Asymmetry and tuning shift of the cervical vestibular evoked myogenic potential indicate saccular dysfunction in idiopathic normal pressure hydrocephalus.

OBJECTIVE: The purpose of this study was to investigate the effects of excessive cerebrospinal fluid (CSF) retention on the peripheral vestibular function and the inner ear fluid in patients with idiopathic normal pressure hydrocephalus (iNPH). METHODS: In 25 patients with iNPH (14 females, age 65-88 years), cervical vestibular evoked myogenic potential (cVEMP) was measured before the spinal tap test. The asymmetry ratios (ARs) and tuning properties in 500 Hz and 1,000 Hz short-tone burst stimuli of cVEMP were evaluated. Furthermore, cVEMP was measured in an age-matched control group of 12 no

Report of an Otic Capsule Disrupting Fracture of the Temporal Bone: Visualization of Pneumolabyrinth and Functional Assessment.

Blunt trauma of the head can result in temporal bone fracture. Different classifications exist, but from a functional perspective, to distinguish otic capsule sparing and otic capsule disrupting fractures is superior to the classic nomenclature. Disruption of the otic capsule is often associated with sensorineural hearing loss, vestibular dysfunction, cerebrospinal leakage, or even intracranial consequences. Pneumolabyrinth describes the condition when air is enclosed within the inner ear. It is a result of a pathological communication between the labyrinth and the middle ear spaces that often

Impulsive testing of individual semicircular canal function.

In order to test the human angular vestibulo-ocular reflex in the dynamic range of normal head movements, we measured 3-dimensional compensatory eye-movement responses to low-amplitude (10-12 degrees), high-acceleration (3000-4000 degrees/s/s), passive, manually delivered head rotations (head "impulses") in the three planes of the semicircular canals in normal subjects, in subjects who had recovered from surgical unilateral vestibular deafferentation, and in patients after acute unilateral peripheral vestibulopathy, that is, from vestibular "neuritis." We found that canal-plane head impulses a

Assessing the effects of concussion using the C3Logix Test Battery: An exploratory study.

The C3Logix is a computerized concussion assessment tool that measures a wider array of symptoms (i.e., balance and oculomotor functioning) than other computerized batteries. Although the C3Logix has been used increasingly by athletic organizations at all levels of play, its utility within the concussion population has not been extensively examined. The current study aimed to determine whether the C3Logix is sensitive to the effects of concussion. A total of 54 student-athletes enrolled at a large southern university completed the C3Logix at baseline and within days following a suspected concu

Low-frequency harmonic acceleration as a test of labyrinthine function: basic methods and illustrative cases.

These preliminary findings reveal that low-frequency harmonic acceleration can be used to detect unilateral peripheral deficits that agree with caloric findings. However, the responses to sinusoidal acceleration are less variable than the caloric responses and allow one to more closely evaluate changes in pathology with time. Therefore, it appears that harmonic acceleration provides additional information that is useful in confirming other test results and that a complete otoneurologic evaluation should include this type of rotational testing.

Dose-response and recovery trajectories of fall prevention rehabilitation in older patients with vertigo: a retrospective cohort study based on vestibular and gait impairment phenotypes.

BACKGROUND: Vertigo and dizziness are common in older adults and are associated with a significantly increased risk of falls. Fall prevention rehabilitation therapy (FPRT) is recommended, but current prescriptions remain largely empirical and do not account for heterogeneity in baseline vestibular and gait impairments. The dose-response relationship between rehabilitation hours and functional improvement, and how baseline phenotypes are associated with recovery trajectories, are poorly understood. METHODS: This retrospective longitudinal cohort study, conducted in accordance with STROBE guide

Effects of Age on the Bone-Conduction Amplitude-Modulated cVEMP Temporal Modulation Transfer Function.

OBJECTIVES: Cervical vestibular evoked myogenic potentials (cVEMPs) reflect saccular stimulation that results in an inhibitory muscle reflex recorded over the sternocleidomastoid muscle. These responses are utilized to study basic vestibular functions and are also applied clinically. Traditionally, cVEMPs have utilized transient stimuli such as clicks and tonebursts to evoke onset responses. Recently, amplitude-modulated tones have been used to elicit cVEMPs (AMcVEMPs). These AMcVEMP responses can provide information about the magnitude, phase synchrony, and nonlinearities from the vestibulo-c

Evaluation of the caloric test by combining 3 response parameters.

The slow-phase velocity of nystagmus is one of the most sensitive parameters of vestibular function and is currently the standard for evaluating the caloric test. However, the assessment of this parameter requires recording the response by using nystagmography. The aim of this study was to evaluate whether frequency and duration of the caloric nystagmus, as measured by using a clinical test with Frenzel glasses, could predict the result of the recorded test. The retrospective analysis of 222 caloric test results recorded by means of electronystagmography has shown a good association between th

Motoric Cognitive Risk Syndrome: Symptoms, Pathology, Diagnosis, and Recovery.

The motoric cognitive risk (MCR) syndrome is a pre-dementia condition, marked by the enhanced risk for Alzheimer's disease (AD) and vascular dementia, together with falls, disability, and abnormal movements. The research studies revealed the distinct neurological and non-neurological clinical gait irregularities during dementia and accelerated functional decline, such as postural and balance impairments, memory loss, cognitive failure, and metabolic dysfunctions. The disabling characteristics of MCR comprise altered afferent sensory and efferent motor responses, together with disrupted visual,

Evaluation of Dizziness in the Emergency Department: Prevalence and Diagnostic Utility of Clinical Scales for Functional Vertigo.

INTRODUCTION: Functional vertigo is commonly missed in the emergency department (ED) and often misdiagnosed as other peripheral vestibular disorders. It is strongly associated with anxiety and depression, yet standardized diagnostic criteria are lacking in the ED setting, leading to unnecessary tests and misdiagnosis. We aimed to assess the diagnostic accuracy of the Vertigo Symptom Scale - Short Form - Autonomic (VSS-SF-A) and the Hospital Anxiety and Depression Scale - Anxiety (HADS-A) and - Depression (HADS-D) for distinguishing functional vertigo from other peripheral vertigos in the ED an

Positional nystagmus is observed in the vast majority of healthy individuals.

INTRODUCTION: Benign paroxysmal positional vertigo (BPPV) is a vestibular disease characterized by brief  positional vertigo. When examined, characteristic patterns of positional nystagmus (PN) are found with specific head position changes. Previous studies have shown a high prevalence of PN among vestibular healthy subjects. Considering the current diagnostic criteria of BPPV and the potentially high prevalence of PN in healthy individuals, this raises the question of potential over diagnosing BPPV, if diagnostics are based exclusively upon objective findings. This study aims to determine the
