Evaluation of Implementation of Newborn Screening for Sickle Cell Disease Program in Selected Hospitals in Dar es Salaam, Tanzania.

Sickle cell disease (SCD) is a major public health concern in Tanzania where approximately 11,000 children are born with the condition annua … two hospitals. Seventy-four (0.70%) newborns had homozygous SCD (HbS/S), whereas 1325 (12.53%) had sickle cell trait (HbA/S). Enrolment of infants diagnosed with SCD into comprehensive care declined substantially over time, from 65.6% in 2022 to 10.5% in 2024 at Temeke RRH, while Amana RRH recorded no enrolments beyond the first year of implementation. Qualitative …

Health education to promote knowledge about sickle cell disease and newborn screening in pregnant women: a community-based pilot study using the healthy beginning initiative.

BACKGROUND: Pregnancy presents a critical period for any maternal and child health intervention that may impact the health of the newborn. W … st ~ 23.6%) understood that the chance of conceiving a child with SCD was 25% for a couple with the sickle cell trait (SCT). Knowledge of the possibility of diagnosing SCD shortly after birth was highly increased in the post test phase of the study when compared to the pre-test phase (93.3% vs. 43.9%, respectively). Concerning the overall knowledge scores, those wi …

Assessing knowledge of sickle cell disease and health beliefs on premarital genetic screening among healthcare trainees at a tertiary institution: A cross-sectional study.

BACKGROUND: The uptake of sickle cell trait (SCT) test is challenged by several factors. A community of healthcare professionals educating the public to undergo screening is critical in reducing the disease burden. We investigated knowledge and attitude towards premarital SCT screening among healthcare trainee students who are the next generation of healthcare practitioners. METHODS: A cross-sectional design was employed, and quantitative data were collected from 451 female students pursuing healthcare programs at a tertiary institution in Ghana. Descriptive, bivariate, and multivariate logis…

Emergency Department Health Care Utilization and Opioid Administration Among Pediatric Patients With Sickle Cell Vasoocclusive Pain Crisis and Coexisting Mental Health Illness.

OBJECTIVES: To determine whether patients with sickle cell disease (SCD) who present to the emergency department (ED) with vasoocclusive pain crises (VOC), and have coexisting mental health (MH) diagnoses, are more likely to have increased health care utilization and more frequent opioid administration compared with those without coexisting MH conditions. METHODS: This is a retrospective study of patients aged 5 to 18 years with SCD who presented to a tertiary care ED with a primary complaint of VOC between January 1, 2013, and December 31, 2017. We excluded patients with sickle cell trait an…

[Hemoglobinopathies and glucose-6-phosphate dehydrogenase deficiency in the sub-Saharan immigrant population of the Center and South Maresme region, Catalonia, Spain].

BACKGROUND AND OBJECTIVE: Prevalence of hemoglobinophaty S and glucose-6-phosphate dehydrogenase (G6PD) deficiency are very increased in certain zones of Western Africa due to the positive pressure exerted by malaria on these congenital defects. In Maresme, a region of East Catalonia, there is a numerous community of African people, coming from areas of Western Africa where sickle cell trait and G6PD deficiency are frequent. The aim of our study was to know the prevalence of both disturbances in this population. POPULATION AND METHOD: We studied 204 individuals of black ethnical background wh…

Beta thalassaemia in Nigeria: myth or fact?

BACKGROUND: The high prevalence of sickle cell disorders and the mild deletional á-thalassaemia among Nigerians is well known, but â-thalassaemia is believed to be almost nonexistent. Beta thalassaemia trait (BTT) was screened for in patients with unexplained recurrent haemolytic anaemia and healthy individuals. METHODS: â-thalassaemia trait (BTT) was screened for using MCH and HbA2 of 27 pg and 3.5% respectively as cut off in 151 Nigerians which included 29 patients and 122 apparently healthy individuals. The subjects were categorized into four, Group I (high HbA2, low MCH), Group II (low Hb

Aspects of the distribution of HbS in the United States.

