BACKGROUND: Avascular necrosis at sites other than femoral head (AVNOFH)/Non-Femoral AVN is a rare entity. No standard of treatment still exists for treating early stages of AVNOFH with most of the cases eventually progressing to a late arthritic stage needing surgical intervention. Bisphosphonates have been shown to prevent disease progression, bone collapse, and the requirement for surgery in avascular necrosis of femoral head. The present study is conducted to evaluate the response of bisphosphonates in the non-surgical management of the early stages of AVNOFH.

Background: Whether antenatal and neonatal vitamin D status have clinical relevance in fracture prevention has not been examined extensively, although observational studies indicate that fetal life may be a sensitive period in relation to bone growth and mineralization during childhood. Objective: We examined whether 25-hydroxyvitamin D3 [25(OH)D3] concentrations in stored neonatal dried blood spot (DBS) samples are associated with pediatric fracture risk. We hypothesized that in particular, low neonatal vitamin D status may be a risk factor for fracture incidence among children.Design: In a re

BACKGROUND: Preliminary experience suggests that deep learning algorithms are nearly as good as humans in detecting common, displaced, and relatively obvious fractures (such as, distal radius or hip fractures). However, it is not known whether this also is true for subtle or relatively nondisplaced fractures that are often difficult to see on radiographs, such as scaphoid fractures.
QUESTIONS/PURPOSES: (1) What is the diagnostic accuracy, sensitivity, and specificity of a deep learning algorithm in detecting radiographically visible and occult scaphoid fractures using four radiographic imagin

The authors report the case of a 23 year old patient, who presented the following combination of injuries after a fall, namely a divergent dislocation of the elbow, a fracture of the radial head, a fracture of the ulna and a perilunate dislocation with scaphoid fracture. The entire injury thus represented a bipolar dislocation of the forearm. The emergency management consisted of a closed reduction of the elbow together with osteosynthesis of the ulna, internal fixation of the scaphoid with a compression screw, and scaphocapitate K wiring. After 26 months, the function of the elbow was satisfa

Reconstruction of proximal pole scaphoid nonunion using medial femoral trochlea osteochondral free flap is indicated in young active patients for whom salvage procedures are not ideal. This article reviews the outcomes data available in the literature and provides a brief report of clinical, radiographic, and patient-reported outcomes for the subset of our patients with greater than 3 years postoperative follow-up. The present literature suggests that medial femoral trochlea reconstruction for proximal pole scaphoid nonunion can restore radiocarpal anatomy, prevent progressive carpal collapse,

PURPOSE: To directly measure strain changes in the scapholunate ligament via magnetic resonance imaging (MRI) when axially loading the wrist in the neutral and extended positions.
METHODS: Six asymptomatic male volunteers without known history of previous wrist injury were enrolled in this MRI-based study. Each subject underwent 3 MRI scans in a 3T scanner: in resting neutral position, in neutral with axial load applied, and in extension with axial load applied. Axial load was applied via extension of an elastic band with known force/elongation curve. We analyzed images and converted them to

PURPOSE: To determine the incidence of associated intrinsic and extrinsic ligament injuries in patients with a nondisplaced or displaced scaphoid waist fracture.
METHODS: During a 3-year period, a study of all scaphoid fractures was performed at our institution. Diagnosis was confirmed by plain radiographs, computed tomography, and magnetic resonance imaging. A 3-part anatomic classification was used to categorize the scaphoid fractures. The study population comprised 40 patients with 41 scaphoid waist fractures who had wrist arthroscopy for treatment and evaluation of the scaphoid fracture a

In a prospective study, 45 patients with fractures of the distal radius and radiologically suspected tears of the scapholunate interosseous ligament were examined. Magnetic resonance imaging was performed prior to wrist arthroscopy. The latter examination gave the definite diagnosis. MRI was performed in conventional technique (without contrast medium) in 25 cases and after additional intravenous injection of contrast medium in the remaining 20 patients. The images were obtained with a 1.0-T clinical imager using a T(2)-weighted turbo spin echo sequence (slice 3 mm, transversal) and a FLASH 2D

INTRODUCTION: Dynamic CT imaging is a promising modality for evaluating wrist pathologies like scapholunate ligament (SL) injuries. The primary objective of this study is to extract carpal angles from dynamic CT datasets using an automated motion analysis algorithm to provide reference values for healthy wrist motion. Secondly, the feasibility of this automatic method to detect SL ligament pathology was evaluated.
METHODS: Dynamic CT scans of healthy wrists and wrists with arthroscopically-confirmed complete SL injuries (Geissler IV) were analysed. Each scan consisted of one static image and

The purpose of this study was to assess the clinical and radiological outcomes of dorsal intercarpal ligament capsulodesis for the treatment of static scapholunate instability at a minimum follow-up of four years. A total of 59 patients who underwent capsulodesis for this condition were included in a retrospective analysis after a mean of 8.25 years (4.3 to 12). A total of eight patients underwent a salvage procedure at a mean of 2.33 years (0.67 to 7.6) and were excluded. The mean range of extension/flexion was 88° (15° to 135°) and of ulnar/radial deviation was 38° (0° to 75°) at final follo

BACKGROUND: Traditionally, acute nondisplaced scaphoid fractures have been treated nonoperatively in a cast, and the expected union rate approaches 90%. Internal fixation of nondisplaced scaphoid fractures has increased in popularity, and a union rate of 100% has been reported. The growing trend is to recommend internal fixation for the majority of acute scaphoid fractures. The true long-term benefits of this more complicated treatment modality have not yet been determined in randomized controlled trials. The purpose of this study was to compare the long-term results of operative fixation of a

CASE: We present the case of a patient who developed scaphoid avascular necrosis in the setting of scaphoid hypoplasia, 37 years after an index finger repositioning for thumb agenesis, due to Holt-Oram syndrome.
CONCLUSION: To the best of our knowledge, this is a unique case of scaphoid avascular necrosis in a patient with Holt-Oram syndrome with scaphoid hypoplasia who underwent index finger repositioning 37 years earlier. Her wrist pain was successfully treated with scaphoid excision and intercarpal fusion.

OBJECTIVE: This prospective study investigated the sensitivity of cone beam computed tomography (CBCT), a low dose technique recently made available for extremity examinations, in detecting scaphoid fractures. Magnetic resonance imaging (MRI) was used as gold standard for scaphoid fractures.
MATERIALS AND METHODS: A total of 95 patients with a clinically suspected scaphoid fracture were examined with radiography and CBCT in the acute setting. A negative CBCT exam was followed by an MRI within 2 weeks. When a scaphoid fracture was detected on MRI a new CBCT was performed.
RESULTS: Radiography
