Moyamoya disease among young patients: its aggressive clinical course and the role of active surgical treatment.

OBJECTIVE: The prognosis for moyamoya disease (MMD) among young patients is known to be worse than that among older patients. The aim of this study was to investigate the clinical features and treatment outcomes of young patients with MMD. METHODS: A total of 204 patients with MMD who underwent encephaloduroarteriosynangiosis, with or without bifrontal encephalogaleosynangiosis, were categorized into three groups according to their ages at the time of surgery, i.e., Group A (n = 23, <3 yr of age), Group B (n = 50, 3-6 yr of age), and Group C (n = 131, >6 yr of age). For each group, patterns o

Transcranial Doppler for internal carotid artery stenting: rationale and design of a randomized controlled trial.

BACKGROUND: Carotid artery stenting (CAS) is an effective revascularization procedure for internal carotid artery stenosis, but it is associated with a high incidence of perioperative cerebral embolism, with asymptomatic and symptomatic embolic events occurring in up to 40% of patients. Transcranial Doppler (TCD) has been shown to enhance fibrinolysis through sonothrombolysis and may reduce embolic burden during carotid interventions. AIMS: To evaluate whether intraoperative and post-procedural TCD monitoring can reduce the number of new diffusion-weighted imaging (DWI) lesions after CAS in p

Fetal-type posterior cerebral artery (fPCA): association with posterior communicating artery aneurysm rupture and treatment outcomes, with practical surgical risk-mitigation strategies.

BACKGROUND: The fetal-type posterior cerebral artery (fPCA) is a circle of Willis variant in which the posterior cerebral artery (PCA) territory is supplied mainly by the internal carotid artery (ICA) through a dominant posterior communicating artery (PComA), with a hypoplastic or absent ipsilateral P1 segment. This carotid dependence alters collateral reserve and the hemodynamics of the ICA-PComA junction, with consequences for aneurysm formation, rupture, and procedural safety. METHODS: We performed a narrative review. We searched PubMed/MEDLINE and Scopus for English-language studies on fP

Tyrosine Kinase Inhibitors as Risk Factors for Cerebral Vascular Disease: Report of Two Cases and Literature Review.

Treatment for chronic myeloid leukemia (CML) with tyrosine kinase inhibitors (TKIs), especially nilotinib and ponatinib, has been associated with atheromatic vascular adverse events including cerebrovascular disease. Herein, we present two patients with CML and long-term nilotinib treatment, who developed severe carotid atherosclerotic stenoses, both extra- and intracranial, resulting in ischemic stroke. The clinical and radiological findings as well as the possible pathophysiological mechanisms of these clinically significant complications are discussed. It seems that new-generation TKIs such

Spontaneous middle cerebral artery dissection: a series of six cases and literature review.

Middle cerebral artery (MCA) dissection is rare, and various clinical presentations, including hemorrhage, ischemia, or comorbidities, and the changes in imaging findings over time hinder treatment decisions. The European Stroke Organization guidelines exclude MCA dissection. Few cases have been reported with no review of the relevant literature. Therefore, we reviewed the relevant literature and our own experience with non-traumatic MCA dissection cases to determine appropriate treatment strategies. At our institution and affiliated institutions, we encountered six cases of MCA dissection-fiv

Indirect Bypass With Pericranial Transposition for Moyamoya Syndrome in an Infant.

Moyamoya syndrome (MMS) is a progressive disease that can result in debilitating strokes. Surgical revascularization is the mainstay of treatment. Selection of the proper bypass technique depends on the vascular anatomy and location of the hypoperfused cerebral territory. We describe here a case of successful indirect bypass utilizing a pericranial flap as well as dural inversion. A seven-month-old female was transferred from an outside facility to our institution for further evaluation and surgical treatment of MMS. She presented with bilateral brain infarcts worse on the left, with right-sid

Clinical Characteristics of Moyamoya Angiopathy in a Pediatric Cohort.

