Impact of diabetes mellitus on myocardial function and clinical outcomes in patients with significant aortic regurgitation.

BACKGROUND: Type 2 diabetes mellitus (T2DM) causes myocardial dysfunction and has been linked to an increased risk of unfavorable cardiovascular events. However, the additive effects of T2DM on myocardial function and its association with clinical outcomes in patients with aortic regurgitation (AR) is undetermined. The study aimed to verify whether T2DM aggravates the deterioration of myocardial deformation and clinical outcomes in AR patients. METHODS: A total of two hundred and fifty-five AR patients, differentiated by the presence or absence of T2DM [AR(T2DM+) and AR(T2DM-), respectively],

Mechanisms and Clinical Significance of Arrhythmia-Induced Cardiomyopathy.

Arrhythmia-induced cardiomyopathy (AIC) is characterized by left ventricular systolic dysfunction for which the primary cause is arrhythmia. The hallmark of AIC is its reversibility once the arrhythmia is properly controlled. Any tachyarrhythmia can potentially cause AIC (often called "tachycardiomyopathy"), with atrial fibrillation (AF) being by far the most common in clinical practice. The pathophysiological mechanisms underlying AIC need further clarification, but the available evidence, principally from animal models, implicates metabolic dysfunction due to increased oxygen requirements, n

Predictors of outcome in patients with moderate mixed aortic valve disease.

OBJECTIVES: Grading the severity of moderate mixed aortic stenosis and regurgitation (MAVD) is challenging and the disease poorly understood. Identifying markers of haemodynamic severity will improve risk stratification and potentially guide timely treatment. This study aims to identify prognostic haemodynamic markers in patients with moderate MAVD. METHODS: Moderate MAVD was defined as coexisting moderate aortic stenosis (aortic valve area (AVA) 1.0-1.5 cm2) and moderate aortic regurgitation (vena contracta (VC) 0.3-0.6 cm). Consecutive patients diagnosed between 2015 and 2019 were included

Fetal Cardiac Function at Midgestation in Women Who Subsequently Develop Gestational Diabetes.

IMPORTANCE: Fetuses in women with gestational diabetes (GD) compared with those without GD show evidence of subclinical cardiac functional and morphological changes. However, it is uncertain whether glycemia or the adverse maternal underlying risk factor profile is the main driver for fetal cardiac remodeling. OBJECTIVE: To assess cardiac morphology and function at midgestation in fetuses of mothers prior to development of GD and compare them with those of unaffected controls. DESIGN, SETTING, AND PARTICIPANTS: During this prospective nonintervention screening study at 19 to 23 weeks' gestat

Dapagliflozin Improves Mitochondrial Function to Reduce the Risk of Atrial Fibrillation in a High-Glucose Environment by Activating the AMPK/PGC-1α/SIRT3 Signalling Pathway.

BACKGROUND: Type 2 Diabetes Mellitus (T2DM) and AF (Atrial Fibrillation) frequently coexist, amplifying cardiovascular risks via shared mitochondrial dysfunction involving metabolic dysregulation, oxidative stress, and inflammation. While the SGLT2 inhibitor dapagliflozin (DAPA) demonstrates cardiorenal protection through mitochondrial improvement, its direct mechanistic actions in T2DM-AF patients remain unknown. Our study aimed to elucidate DAPA's mitochondrial regulatory mechanisms in T2DM-AF to break the vicious cycle of mitochondrial dysfunction remodelling, addressing an unmet clinical n

Case Report: Post-Total anomalous pulmonary venous connection pulmonary hypertension - novel treatment using sirolimus and atrial flow regulator implantation.

OBJECTIVE: In patients who have undergone surgical repair of TAPVC and who develop treatment-resistant pulmonary vein stenosis and postcapillary pulmonary hypertension, pre-capillary pulmonary hypertension may also develop as a consequence of arterial remodelling. We present a 3-year-old boy with surgically corrected TAPVC in whom AFR 8   ×   5 was implanted using a 9F delivery system due to end-stage precapillary PH. CASE REPORT: In a male term newborn, obstructive supracardiac TAPVR was diagnosed, and he underwent surgical repair on the third day of life. At three months of age, he was hosp

Clinical utility of the 4S-AF scheme in predicting progression of atrial fibrillation: data from the RACE V study.

