy (BV) and unilateral vestibulopathy (UV) patients during functional mobility tasks in a semi-standardized environment. This study could provide useful information for rehabilitation and optimization of vestibular implant stimulation.
METHODS: Fifty-nine participants (19 BV, 20

We report the detailed documented case of a 57-year-old homosexual HIV-positive man with bilateral cochleovestibular deficits as a first symptom of syphilis infection in early stage II disease. As a morphological substrate, a strong enhancement of both inner ears and vestibulocochlear nerves were found on gadolinium-enhanced MR scans. The serological tests identified an active infection with Treponema pallidum. After a high-dose treatment with penicillin G and prednisolone, the auditory and vestibular functions and the MR morphology of the vestibulocochlear nerves and inner ears on both sides

INTRODUCTION: Persistent Postural-Perceptual Dizziness (PPPD), as an umbrella term for functional dizziness, encompasses a wide range of subjective symptoms affecting visual, vestibular, and motor functions. We developed the Athens-Lübeck Questionnaire (ALQ) as a bedside tool to differentiate specific symptom subtypes, which could inform more targeted research into the pathogenesis of the syndrome and facilitate tailored physiotherapeutic interventions.
METHODS: A total of 96 patients with primary or secondary PPPD were included in a prospective cross-sectional study conducted at two tertiary

OBJECTIVES: The number of people participating in sport self-contained underwater breathing apparatus (SCUBA) diving has increased tremendously, bringing with it a rise in diving accidents. Alternobaric vertigo (AV) is a common problem in SCUBA divers. We investigated the relationship between Eustachian tube function and incidence of AV in sport SCUBA divers. We also followed the progress of these divers after Eustachian tube function improved.
METHOD: Forty-four patients who experienced a SCUBA diving accident affecting the middle ear (11 men and 33 women; mean ± SD: 37.5 ± 11.5 yr) and 20 h

PURPOSE: The masseter acoustic/vestibular evoked myogenic potential (mVEMP) is a recently developed electrophysiological tool used to assess otolithic function. The mVEMP assesses brainstem integrity and is sensitive in detecting brainstem lesions. The masseter muscles are supplied by the mandibular branch of the fifth cranial nerve and help stabilize the jaw in response to high-intensity sound. The temporalis and masseter muscles function together to stabilize the mandible. Both muscles are supplied by the trigeminal nerve. Hence, the study hypothesized that mVEMP could also be recorded from

Vertigo is an illusion of rotatory or linear movement that demonstrates a functional or lesional disturbance of the vestibular system, from periphery to central connections. According to the ANAES report (1997), benign paroxysmal positional vertical vertigo, vestibular neuronitis and Ménière's disease account for 40-50% of all mixed vertigo etiologies. Central etiologies may account for 20-40% of causes and 10-40% remain more difficult to classify, and are usually classified under the term of "peripheral vestibulopathy." These include vertigo due to neurovascular compression syndrome of the VI

Recordings were made from phrenic, abdominal and intercostal nerves following electrical stimulation of the vestibular nerve to test for the presence of vestibulo-respiratory connections in cats that were decerebrate, paralyzed, and artificially ventilated. Short stimulus trains (2 or 3 shocks) at intensities < or = 125 microA typically elicited responses bilaterally in all of the respiratory nerves; the onset latency of the evoked activity was < 15 ms from the effective shock. The mean peak-to-peak amplitudes of integrated vestibulo-respiratory responses were 15% of the average amplitude of s

Parameters of the electrical activity of the isolated vestibulocerebellar complex of the frog were studied under in vitro conditions. In the region of the vestibular nucleus (nc. VIII), in the presence of stimulation of the stato-acoustic nerve (n. VIII), responses from efferent vestibular neurones and from unidentified (probably vestibulospinal) neurones were recorded. The latent periods of their excitatory postsynaptic potentials (EPSPs, 1.4-2.2 ms) were indicative of mono- and disynaptic connection. Inhibitory postsynaptic potentials (IPSPs) were also observed. Stimulation of the auricular

Dizziness and subjective balance disturbances are common complaints in cervical pain syndromes. We assessed balance function with posturography using vibration-induced and galvanically-induced body sway in consecutive patients (n = 116) with cervico-brachial pain syndrome of more than 3 months' duration. A total of 83% of the patients showed signs of cervical root compression on MRT scans. The incidence of complaints of vertigo was 50%. The patients manifested significantly poorer postural control than sex- and age-matched controls (n = 20). Disorders of the neck should be considered when asse

Ménière's disease is a clinical disorder, characterized by fluctuating hearing loss, recurrent spontaneous episodic vertigo, tinnitus and aural fullness, which may be defined as the idiopathic syndrome of endolymphatic hydrops. The most important test for diagnosis of Ménière's disease is the glycerol test. This is a simple and rapid method and several authors have confirmed its efficiency for identifying endolymphatic hydrops. This test provides information on the cochlear response to the osmotic changes produced by glycerol in the inner ear, whereas modifications in the vestibular labyrinth

The police brought a 65-year-old female patient to the EADU after being found 'roaming the streets' in an apparent state of confusion. This was her third admission under the same circumstances during the last 3 years. Neurological examination revealed (1) cognitive impairment, (2) oculomotor apraxia, (3) abnormal cancellation of vestibular ocular reflex, (4) mild ataxia and (5) mild hypotonia. Renal function was abnormal and liver function was normal. No retinal disturbance was found. The head CT on admission was normal for stroke and the lumbar puncture was negative for encephalitis. Her brai

OBJECTIVE: Our vestibular function is gradually deteriorating during aging, although, its behavioral consequences are not easily recognized due to a substitution process by other sensory modalities as visual or proprioceptive inputs.
METHODS: To reveal such a hidden substitution process by visual signals, the measurement of the static as well as the dynamic subjective visual vertical (SVV) was performed among 63 healthy subjects of different age.
RESULTS: The static SVV was found to be stable among all subjects, whereas the shift of the dynamic SVV during rotation of a background scene gradu
