[Long-term results in the surgical treatment of pilon tibial fractures. A retrospective study].

BACKGROUND: While Pilon fractures of the tibia have been treated for decades by primary open reduction and internal fixation by plate osteosynthesis, during the last 10 years differential treatment was developed: After primary open reduction nowadays patients are treated with (according to type of fracture and tissue damage). As well as primary open reduction and internal fixation a two-step treatment (primary external fixator and delayed ORIF) or consolidation by external fixator combined with minimal invasive osteosynthesis (cannulated screws and K-wires) has been implemented. Furthermore, t

Outcome of Clavicle Fractures Treated by Various Modalities.

INTRODUCTION: Clavicle fractures account for a significant proportion of shoulder girdle injuries, with varying treatment modalities employed, ranging from conservative management to surgical intervention. This study aims to evaluate the outcomes of clavicle fractures treated conservatively versus those treated with open reduction and internal fixation (ORIF). MATERIALS AND METHODS: A retrospective analysis was conducted on 94 patients with clavicle fractures treated between 2019 and 2023. PATIENTS WERE CATEGORIZED INTO TWO GROUPS: those receiving conservative treatment (n = 52) and those un

Results of treatment of 145 fractures of the third metacarpal/metatarsal condyles in 135 horses (1986-1994).

The objectives of this study were to correlate condylar fracture characteristics and type of treatment with subsequent capacity for athletic ability, and to determine the characteristics of healing that affect prognosis after fracture fixation. Medical records, post operative radiographic studies and race records were examined for 135 horses sustaining 145 fractures. Sixty-five percent of horses overall started in a race post injury (SPI) in a mean time of 9.7 months with a mean of 13.7 races post injury. Having raced pre-injury did not confer an advantage to starting post injury, though nonst

Imaging techniques for distal radius fractures and related injuries.

In patients with distal radius fractures, routine roentgenographic examination in at least two or three planes should be performed first and is usually sufficient for correct diagnosis and adequate treatment. A careful examination of the soft tissues of the forearm and wrist on conventional roentgenograms may be key for correct diagnoses. Routine roentgenographic examination of the wrist, preferable in four views, should be performed in every patient with a distal radius fracture acutely as well as at follow-up. The technique is easy to perform, universally available, and inexpensive. Advanced

The Importance of Immediate Bone Block Autograft to Successfully Restore the Function and Aesthetic of the Anterior Alveolar Process and Teeth.

Maxillofacial traumatic injuries can damage the jaw, teeth, and soft tissues of the head and neck region. When these injuries occur, best practice is to reconstruct as comprehensively as is clinically prudent at time of injury. Smart and efficient procedures during the initial surgery can minimize subsequent reconstructive procedures in scope and number, minimize expense, and result in a better final aesthetic and functional outcome. Restoration of anterior alveolar jaw fractures with comminuted or avulsed segments becomes a complex prospect when left untreated after initial trauma or injury a

Treatment of distal radius fractures with a wrist-bridging external fixation: the value of alternating electric current stimulation.

The radiological and functional outcomes of two groups of comminuted intraarticular fractures of the distal radius that were treated with wrist-bridging external fixation, with or without an alternating electric current stimulation (EF + ES or EF group, respectively), were compared. The radial length, radial inclination and palmar tilt were measured on radiographs taken immediately after removal of wrist external fixator and also after bone union had occurred. Furthermore, active range of motion was compared at the final consultation. Bridging callus was observed earlier postoperatively in the

The triceps-preserving approach for semiconstrained total elbow arthroplasty.

Total elbow arthroplasty (TEA) is a useful tool to relieve pain and provide return to function for many conditions affecting the elbow. For conditions ranging from inflammatory arthropathies to comminuted intra-articular distal humeral fractures in the elderly, TEA is an excellent treatment alternative. Numerous surgical approaches for TEA have been described. Most surgeons use either a direct posterior or posterior-lateral incision. TEA is not without its complications. One such complication is insufficiency of the triceps. Many surgical approaches have been described to try to decrease the p

Hybrid kyphoplasty with short-versus intermediate- and long-segment pedicle screw fixations for the management of thoracolumbar burst fractures.

BACKGROUND: This study aimed to determine if the hybrid short-segment (HSS) technique is a good alternative to the intermediate-segment (IS) and long-segment (LS) techniques in pedicle screw fixations for acute thoracolumbar burst fractures (TLBFs). METHODS: In this retrospective evaluation, we examined 43 patients who underwent surgical treatments, including one- or two-level suprajacent (U) and infrajacent (L) pedicle screw fixations, for acute single-level TLBFs with neurological deficits between the T11 and L2 levels from July 2013 to December 2019. Among these patients, 15 individuals un

Tibial periosteum flap combined with autologous bone grafting in the treatment of Gustilo-IIIB/IIIC open tibial fractures.

PURPOSE: Gustilo IIIB/C injuries are common for tibia diaphysis fractures with high rates of nonunion, osteomyelitis, and amputation. However, the managements on tibial Gustilo IIIB/C injuries are still controversial and individual. The aim of this study is to introduce the tibial periosteum flap combined with autologous bone grafting to treat Gustilo-IIIB/IIIC injuries. METHODS: Sixteen Gustilo type IIIB/C tibial fracture patients who underwent tibial periosteum flaps with autologous bone grafting surgeries were retrospectively studied. In the first stage, the wound was treated with debridem

A Comparison of Plate Versus Screw Fixation for Segmental Scaphoid Fractures: A Biomechanical Study.

BACKGROUND: Volar locking plate fixation may offer several advantages over headless screw fixation for scaphoid nonunion, or segmental or comminuted fractures: (1) increased surface area for bony healing; (2) preserved vascularity; and (3) maintenance of a gap for graft insertion. The purpose of this study is to compare headless screw and locking plate fixation of segmental scaphoid fractures and to determine whether either fixation provides a greater mechanical advantage in osteoporotic versus nonosteoporotic bone. METHODS: Sixteen matched-pair cadaver scaphoids were dissected from a range o

Predictors of clinical outcomes after surgical treatment of displaced acetabular fractures in the elderly.

Outcomes following the open reduction and internal fixation (ORIF) of displaced acetabular fractures in the elderly have been inconsistent. Several factors associated with significant poor clinical outcomes have been reported; however, the factor that contributes independently to the outcomes remains unknown. This study aims to identify independent prognostic factors for the outcomes of surgically treated displaced acetabular fractures in the elderly. A total of 86 elderly patients with displaced acetabular fractures were treated by ORIF between May 1990 and June 2010. Matta's criteria and mod