AIMS: The recent 4S-AF (scheme proposed by the 2020 ESC AF guidelines to address stroke risk, symptom severity, severity of AF burden and substrate of AF to provide a structured phenotyping of AF patients in clinical practice to guide therapy and assess prognosis) scheme has been proposed as a structured scheme to characterize patients with atrial fibrillation (AF). We aimed to assess whether the 4S-AF scheme predicts AF progression in patients with self-terminating AF. METHODS AND RESULTS: We analysed 341 patients with self-terminating AF included in the well-phenotyped Reappraisal of Atrial

Emerin plays a crucial role in nuclear invagination and in the nuclear calcium transient.

Alteration of the nuclear Ca2+ transient is an early event in cardiac remodeling. Regulation of the nuclear Ca2+ transient is partly independent of the cytosolic Ca2+ transient in cardiomyocytes. One nuclear membrane protein, emerin, is encoded by EMD, and an EMD mutation causes Emery-Dreifuss muscular dystrophy (EDMD). It remains unclear whether emerin is involved in nuclear Ca2+ homeostasis. The aim of this study is to elucidate the role of emerin in rat cardiomyocytes by means of hypertrophic stimuli and in EDMD induced pluripotent stem (iPS) cell-derived cardiomyocytes in terms of nuclear

Bradycardia in Athletes: Prevalence, Mechanisms, and Risks.

BACKGROUND: Sinus bradycardia is a well-recognized physiological adaptation in endurance athletes, primarily attributed to sinus node remodeling or increased vagal modulation. Although genetic influences on resting heart rate (HR) have been observed, the genetic contribution to athletic bradycardia has not been elucidated. METHODS: We phenotyped current and former elite endurance athletes in the Pro@Heart cohort study using multimodal cardiac imaging, cardiopulmonary exercise testing, and Holter monitoring. Genetic susceptibility to bradycardia was assessed using a validated HR-associated pol

Right ventricular remodeling after veno-venous extracorporeal membrane oxygenation bridge to lung transplantation in adults with severe lung disease.

BACKGROUND: Right ventricular (RV) dysfunction is a common complication of advanced lung disease, yet data on RV remodeling and long-term outcomes following veno-venous extracorporeal membrane oxygenation (VV-ECMO) as a bridge to lung transplantation remain limited. METHODS: We conducted a retrospective cohort study of consecutive adult patients bridged to lung transplant on VV-ECMO from January 1, 2015, to December 31, 2022. Transthoracic echocardiograms were analyzed pre-transplant (on VV-ECMO) and post-transplant in the "early" ( ≤ 3 months) and "late" (3-12 months) phases. Within-patient

Echocardiographic characteristics of new-onset methamphetamine-associated heart failure: a retrospective cohort study.

BACKGROUND: Methamphetamine-associated heart failure (MAHF) is a growing cause of healthcare burden, yet the echocardiographic phenotype at the time of initial diagnosis is poorly characterized. We tested the hypothesis that new-onset MAHF exhibits a distinct echocardiographic phenotype that could aid in early disease identification. METHODS: We conducted a retrospective cohort study comparing echocardiographic parameters in patients with new-onset MAHF versus controls with non-ischemic cardiomyopathy unrelated to methamphetamine use. Patients with MAHF (n = 75) were identified by positive ur

Biomarkers in heart failure.

Heart failure (HF) is a leading cause of morbidity and mortality worldwide, characterized by a complex pathophysiology and heterogeneous clinical trajectories. Circulating biomarkers have progressively moved from ancillary diagnostic tools to indispensable instruments that inform diagnosis, prognostication, and therapeutic guidance. B-type natriuretic peptide (BNP) and N-terminal pro-B-type natriuretic peptide (NT-proBNP) remain the cornerstones of biomarker-guided management, yet their interpretation is influenced by age, renal function, obesity, and atrial fibrillation. Cardiac troponins, or

ANGPTL2 and LTBP2: key mediators of inflammation and mitochondrial dysfunction in atrial fibrillation.

