The yeast Pityrosporum orbiculare is thought to cause the folliculitis associated with seborrheic eczema. However, a combination of mechanical and microbiological factors may be involved, with follicular occlusion leading to yeast overgrowth and folliculitis. A follow-up was done on the patients of the German multicenter study with severe conglobate acne who had been treated with different dosages of 13-cis-retinoic acid. Eighty-seven patients were monitored from 12 to 21 months. Optimal long-term therapeutic effects were obtained with an initial dose of 1.0 mg/kg body weight, for 3 months, followed by another 3-month-treatment period with 0.2 mg/kg body weight. The acitvities of six enzymes were determined by fluorometric micromethods in epidermal tumors (basal and squamous cell epithelioma, verruca seborrhecia and keratoacanthoma) and compared to the activities in the adjoining, apparently normal epidermis. INTRODUCTION: Erythroderma is a severe dermatologic condition with a broad differential diagnosis encompassing inflammatory, drug-induced, neoplastic, and infectious etiologies. Scabies is the most common neglected tropical disease (NTD) and can manifest in two common forms: classic and crusted types. Although rare, crusted scabies is an important and frequently underrecognized cause of erythroderma (accounting for <0.5% of cases) that poses diagnostic challenges due to its clinical overlap with other dermatoses associated with erythroderma.
