Usefulness of R2* maps generated by iterative decomposition of water and fat with echo asymmetry and least-squares estimation quantitation sequence for cerebral artery dissection.

INTRODUCTION: Acute intramural hematoma resulting from cerebral artery dissection is usually visualized as a region of intermediate signal intensity on T1-weighted images (WI). This often causes problems with distinguishing acute atheromatous lesions from surrounding parenchyma and dissection. The present study aimed to determine whether or not R2* maps generated by the iterative decomposition of water and fat with echo asymmetry and least-squares estimation quantitation sequence (IDEAL IQ) can distinguish cerebral artery dissection more effectively than three-dimensional variable refocusing f

A novel atlantoaxial subluxation fixation technique combining transarticular screws with transverse rod and Gallie's wires: A report of two cases.

The atlantoaxial segment of the cervical spine is unique for its distinctive anatomical features. Given the variation in the anatomy of this region, surgeons must possess a profound knowledge of its anatomy to be able to combine multiple surgical approaches tailored to the specific anatomical variants of the patient's condition. Insertion of C1 lateral mass screws can be technically demanding, particularly in cases where dissection leads to iatrogenic injury of the C1/C2 venous sinus and causes significant bleeding or in patients with dysplastic C1 anatomy or vertebral artery anomalies. Gallie

Opalski syndrome: a rare variant of Wallenberg syndrome: a case report and literature review.

BACKGROUND: Opalski syndrome is a rare variant of Wallenberg syndrome (lateral medullary syndrome) that demonstrates concomitant ipsilateral hemiplegia due to infarctions within the lateral medulla and the cervical spinal cord, which also extend to the post-pyramidal decussation to affect the corticospinal tract. CASE PRESENTATION: A 56-year-old man initially presented with a unilateral headache with right cervical pain. Consequently, he developed symptoms that indicate Wallenberg syndrome: vertigo, dysphonia, dysarthria, right limb ataxia with a tendency to fall, and ptosis, in addition to i

A Contralateral Transcondylar Fossa Approach with Bilateral V3 Segment Exposure for Repairing Complex Vertebral Artery Aneurysms.

BACKGROUND AND IMPORTANCE: Microsurgical treatment for vertebral artery aneurysms can be approached through the lateral aspect of the brainstem and cerebellum. A contralateral approach may be selected in complex aneurysms with tortuous running and the intracranial vertebral artery distal to the aneurysm located in the contralateral cerebellopontine angle. When repairing the aneurysm, exposing the V3 segment before craniotomy is advantageous. We describe the detailed surgical procedures of the contralateral transcondylar fossa approach with bilateral V3 segment exposure for the repair of a comp

Atlanto-occipital assimilation with arcuate foramen, vertebral artery hypoplasia, and elongated styloid process: a case report.

We encountered a partial type of atlanto-occipital assimilation (AOA), characterized by the fusion of the atlas (C1) with the base of the occiput, in a male cadaver during a student dissection course. The lateral mass and lateral part of the anterior arch of C1 were fused with the corresponding basilar part of the occipital bone on both of the right and left sides. The transverse process of C1 was involved in the fusion on the right side but not on the left side. The atlanto-occipital joint was obliterated on the right side, while it was present on the left side. Middle part of the anterior ar

MDCT and MRI evaluation of cervical spine trauma.

PURPOSE: Cervical spine injuries following major trauma result in significant associated morbidity and mortality. Devastating neurological injury, including complete and incomplete tetraplegia, are common sequelae of cervical spine trauma and cause profound and life-altering medical, financial, and social consequences. Most cervical spine injuries follow motor vehicle accidents, falls, and violence. The proliferation of multidetector computed tomography allows for fast and accurate screening for potential bony and vascular injuries. Magnetic resonance imaging is useful for evaluation of the su

Cervico-thoracic ganglion: its clinical implications.

Lesions of the cervicothoracic ganglion (CTG) result in interruption of sympathetic fibers to the head, neck, upper limb, and thoracic viscera. The accurate understanding of the anatomy of the CTG is relevant to sympathectomy procedures that may be prescribed in cases where conventional intervention has failed. This study documents the incidence and distribution of the CTG to avoid potential complications such as Horner's syndrome and cardiac arrhythmias. This study utilized 48 cadavers, in which a total of 89 sympathetic chains were dissected. The inferior cervical ganglion (ICG) and the firs

Does Variation of the Vertebral Artery (VA) Origin Affect Vertebral Artery Dominance? A Retrospective, Single Centre Cohort Study.

INTRODUCTION: The vertebral arteries (VA), usually arising from the subclavian arteries, supply the posterior circulation of the brain. Anatomical variations in VA origin occur in up to 5.8% of cases, most commonly with direct aortic origin of the left VA. As paired vessels, the sizes of VAs are often mismatched, a phenomenon known as VA dominance. This results in unequal haemodynamic stress, which has been associated with increased risks of atherosclerosis, posterior circulation strokes and vertebrobasilar dissection. Despite their recognised clinical significance, limited studies have examin

Age-related morphologic changes of the vertebral artery in the transverse process. Analysis by multidetector computed tomography angiography.

BACKGROUND CONTEXT: The V2 segment of the vertebral artery (VA) ascends and passes through the transverse foramen (TF) of the C6-C1 vertebrae. Atherosclerosis of the VA and degenerative changes in the cervical spine are likely to occur with aging, and subsequent morphologic changes may alter the normal anatomy. PURPOSE: The aim was to determine the morphologic changes of TF and VA in relation to aging. STUDY DESIGN/SETTING: This was a retrospective cross-sectional study. PATIENT SAMPLE: One hundred ten consecutive patients who had undergone computed tomography angiography were included. ME

High incidence and clinical characteristics of fibromuscular dysplasia in patients with spontaneous cervical artery dissection: The ARCADIA-POL study.

The association between fibromuscular dysplasia (FMD) and spontaneous cervical artery dissection (SCeAD) has been recognized, but the available evidence on this relationship is scant. Therefore, the main goal of our study was to systematically evaluate FMD frequency, clinical characteristics and vascular bed involvement in patients with SCeAD. Among 230 patients referred to the ARCADIA-POL study, 43 patients (mean age 44.1 ± 8.9 years; 15 men and 28 women) with SCeAD were referred. Also, 135 patients with FMD were compared to patients with and without SCeAD. Patients underwent: ambulatory bloo