Frequencies of HbS obtained by several screening clinics are analyzed for age, sex, and location effects. All seem to be present in some form, though age and sex effects may be conditional on location. An attempt is made to elaborate the common observation of increasing frequency with age. This is shown to be the result of differences in fertility favoring the normal. A simulation which includes 25% admixture was done. The results indicate a genetically relevant New World experience for the population to be about 9--12 generations with the heterozygote having fitness of 0.96--0.99.

Fetal hemoglobin in sickle cell anemia: molecular characterization of the unusually high fetal hemoglobin phenotype in African Americans.

Fetal hemoglobin (HbF) is a major modifier of disease severity in sickle cell anemia (SCA). Three major HbF quantitative trait loci (QTL) are known: the Xmn I site upstream of (G)γ- globin gene (HBG2) on chromosome 11p15, BCL11A on chromosome 2p16, and HBS1L-MYB intergenic polymorphism (HMIP) on chromosome 6q23. However, the roles of these QTLs in patients with SCA with uncharacteristically high HbF are not known. We studied 20 African American patients with SCA with markedly elevated HbF (mean 17.2%). They had significantly higher minor allele frequencies (MAF) in two HbF QTLs, BCL11A, and HM

[Leprosy: a paradigm for the study of human genetic susceptibility to infectious diseases].

Fifty years ago, the first identification of a non Mendelian genetic contribution to the development of a common infectious disease, i.e. the association between malaria and sickle-cell trait, was shown using a supervised approach which tests a limited number of candidate genes selected by hypothesis. Since then, the few genes that were convincingly associated with susceptibility to human infectious diseases were identified following the same strategy. The study of leprosy has contributed to modifying this way of thinking. In the absence of a satisfying experimental model and because of the im

Glucose-6-phosphate dehydrogenase deficiency: an etiology for idiopathic priapism?

INTRODUCTION: Efforts to identify the health risk associations for priapism may reveal pathophysiologic mechanisms for the disorder and suggest a scientifically rational approach for correcting it. AIM: We describe a clinical presentation of idiopathic recurrent priapism in a patient with glucose-6-phosphate dehydrogenase (G6PD) deficiency and consider a possible nitric oxide (NO)-dependent mechanistic basis from which the medical condition causes priapism. METHODS: The case report profiled a 35-year-old African-American man with G6PD deficiency who presented with a rapid progression of recu

Decreased placental and umbilical cord glycogen levels associated with meconium-stained amniotic fluid.

Most frequently, placental glycogen has been studied as an index of fetal nutrition. There are no published studies of placental glycogen as … ction, obesity, diabetes mellitus, premature rupture of membranes, intrauterine growth retardation, sickle cell trait, and acute meconium staining of amniotic fluid at delivery. The data showed that the only significant differences were in the subgroup complicated by meconium-stained amniotic fluid in which the placental disks and umbilical cords had significantly …

Phenylketonuria, congenital hypothyroidism and haemoglobinopathies: public health issues for a Brazilian newborn screening program.

In this study, the frequency of detected congenital hypothyroidism, phenylketonuria and haemoglobinopathies in the State of Rio de Janeiro's (Brazil) Newborn Screening Program (NBSP) was analyzed between the years of 2005 and 2007. There were two Newborn Screening Reference Centers (named NSRC A and B) with programmatic differences. In 2007, overall detection coverage reached 80.7%. The increase in the incidence of congenital hypothyroidism (1:1,030 in 2007) was attributed to the reduction of neonatal TSH value limits over time. The incidence discrepancy of phenylketonuria between NSRC A (1:28

Haemoglobin variants, beta-thalassaemia and G-6-PD types in Liberia.

Haemoglobin variants, beta-thalassaemia and glucose-6-phosphate dehydrogenase (G-6-PD) types were studied in 702 individuals from Buchanan, Liberia. In this population haemoglobins S and C, beta-thalassaemia and G-6-PD deficiency were found together. There was a considerable tribal variation. In the tribes of eastern Liberia the S- and C-genes were uncommon and the beta-thalassaemia gene was rather frequent, while in western Liberia the S- and C-genes were more frequent and beta-thalassaemia uncommon. In the central and northern parts the S- and beta-thalassaemia genes were found together in r
