The response to endotoxin in guinea pigs after intravenous black currant seed oil.

The influence on the metabolic response to endotoxin of three days of total parenteral nutrition with lipids high in gammalinolenic acid (18:3 omega 6, GLA) compared to soy oil (SO) was examined in acute operatively stressed guinea pigs. GLA is the precursor of dihomogammalinolenic acid (DHLA), the substrate for synthesis of "1" series prostaglandins such as PGE1, which have previously been shown to be protective in endotoxin lung injury and traumatic shock. Guinea pigs fed an intravenous diet containing black currant seed oil (BCO) emulsion (20% GLA) or soy oil emulsion (0% GLA) for 2.5 days

[Effect of epidural morphine anesthesia on NO-ergic activity of nociceptive neurons of spinal ganglia and spinal cord].

The effect of epidural morphine anesthesia on NO-ergic mechanisms was studied during primary and secondary sensitization. Nitro-oxide synthase (NOS) activity was evaluated by the method of Home and Vincent (1989) (identification of NADPH diaphorase). NOS and NADPH diaphorase are two double enzymes in cells with synchronously changing activity. NOS activity corresponds to the level of NO secretion. The enzyme was measured on a microdensitometer. The findings evidence active involvement of NO-ergic mechanisms in the formation of primary and secondary hyperalgesia, and injection of morphine cause

Impact of presenting physiology and associated injuries on outcome following traumatic rupture of the thoracic aorta.

We hypothesized that the predominant factor influencing outcome of traumatic rupture of the thoracic aorta (TRA) was the degree of shock on presentation and associated injuries. We reviewed our experience with TRA over a 15-year period. Patients were classified as "unstable" if presenting systolic blood pressure was <90 mm Hg or if it decreased to <90 mm Hg after admission. We determined the presence of closed head injury, cardiac risk factors, a preoperative acute lung injury (ALI). The influence of these factors on mortality, postoperative adult respiratory distress syndrome (ARDS), and para

Extensive wound excision in shock stage in patients with major burns.

To stop excessive plasma loss, alleviate noxious effects of devitalized tissues on the body and shorten the hospitalization time, we performed extensive escharectomy during the shock period in extensively burned patients. Group A consisted of 21 patients aged 9-45 years (26.1 +/- 7.9 years), with a mean total burn area of 63.2% +/- 18.1% TBSA, and full-thickness injury involving 35.9% +/- 19.6% TBSA. The first escharectomy was done at 24.1 +/- 13.9 hours postburn, and excision area averaged 32.3% +/- 6.7% TBSA (24%-46%). In 15 of them, Swan-Ganz catheter was introduced to monitor the hemodynam

[Functional methods of examination of the victims of the earthquake with crush syndrome in the Armenian S.S.R].

The authors have developed a complex of measures including the known (tetrapolar rheoplethysmography, liver and kidney radiography) and the new tests (soft tissue scintigraphy) that have been used for the first time in this condition. Employment of these tests for the examination and monitoring the treatment of patients who were injured in the earthquake in Armenia has helped quantitatively assess the structural and functional shifts in the vital organs and injured limbs in the crush syndrome, thus being valuable for the organization and monitoring the efficacy of therapy and for predicting th

[Characteristics of the diagnosis and prevention of pulmonary complications of severe cranio-cerebral injuries].

Under study was the clinico-roentgenological dynamic of the lung state in 79 patients with severe cranio-cerebral trauma. High continuous peridural blocade performed from the moment of admission to the hospital was shown to prevent the development of hard reactive alterations in the lungs. This method however fails to control acute severe aspiration pneumonia.

The diagnosis of intra-abdominal injury in patients with cervical cord trauma.

A review of the records of 288 patients sustaining blunt cervical column and/or cord injuries revealed that twelve (4.2%) had significant intra-abdominal injuries, all occult, and all detected by peritoneal lavage. Three of 58 patients in shock (BP less than 100 mm Hg) with neurologic deficits were found to have intra-abdominal injuries. Shock in another 15 was the result of major associated injuries and/or the loss of sympathetic vascular tone. Thus 40 of these 58 patients (69%) had neurogenic shock. An analysis of the mechanisms of injury and associated injuries indicated that those at risk

Hemorrhagic, hypovolemic shock resuscitated with Ringer's solution using bicarbonate versus lactate: A CONSORT-randomized controlled study comparing patient outcomes and blood inflammatory factors.

BACKGROUND: Isotonic crystalloids are the preferred solution for the initial clinical management of patients with multiple trauma, among which lactated Ringer's solution and normal saline are the most widely used, but both have clinical limitations. Bicarbonated Ringer's solution (BRS), which provides physiological levels of bicarbonate ions and electrolyte ions, can be used to supplement missing extracellular fluid and correct metabolic acidosis. METHODS: A prospective, randomized controlled study enrolled 63 patients with traumatic hepatic rupture and hemorrhagic shock. They were randomly a

Fresh frozen plasma resuscitation attenuates platelet dysfunction compared with normal saline in a large animal model of multisystem trauma.

BACKGROUND: Platelet dysfunction following trauma has been identified as an independent predictor of mortality. We hypothesized that fresh frozen plasma (FFP) resuscitation would attenuate platelet dysfunction compared with 0.9% normal saline (NS). METHODS: Twelve swine were subjected to multisystem trauma (traumatic brain injury, liver injury, rib fracture, and soft tissue injury) with hemorrhagic shock (40% of estimated blood volume). Animals were left in shock (mean arterial pressure, 30-35 mm Hg) for 2 hours followed by resuscitation with three times shed volume NS (n = 6) or one times vo

mtHsp70 overexpression enhances neuroprotection through ATF5-dependent UPRmt signaling after traumatic brain injury.

Traumatic brain injury (TBI) induces secondary neuronal damage, in which mitochondrial dysfunction plays a central role. Mitochondrial heat shock protein 70 (mtHsp70) is a key mitochondrial chaperone involved in protein folding and proteostasis, yet its role in TBI pathology remains unclear. In the present study, we investigated the neuroprotective function of mtHsp70 and its underlying mechanisms using a controlled cortical impact (CCI) mouse model. We found that CCI selectively reduced mtHsp70 levels within mitochondria, accompanied by its cytoplasmic accumulation, while total cellular mtHsp

Emergency treatment and early fluid resuscitation following electrical injuries.

Injuries caused by high-tension electrical current are rare, but pathophysiologically unique with destructive effects. As a form of thermal trauma, electrical injuries represent a connection of skin burns and deep tissue destruction unpredictable in its depth which mostly resembles a crush injury. Emergency treatment measures begin with the separation from the electrical contact if any and prompt transport to the nearest institution with all means for cardiorespiratory resuscitation and complete recovery. Resuscitation of the patient after electric shock continues then with fluid replacement u

Acute posttraumatic acalculous cholecystitis.

A series of 18 patients who had acute posttraumatic acalculous cholecystitis over a 12 year period was presented. An attempt was made to determine the etiologic factors involved in the pathogenesis of the disease. Large amounts of parenteral narcotics administered over a prolonged period were evident in all patients. Narcotic-induced biliary stasis appeared to be the prime factor involved in the genesis of acalculous cholecystitis after trauma. Other factors such as the presence of shock, respiratory failure, acute renal failure, parenteral hyperalimentation, and multiple transfusions were les
