Acute pelvic pain in women is a common presenting complaint that can result from various conditions. Because these conditions can be of gynecologic or nongynecologic origin, they may pose a challenge to the diagnostic acumen of physicians, including radiologists. The paper pools the experience of ultrasound assessment of acute mesadenitis in children with acute abdominal pain syndrome. It reviews the literature on its etiology, pathology, and diagnosis and treatment principles. B-mode and Doppler echograms made by general practice ultrasound diagnosticians are shown. The technique is demonstrated to be effective and of informative value in the early stages of diagnosis in the patient contingent in question.

Nine patients with primary immunodeficiency who received bacillus Calmette-Guerin (BCG) vaccine at birth developed disseminated BCG lesions and presented clinically with generalized skin rash and skin nodules. Fine-needle aspiration biopsy of the skin nodules and/or enlarged lymph nodes was performed in all patients. Cervical lymphadenopathy is the most common presentation of granulomatous inflammation of the neck in children and is usually caused by NTM infection. Although certain granulomatous infections have characteristic imaging features, there is considerable overlap in the imaging appearance of the various disorders. The diagnosis is usually based on a combination of clinical features, histopathologic examination, serologic tests, and culture results.

Cervical adenopathy is often found on examination, but rarely causes clinical problems. If the adenopathy persists for a longer period then three to four weeks and no infectious causes are found, further investigations are needed, especially to exclude a malignancy. The chronic infectious adenopathies are discussed: mycobacterial (tuberculous and atypical), toxoplasmosis, cat-scratch disease and actinomycosis.

European erythema migrans disease, lymphocytoma, and acrodermatitis chronica are a group of disorders associated with the bite of ixodid ticks. These disorders are now thought to be due to a single, or closely related, ixodid tick spirochetes. European erythema migrans disease closely resembles Lyme disease. Serological evaluation may help to separate spirochetal lymphocytoma from other pseudolymphomas of nonspirochetal origin and from lymphoma. Acrodermatitis chronica atrophicans, so far observed mainly in Europe, is presumably a late manifestation of this group of spirochetal disorders.

Sinus histiocytosis with massive lymphadenopathy is an unusual disease characterized by prominent lymph node enlargement, especially in the cervical region. The clinical course is benign although prolonged, and no specific treatment is required. The etiology and pathogenesis are unknown. Cervical adenopathies are common in children. The four categories, based on clinical findings are acute unilateral adenitis, acute bilateral adenitis, subacute (chronic) adenitis, and cervical node cancers. Most cases of acute pyogenic adenitis-both unilateral and bilateral-respond well to early antibiotic treatment. A white North American girl with clinical features of the mucocutaneous lymph node syndrome had a myocardial infarct and angiographic evidence of a coronary artery aneurysm and mitral regurgitation. The mucocutaneous lymph node syndrome has been extensively diagnosed in Japan in recent years. It appears to be a distinct entity, although not precisely separated from polyarteritis nodosa in childhood. The condition may be more common than previously realized.