BACKGROUND: Atrial fibrillation (AF) is a common arrhythmia characterized by structural and electrophysiological remodeling. Evidence suggests that inflammation and mitochondrial dysfunction are key pathophysiological drivers, but the core genes underlying these processes remain unclear. METHODS: Using transcriptomic data from public datasets (GSE2240, GSE108660, and GSE62871), key genes associated with inflammation and mitochondrial dysfunction were identified through WGCNA, differential gene expression analysis, and protein-protein interaction analysis. Subsequently, GSEA was performed to id

Tricuspid Regurgitation Peak-to-Mean Velocity Ratio: Linking Doppler Morphology to Right Heart Hemodynamics in Pulmonary Hypertension.

BACKGROUND: The peak-to-mean tricuspid regurgitation velocity ratio (Vmax/Vmean) is a quantitative descriptor of continuous-wave Doppler waveform morphology. Its physiologic and clinical significance in pulmonary hypertension (PH) remains incompletely characterized. We investigated the relationship among Vmax/Vmean, right heart structure and function, and clinical outcomes in patients with precapillary PH. METHODS: This single-center cohort study included 203 patients with precapillary PH. Baseline echocardiography was performed near the time of right heart catheterization, with repeat echoca

Differential Biventricular Strain Recovery With Selective Fibrosis Biomarker Regression After Sleeve Gastrectomy: A Prospective CMR Feature-Tracking Study.

PURPOSE: Obesity injures the heart through hemodynamic, fibrotic, and adipokine pathways, but whether these reverse together or on different timelines after bariatric surgery is unknown. We performed, to our knowledge, the first integrated assessment of biventricular and biatrial cardiovascular magnetic resonance feature-tracking (CMR-FT), fibrosis and adipokine biomarkers, cardiovascular risk, and quality of life after laparoscopic sleeve gastrectomy (LSG), testing whether recovery is uniform or uneven. MATERIALS AND METHODS: In this single-center prospective cohort, 40 patients underwent LS

Preprocedural Three-Dimensional Vena Contracta Area in Ventricular Secondary Mitral Regurgitation: Association with Outcome after Mitral Valve Intervention.

OBJECTIVE: Increasing effective regurgitant orifice area (EROA) has been associated with poor outcomes in patients with ventricular secondary mitral regurgitation (VSMR). However, measuring EROA with two-dimensional (2D) echocardiography has important limitations. Overcoming most of these limitations, three-dimensional (3D) color Doppler transesophageal echocardiography allows direct assessment of the vena contracta area (VCA), whose prognostic value, however, has not been specifically investigated in patients with VSMR undergoing mitral valve intervention. METHODS: A total of 218 patients (6

Rheumatic heart disease is not over: Cardiac cirrhosis and multivalvular sequelae in an endemic setting - A case series and review.

BACKGROUND: Rheumatic Heart Disease (RHD) remains a major cause of valvular morbidity in endemic regions despite declining incidence in high-income countries. While early disease manifests as acute rheumatic fever, contemporary presentations increasingly reflect advanced structural sequelae. Congestive hepatopathy and cardiac cirrhosis secondary to chronic right-sided failure remain under-recognized manifestations of advanced rheumatic valvular disease. OBJECTIVES: To describe three cases of advanced rheumatic multivalvular disease presenting predominantly with ascites and hepatic congestion

Case Report: Concurrent atrial and ventricular septal defect in a young Sphynx cat.

Concurrent atrial septal defect (ASD) and ventricular septal defect (VSD) are rare in cats. A 7-month-old intact female Sphynx cat was presented for evaluation of respiratory distress and exercise intolerance. Thoracic radiographs showed generalized cardiomegaly and pulmonary infiltrates, consistent with cardiogenic pulmonary edema. Transthoracic echocardiography revealed an ostium secundum-type ASD and a perimembranous VSD, with structurally normal atrioventricular valves and annuli. These features supported the diagnosis of concurrent ASD and VSD, rather than an atrioventricular septal defec
