Intracranial venous sinus hypertension: cause or consequence of hydrocephalus in infants?

From a previous study of achondroplasia as well as from the observation of patients with hydrocephalus associated with craniostenosis, the authors have concluded that an increased superior sagittal sinus venous pressure (SSVP) could be the cause of the enlarged ventricles. However, other workers have demonstrated that an increased SSVP could be the consequence of increased intracranial pressure (ICP). Therefore, the authors undertook a study to determine if there was a physiological test that could distinguish between rare instances of increased SSVP caused by structural and irreversible narro

"Growing" cerebellum in an infant after shunt insertion.

INTRODUCTION: Supratentorial cortical mantle growth after shunt surgery in infants with posthemorrhagic hydrocephalus is common. However, cerebellar growth and Chiari are rare. PATIENT DESCRIPTION: We describe a term newborn with an intraventricular hemorrhage and posthemorrhagic hydrocephalus who underwent endoscopic third ventriculostomy followed by shunt placement at age 4 months. RESULTS: After shunt placement, her head circumference growth rate rapidly decreased from the ninety-seventh percentile to the third percentile. Six months after a shunt placement, cerebellar disproportional gro

Acute infantile thrombocytosis and vitamin K deficiency associated with intracranial haemorrhage.

A 5-week-old breast-fed girl presented unconscious and convulsing with heavily blood-stained CSF under increased pressure. This was associated with oozing from venepuncture sites, a grossly prolonged prothrombin time, and a raised platelet count. The prothrombin time became normal 18 hours after she had been given parenteral vitamin K and a transfusion of fresh plasma and red cells, but the thrombocytosis persisted. A moderate communicating hydrocephalus also remained and was treated, and one year later health and development appeared normal. The thrombocytosis resolved after a few weeks and h

[Clinical study on developmental hydrocephalus and its operative timing in lumbo-sacral meningomyelocele].

Nine infants with lumbo-sacral meningomyelocele were evaluated in the diagnosis and the operative timing of hydrocephalus. Seven cases received early operation for meningomyelocele within 36 hours after birth. Two cases with closed meningomyelocele were operated on 8 days and 32 days after birth. All of them were not infected in the central nervous system before and after its surgery. Five out of nine cases had rapidly developed hydrocephalus within a few weeks, and received ventriculo-peritoneal shunt (Shunt group: Case1-5). Two cases with gradual enlargement of the lateral ventricles and two

Diffusion-weighted imaging evaluation of subtle cerebral microstructural changes in intrauterine fetal hydrocephalus.

OBJECTIVE: Hydrocephalus is an important etiological factor in neurological decline. With the advent of fetal ultrasound, fetal hydrocephalus is now more frequently detected than in the past. Ultrasonography (USG) provides information on general morphology, but microstructural changes that may play a prognostic role are beyond the resolution of that technique. These changes may theoretically be revealed by diffusion-weighted magnetic resonance imaging (DW-MRI). In this study, our preliminary findings of DW-MRI on the hydrocephalic fetuses are presented. MATERIALS AND METHODS: Twelve fetuses w

Hydrocephalus Revisited: New Insights into Dynamics of Neurofluids on Macro- and Microscales.

New experimental and clinical findings question the historic view of hydrocephalus and its 100-year-old classification. In particular, real-time magnetic resonance imaging (MRI) evaluation of cerebrospinal fluid (CSF) flow and detailed insights into brain water regulation on the molecular scale indicate the existence of at least three main mechanisms that determine the dynamics of neurofluids: (1) inspiration is a major driving force; (2) adequate filling of brain ventricles by balanced CSF upsurge is sensed by cilia; and (3) the perivascular glial network connects the ependymal surface to the

[A case of choroid plexus angioma with subarachnoid hemorrhage--case report (author's transl)].