Moyamoya angiopathy is a rare cerebral vasculopathy characterized by a progressive stenosis of the terminal portion of the internal carotid arteries and the development of abnormal collateral vessels. Children with moyamoya angiopathy become symptomatic because of cerebral ischemic complications, and many patients eventually need revascularization. In most pediatric patients with this disease, the etiology is likely genetic. We aim to report clinical characteristics of a single-center cohort of pediatric patients with moyamoya. We performed a retrospective chart review of patients <18 years wi

Cerebral circulation improves with indirect bypass surgery combined with gene therapy.

Angiogenesis involves new blood vessels sprouting from preexisting blood vessels. This process may serve to improve brain circulation. Moyamoya disease (MMD) is a cerebrovascular disorder causing intracranial stenosis which significantly reduces the blood supply to the brain. Mainly stroke is the first symptom of the disorder, so treatments that reduce the risk of stroke are used for patients with MMD. To prevent stroke for those with chronic cerebral hypoperfusion, more blood needs to flow to the brain, which was thought to be achieved by enhancing angiogenesis. Indirect bypass surgery, such

[Ultrasonographic control of cerebral blood flow in patients with stenosis of brachiocephalic arteries before and after carotid endarterectomy].

The authors studied the parameters of arterial and venous blood flow in patients presenting with haemodynamically significant atherosclerosis of the carotid arteries before and after carotid endarterectomy, as well as while carrying out functional tests. Comparing the indices of arterial blood flow in the Study Group consisting of 52 patients and the Control Group comprising 15 apparently healthy volunteers at rest prior to the operation revealed statistically significant differences of volumetric parameters of blood flow in the internal carotid artery. The obtained results showed that the bre

Neuroprotective effects of Ilexonin A following transient focal cerebral ischemia in rats.

Ilexonin A is a compound isolated from the root of a plant commonly used in traditional Chinese medicine. The aim of the present study was to investigate the possible protective mechanism of Ilexonin A in rats subjected to occlusion of the middle cerebral artery (MCAO). Transient focal cerebral ischemia was induced by 2 h of MCAO, followed by reperfusion. Ilexonin A at doses of 20, 40 and 80 mg/kg were administered via intraperitoneal injection immediately following ischemia/reperfusion. The expression levels of glial fibrillary acidic protein (GFAP), ionized calcium‑binding adapter molecule‑1

Cerebral hemodynamics as a predictor of stroke in adult patients with moyamoya disease: a prospective observational study.

Moyamoya disease is an obliterative vasculopathy of the large arteries at the base of the brain. In the US, it most commonly affects women in their 3rd and 4th decades of life, frequently causing ischemic stroke. The natural history of this disorder is not well described. It is very likely that hemodynamic factors play an important role in the risk of future stroke, as has been established in atherosclerotic carotid occlusive disease. The authors describe an ongoing, prospective observational study designed to test the hypothesis that increased oxygen extraction in the cerebral hemisphere beyo

Virtual cut flow, an innovative noninvasive 4D ASL MRI biomarker of EIC bypass patency.

Extracranial-intracranial (EIC) bypasses are used to treat Moya-Moya syndrome and complex aneurysms. Evaluating bypass patency is vital. Current methods include intraoperative techniques like indocyanine angiography, micro-doppler probes, and postoperative non-invasive imaging. However, adjudicating the frequent discordant imaging results is done via conventional digital subtraction angiography (DSA) despite its associated risks. Our study aims to quantitatively evaluate bypass patency by introducing a novel biomarker, the virtual cut-flow index (VCFI) derived from 4D-PACK MRI imaging. We pros

Long-term safety and efficacy of distal aneurysm treatment with flow diversion in the M2 segment of the middle cerebral artery and beyond.

BACKGROUND: Indications for flow diversion stent (FDS) treatment are expanding. However, there is still a lack of evidence for the long-term outcome in distally located aneurysms in the M2 segment of the middle cerebral artery (MCA) and beyond. METHODS: Consecutive subjects (from June 2013 to August 2020) with MCA aneurysms in the M2 segment or beyond treated with FDS were reviewed retrospectively. The primary endpoints for clinical safety were the absence of mortality, stroke event, re-rupture of the aneurysm, and worsening of clinical symptoms. The primary endpoint for treatment efficacy wa
