Systematic Review and Meta-Analysis: Recurrent Laryngeal Nerve Variants and Their Implication in Surgery and Neck Pathologies, Using the Anatomical Quality Assurance (AQUA) Checklist.

INTRODUCTION: The recurrent laryngeal nerve (RLN) is the structure responsible for sensory and motor innervation of the larynx, and it has been shown that its lesion due to a lack of surgical rigor led to alterations such as respiratory obstruction due to vocal cords paralysis and permanent phonation impairment. The objectives of this review were to know the variants of the RLN and its clinical relevance in the neck region. METHODS: This review considered specific scientific articles that were written in Spanish or English and published between 1960 and 2022. A systematic search was carried o

[Vascular rings of aortic origin: the surgical experience in 43 cases].

INTRODUCTION AND OBJECTIVES: Aortic vascular rings can produce compression and obstruction in the airway and/or esophagus early surgical treatment in the first days or months of life. The reported incidence range between 0.3-0.6% with an 0.5% in our series. The following is a retrospective analysis with clinical, radiological and angiocardiographic study of the patients operated on in our service with the diagnosis of aortic vascular ring focused on the surgical results. METHODS: A total of 43 children with ages ranging between 21 days and 10 years (mean age 17 months) underwent surgical trea

Less perilaryngeal gas leakage with SLIPA™ than with LMA-ProSeal™ in paralyzed patients.

PURPOSE: The aim of this study was to compare the Streamlined Liner of the Pharynx Airway (SLIPA™) with the ProSeal Laryngeal Mask Airway (LMA-ProSeal™) in mechanically ventilated paralyzed patients undergoing laparoscopic gynecologic surgery. METHODS: One hundred and one patients were allocated randomly to SLIPA (n = 50) or to LMA-ProSeal (n = 51) treatment groups. After induction of general anesthesia and insertion of the assigned supralaryngeal airway (SLA) device, we made note of the occurrence of any gastric insufflation and perilaryngeal leakage. We then evaluated the anatomical fit of

Idiopathic vocal fold paralysis: clinical course and outcomes.

This investigation details the results of a retrospective chart review of 35 patients with idiopathic vocal fold paralysis. Clinical findings at presentation, as well as patient outcomes in both the short and long-term duration are comprehensively reported. Spontaneous improvement of vocal fold function was observed in 25% of patients with long-term follow up. Of those with unilateral paralysis, 27% elected to undergo surgical intervention for improvement of symptoms. Four out of the five (80%) patients with bilateral paralysis required surgical intervention, including tracheotomy to relieve s

The upper airways. II. The larynx and trachea.

The laryngotracheal airway is the bottleneck of the respiratory system. Whereas disease in the nose or in the bronchial airways will affect respiratory airflow in a limited way, laryngotracheal disease must always be considered potentially life threatening. The nasal airway is the normal access to the lungs. As such, it serves vital functions in modifying the condition of ambient air before its access to the lungs. Impairment of the efficacy of nasal function in that regard will inevitably cast an unaccustomed burden on the lower airways and may possibly play a role in the long-term developmen

Recovery of vocal fold immobility following isolated patent ductus arteriosus ligation.

OBJECTIVE: Identify laryngoscopic and functional outcomes of infants with vocal fold immobility (VFI) following patent ductus arteriosus (PDA) ligation and identify predictors of recovery. METHODS: Retrospective review of patients with VFI following PDA ligation from 2001 to 2012 at a single institution. Inclusion criteria were: (1) PDA ligation as only cardiac surgical procedure; (2) left VFI documented by laryngoscopy; (3) minimum follow up 120 days, with at least 2 laryngoscopies performed. Resolution of VFI was determined at follow-up laryngoscopy. Univariate logistic regression models we

Otolaryngology clinic of North America: evidence-based practice: management of hoarseness/dysphonia.

This article reviews the evidence for the evaluation and management for patients with dysphonia. The evidence behind laryngoscopy, laryngostroboscopy, laryngeal imaging, laryngeal electromyography, and disease-specific questionnaires are reviewed. The evidence for management of some of the common conditions leading to dysphonia is also reviewed. This article reviews the evidence for voice therapy for various voice pathologies; medical management of dysphonia, including antibiotics, steroids, and antireflux therapy; and surgical management of glottic insufficiency and some benign laryngeal mass

Recovery of respiratory activity after C2 hemisection (C2HS): involvement of adenosinergic mechanisms.

Consequences of spinal cord injury (SCI) depend on the level and extent of injury. Cervical SCI often results in a compromised respiratory system. Primary treatment of SCI patients with respiratory insufficiency continues to be with mechanical ventilatory support. In an animal model of SCI, an upper cervical spinal cord hemisection paralyzes the hemidiaphragm ipsilateral to the side of injury. However, a latent respiratory motor pathway can be activated to restore respiratory function after injury. In this review, restoration of respiratory activity following systemic administration of theophy

Laryngeal dysfunction after thyroid surgery: diagnosis, evaluation and treatment.

Because of the close anatomical relationships between thyroid gland and laryngeal nerves, sensory-motor impairment of the laryngeal functions is a well known possible complication of thyroid surgery. Laryngeal nerve paralysis can present with various and often associated symptoms like dysphagia, aspiration, voice alteration or dyspnea. Several examination procedures are mandatory to perform a complete neuro-laryngeal evaluation: rigid and flexible video-stroboscopy will assess the abductor, adductor and tensor functions in breathing, sniffing, talking and eventually singing tasks. Laryngeal el

[The evaluation of intracordal silicone injection in patients with postoperative recurrent nerve paralysis].

This study aimed to evaluate the usefulness and efficacy of intracordal silicone injection in the treatment of cordal paralysis after thoracic surgery or due to tumor invasion of the recurrent nerve. A total of twenty two patients with cordal paralysis was treated with this procedure. The cause of the cordal paralysis was injury to the recurrent nerve incurred during operation for oesophageal or lung cancer in nineteen patients, and direct invasion of malignant neoplasm to the recurrent nerve in the other three patients. Among the nineteen postoperative patients, fifteen underwent silicone inj

A retrospective analysis of stridor after vestibular schwannoma surgery.

BACKGROUND: Transient lower cranial nerve deficits may occur after surgery in the posterior cranial fossa. Stridor has been reported after cerebellopontine angle epidermoid resection. The aim of this retrospective study is to find out whether any preoperative, intraoperative, and postoperative factors lead to stridor after resection of vestibular schwannoma. METHODS: Data of patients who underwent vestibular schwannoma resection from 2006 to 2011 were collected. We collected the following factors--age, sex, weight, diabetes, hypertension, preoperative cranial nerve deficits, tumor characteris

Semi-skeletonized internal mammary grafts and phrenic nerve injury: cause-and-effect analysis.

Phrenic nerve injury after cardiac surgery increases postoperative pulmonary complications. The purpose of this study was to analyze the causes and effects of phrenic nerve injury after cardiac surgery. Prospectively collected data on 2084 consecutive patients who underwent cardiac surgery from Jan. 1995 to Feb. 2002 were analyzed. Twenty-eight preoperative and operation related variables were subjected to logistic analysis with the end point being phrenic nerve injury. Then phrenic nerve injury and 6 perioperative morbidities were included in the analysis as variables to determine their indep