Authors report a case of choroid plexus angioma of lateral ventricle, in a 42 years old woman, initiated with an episode of subarachnoid hemorrhage. The localization of lesion, in the anterior horn and body of right lateral ventricle, producing primary intraventricular bleeding was revealed by pneumoencephalography, especially polytome pneumotomography. However, it was difficult to find by angiography. Patient was successfully removed angioma and showed good recovery. In review of the literatures, the choroid plexus angioma is found more frequently in children than adults, caused by primary in

Acute communicating hydrocephalus after intracranial arachnoid cyst decompression: A report of two cases.

BACKGROUND: Arachnoid cysts (AC) may cause hydrocephalus and neurological symptoms, necessitating surgical intervention. Cyst drainage may result in postoperative complications, however, these interventions are not normally associated with the subsequent development of acute hydrocephalus. Herein, we present two unique cases of AC drainage with postoperative development of acute communicating hydrocephalus. CASE DESCRIPTION: Case 1. A 75-year-old female presented with progressive headaches, cognitive decline, and questionable seizures. Her neurological examination was non-focal, but a head co

CSF shunts for dementia, incontinence, and gait disturbance.

From a review of our experience in the past 4 years and of the literature generally, the following comments can be made about selecting patients with idiopathic NPH for a shunt procedure. (a) In the clinical presentation, either significant gait difficulty or the full triad of dementia, ataxia, and incontinence should be present. If dementia occurred first or is the major symptom, shunting may not improve the patient. (b) A CT scan with periventricular low density and/or small sulci along with expansion of the entire ventricular system (especially the temporal horns) is strongly associated wit

Impaired cerebrospinal fluid circulation and cerebral lymphatic drainage in a rat model of chronic hydrocephalus.

The cerebrospinal fluid (CSF) not only protects the brain but also maintains homeostasis by removing metabolic waste produced by brain activity. This study hypothesizes that chronic CSF circulatory dysfunction, such as chronic hydrocephalus or normal pressure hydrocephalus (NPH), may be a critical condition in neurodegenerative diseases associated with metabolic waste accumulation. To investigate the CSF circulation and cerebral lymphatic drainage in a rat model of chronic hydrocephalus induced by kaolin injection, we performed time-dependent evaluations of intraparenchymal injection of tracer

Renal tubular dysfunction in patients with primary Sjögren syndrome.

BACKGROUND AND PURPOSE: There are only a few studies in the literature regarding the influence of atmospheric pressure on intracranial homeostasis and the mechanism of the relation has not been clarified. The aim of the study was to evaluate the influence of atmospheric pressure and ambient temperature on parameters of intracranial volume-pressure homeostasis including intracranial pressure and cerebral perfusion pressure as well as on blood pressure and body temperature. MATERIAL AND METHODS: The authors analyzed the influence of atmospheric pressure on intracranial pressure, blood pressure,

Complications of elective intracranial pressure monitoring in adult hydrocephalus.

Continuous invasive monitoring of intracranial pressure (ICP) can be used in the diagnosis and management of various types of chronic cerebrospinal fluid (CSF) circulation disorders, such as hydrocephalus, shunt dysfunction and idiopathic intracranial hypertension. The risk profile and incidence of adverse events of this surgical procedure in this patient population is not well established. We aimed to investigate and describe the risks of ICP monitoring in adult patients with chronic CSF circulation disorders. We analysed 152 patients undergoing continuous ICP monitoring between 2010 and 2019

[Computerized tomography in patients with neurocysticercosis with intracranial hypertension due to obstructive hydrocephalus: comparison with Dimer-X ventriculography].

The authors reviewed the alterations observed on computerized tomography (CT) examinations of 16 patients with increased intracranial pressure and obstructive hydrocephalus due to cerebral cysticercosis. Plain radiograms of the skull were available in all cases and Dimer-X ventriculography in 12 cases. In 7 cases there were radiologic signs of increased intracranial pressure. CT scan showed normal cerebral parenchyma in 9 cases and low density areas, with various size, with or without surround contrast enhancement, and/or small calcifications in the parenchyma. Hydrocephalus was the only alter

Cockayne syndrome: an atypical case.

