Multiple sclerosis (MS) is an inflammatory demyelinating disease of the central nervous system. The disability caused by inflammatory demyelination clinically dominates the early stages of relapsing-remitting MS and is reversible. Once there is considerable loss of axons, MS patients enter a secondary progressive stage. Disease-modifying drugs currently in use for MS suppress the immune system and reduce relapse rates but are not effective in the progressive stage. Posterior facet arthritis is rarely described. The authors report a case of arthritis of the posterior lumbar facet in a woman consulting for back pain associated with relative functional impairment. Modern imaging, in slices, enables better examination of the posterior arch and better diagnosis of pathologies. PURPOSE: We present a retrospective study of 15 cases with severe posttuberculous kyphosis of thoracolumbar region that underwent posterior vertebral column resection.

We reviewed 64 anterolateral decompressions performed on 63 patients with tuberculosis of the dorsal spine (D1 to L1). The mean age of the patients was 35 years (9 to 73) with no gender preponderance. All patients had severe paraplegia (two cases grade III, 61 cases grade IV). The mean number of vertebral bodies affected was 2.6; the mean pre-treatment kyphosis was 24.8 degrees (7 to 84). An average of 2.9 ribs were removed in the course of 64 procedures. The mean time taken at surgery was 2.45 hours when two ribs were removed and 3.15 hours when three ribs were removed. Twelve patients (19%)

The results of treatment in 50 patients with tuberculosis of the thoracic and lumbar spine are presented. The routine surgical treatment was used in 32 cases. In 11 patients, the use of an extrafocal external fixation apparatus made it possible to achieve the axis and height of the diseased portion of the vertebral column and to start early postoperative activation.

A 29-year-old woman, admitted with a left thigh abscess, was subsequently found to have tuberculosis of the spine. The disease was complicated by fistulous involvement of the sigmoid colon which led to bowel perforation and dissection of the resulting retroperitoneal abscess into the thigh. The clinical presentation was that of a polymicrobial soft tissue abscess, with the tuberculous etiology delineated only on further evaluation.

One hundred and three patients with spondylitis tuberculosa were treated with 104 vertebrotomies, without deaths. The results of 71 cases, one until 24 years after the operation, were evaluated. Most of them showed bony healing, only one had a soft tissue rezidivism. Through the modern antituberculous therapy the time of immobilization (plaster-bed and cast) could have been shortened in the last 13 years from 38,4 to 20,3 months.
