BACKGROUND: Resection of a residual retroperitoneal tumor mass (RRRTM) is standard procedure after combination chemotherapy for metastatic nonseminomatous testicular germ cell tumors (NSTGCT).
METHODS: At the University Medical Center Groningen, 79 consecutive patients with disseminated NSTGCT were treated with cisplatin combination chemotherapy between 2005 and 2007. Laparoscopic RRRTM was performed for patients with RRTM located less than 5 cm ventrally or laterally from the aorta or the vena cava. The 29 patients who fulfilled the criteria had a median age of 25 years (range, 16-59 years).

From March, 1988, to October, 1993, fifty-six consecutive patients with rectal adenocarcinoma in clinical stage T3NxM0, underwent preoperative radiation therapy at the "Casa Sollievo della Sofferenza" Hospital of San Giovanni Rotondo (Italy). BACKGROUND: IgG4-related disease (IgG4-RD) can affect any tissue. Characteristic symptoms include tumor-like swelling of one or more organs, lymphadenopathy, involvement of the pancreatic and/or bile ducts, aortitis and retroperitoneal fibrosis, inflammatory swelling of the lacrimal or salivary glands, and nephropathy. Laboratory changes include hyperproteinemia, increased IgG4 concentration, eosinophilia, and elevated circulating plasmablasts. Presence of any of these symptoms should prompt a differential diagnosis focused on IgG4-RD.

We report a case of a 63-year-old man presenting with abdominal pain and hydronephrosis secondary to periaortic fibrosis (PAF) 8 months after an endovascular aortic repair (EVAR) using a woven polyester bifurcated graft. De novo delayed PAF after open repair is rare and even more infrequent after EVAR. All 3 previously reported cases occurred after woven polyester grafts and no reported cases after polytetrafluorethylene grafts. Management included steroidal anti-inflammatory treatment and bilateral double J tube placement. Satisfactory results were obtained.

Immunoglobulin G4-related disease (IgG4-RD) is an autoimmune disorder characterized by the infiltration of one or more organs with IgG4-positive plasma cells resulting in inflammatory lesions and fibrosis. A urinoma is an encapsulated extravasation of urine which can be secondary to trauma or which can occur spontaneously. The essential factors are continued renal function, rupture of the collecting system and distal obstruction. The extravasated urine causes lipolysis and stimulates an intense fibrous reaction which forms a thick wall. This lipolysis may be due to a mass effect as no direct effect of the urine on adipocytes has ever be demonstrated. Early diagnosis by CT scan allows easier treatment: in certain cases, percutaneous drainage alone may be sufficient.