We report a case of Cockayne syndrome with several atypical features. The patient displayed many of the typical features, including cachectic bird-headed dwarfism, photodermatitis, normal pressure hydrocephalus, and extraphyramidal and pyramidal tract signs. However, she also displayed the unusual and previously unreported features of late age at onset, relative stability until age 19, and fertility, with a successful pregnancy. Brain biopsy showed hypomyelination, no active demyelination, and fibrillary gliosis.

[Disturbance of Consciousness in a Patient with an Indwelling Lumbar-Peritoneal Shunt Following Retroperitoneal Laparoscopic Radical Right Nephrectomy : A Case Report].

An 83-year-old man with an indwelling lumbar-peritoneal (L-P) shunt (for idiopathic normal-pressure hydrocephalus) underwent retroperitoneal laparoscopic radical right nephrectomy for renal cell carcinoma (pT1aN0M0). Peritoneal perforation occurred intraoperatively, and he developed postoperative disturbance of consciousness. Computed tomography showed mild ventricular enlargement, which was attributed to L-P shunt failure secondary to increased pneumoperitoneum pressure. His level of consciousness was improved when we raised his head. Few reports have discussed complications observed during r

A Randomized Double-Blinded Clinical Study of Early Volumetric Changes After Shunt Surgery and MRI-Resistance of the Codman Certas® Plus Shunt Valve.

OBJECTIVE: Linear radiological measures have low sensitivity to detect changes in ventricular volume in patients with idiopathic normal pressure hydrocephalus. Ventricular volumetry is accurate and sensitive in detecting subtle changes in cerebrospinal fluid volumes. The Codman Certas® Plus is an adjustable shunt valve with 8 settings and resistant to magnetic resonance imaging (MRI)-induced inadvertent adjustments in vitro. The aim of this study was to investigate early volumetric changes in ventricles after ventriculoperitoneal shunting in relation to shunt setting and linear measures. We al

[Post-operative complications peculiar to lumboperitoneal shunt: possible consequences due to side leakage of CSF from around the inserted spinal tube into the lumbar epidural space].

This is a report on a few possible consequences of occasional side leakage of CSF into the lumbar epidural space. In this report, we refer to such CSF side leakage from around the inserted spinal tube which occurred during the early stages following our lumboperitoneal shunt operations in two of our patients. This leakage can lead to over-drainage of CSF, and such complications as post-operative orthostatic headache, subdural effusion, and/or subdural hematoma can also result. The diagnosis of these complications can be verified if CSF components in the lumbar epidural space are visible in T2

Chronic Histoplasma capsulatum infection of the central nervous system successfully treated with fluconazole.

A 48-year-old man with a long-standing history of communicating hydrocephalus is reported. Ventriculoperitoneal shunting led to clinical improvement, but symptoms recurred despite surgical re-exploration switching the shunt to an atrial drainage. Ten months after the last surgical procedure, an acute myelopathy developed. Concomitant pharyngeal granuloma examination identified Histoplasma capsulatum (Hc) yeasts. Despite initial response to amphotericin B, Hc was isolated from cerebrospinal fluid (CSF), valve reservoir and distal catheter after two courses of therapy. Fluconazole successfully s

New non-invasive sonographic modality for intracranial pressure/volume monitoring.

INTRODUCTION: The wide use of intracranial pressure and cerebral perfusion pressure monitoring has improved the management of patients with severe head injuries. The rare but worrying complications associated with the application of such monitoring makes the idea of a non-invasive method of monitoring very attractive. MATERIALS AND METHODS: A new non-invasive ultrasonographic technology was used to measure cerebral perfusion pressure in 27 normal volunteers. The average monitoring time was 45.3+/-0.2 min, and the average perfusion pressure recorded was 77.4+/-0.3 mmHg. No complications were r
