Osteosynthesis and hemiarthroplasty of fractures of the proximal humerus: outcomes in a consecutive case series.

HYPOTHESIS: This study addresses the outcome after osteosynthesis or hemiarthroplasty, using a cohort of patients that was enrolled in a previous prospective study on humeral head perfusion and was consequently treated using a common conceptual approach. MATERIALS AND METHODS: Between 1998 and 2001, 98 patients with 100 fractures of the proximal humerus were treated surgically by a single surgeon with open reduction and internal fixation (ORIF) (51/100, group A, median age 54 years; range, 21-88) or with hemiarthroplasty (49/100, group B, median age 66 years; range, 38-87). Seventy-six of 98

Experience with the AO Pi-plate for displaced intra-articular fractures of the distal radius.

We have used open reduction and internal fixation (ORIF) with the AO pi-plate in 32 displaced, intra-articular fractures of the distal radius. The indication was a displaced intra-articular fracture with a step-off of more than 1 mm and a gap between fragments of more than 3 mm, judged from the primary computed tomograms (CT). All fractures were classified as AO type C3. Twenty-nine patients were followed-up after a mean of 23 (9-46) months. The dorsal tilt, the radial length, the radial inclination, the articular step-off, and the intra-articular gap between fragments were substantially impro

A machine learning model for predicting postoperative complication risk in young and middle-aged patients with femoral neck fractures.

OBJECTIVE: Femoral neck fractures are the most common type of hip fracture, and the postoperative complications associated with these fractures significantly affect patients' quality of life and healthcare costs. The objective of this study was to develop a predictive model using machine learning (ML) techniques to assess the risk of postoperative complications in young and middle-aged patients with femoral neck fractures. METHODS: We retrospectively analyzed data from 899 young and middle-aged patients with femoral neck fractures who underwent surgical treatment between September 2019 and Ju

Is CT-A Always Reliable in Detecting Active Bleeding in Closed Pelvic Fractures? Management of a Case with Multiple Closed Pelvic Fratures and Internal Iliac Artery Bleeding.

One of the most important complications of pelvic injuries is hemorrhage which can be attributed to the venus plexus of the pelvis, the damaged bone on the fracture site, or in 15% of cases to arterial cause. In the last case mortality could reach 70%. Clinical case presentation, a 77-year-old man, presented in the emergency department of our hospital hemodynamically unstable due to fall from height (3 meters) with comminuted bilateral fractures of the pubic rami, right sacral and iliac wing fracture, right acetabular fracture, fractures of transverse processes of the first, second, and fifth

The effect of medial calcar support on proximal humeral fractures treated with locking plates.

BACKGROUND: Studies have reported mixed results on the importance of medial calcar support for the treatment of proximal humeral fractures. The purpose of this study was to compare radiographic and functional outcomes of patients who had displaced proximal humeral fractures with varying levels of medial support. METHODS: We performed a retrospective comparative cohort study. The study was conducted at a Level III trauma center in Taiwan. Seventy patients with proximal humeral fractures were collected retrospectively from 2015 to 2019. Only patients with two-, three-, or four-part types (Neer

The Etiology and Pathogenesis of Kienböck Disease.

Kienböck disease is a condition that typically occurs in the "at-risk" patient, in the "at-risk" aspect of the proximal condyle of the "at-risk" lunate. In the active male, repetitive loading causes the stress fracture that commences in the single layer proximal subchondral bone plate. The lunate fracture commences at the point the lunate cantilevers over the edge of the distal radius, and then takes on the shape of the radius. We postulate that the stress fracture violates the parallel veins of the venous subarticular plexus-leading to localized venous hypertension and subsequent ischemia and

Management of Proximal Tibia Fractures Using Wire Based Circular External Fixator.

INTRODUCTION: Management of high grade proximal tibia or tibial plateau fractures is often associated with complications. The use of wire fixators for the definitive treatment of such fractures entails a minimally invasive technique of insertion that gives good fracture reduction and stability combined with minimal postoperative complications. AIM: To assess the outcome of treatment of such fractures by the use of Joshi's external stabilization system (JESS), which is a wire based, circular external fixator system. MATERIALS AND METHODS: A prospective, uncontrolled study was done using JESS

Updated German National S1 Guideline on Pediatric Lower Leg Shaft Fractures.

Fractures of the diaphysis of the lower leg account for approximately 6% of all fractures in children and adolescents. A distinction must be made between isolated tibial shaft fractures, which are primarily managed conservatively, and combined lower leg shaft fractures (involving both the tibia and fibula). The latter are significantly less stable and therefore more frequently treated with osteosynthesis. If the fracture is non-displaced or tolerably displaced, immobilization in a long leg cast for about 4 weeks is sufficient. This requires that no secondary displacement occurs within the cast

Functional and radiographic outcomes after surgical management of complex proximal ulna fractures: a retrospective case series.

BACKGROUND: Complex proximal ulna fractures (including comminuted olecranon fractures and fracture-dislocations of the elbow) pose challenging management due to concomitant injuries to key stabilizers (radial head, coronoid process, and collateral ligaments). This study evaluates functional and radiographic outcomes following surgical treatment and explores the prognostic value of associated injuries, age stratification, and a new coronoid-centric classification system. METHODS: We retrospectively reviewed 43 patients with complex proximal ulna fractures treated between 2019 and 2024, with a

[Open posterior dislocation of the elbow with fractures of the radial head and coronoid process and multiple diaphyseal fractures of the ulna].

A healthy, right-handed 34-year-old man was injured by repeated direct blows delivered to his left upper arm with a baseball bat. These blows led to a posterior dislocation of the elbow joint, associated with fractures of the radial head and the coronoid process. There was bone loss on the fractured joint surface of the olecranon, with simultaneous ipsilateral trauma to the ulna. The correct surgical approach in the case of such fractures is still the subject of some controversy. The treatment can have serious complications. The results of treatment are often poor, especially because of persis

A technique for extraction of the distal segment of a broken femoral nail using a flexible reamer.

INTRODUCTION: Extraction of broken femoral nails in peri-implant fractures is becoming an increasingly common problem faced by orthopaedic surgeons. Different closed and open techniques for removal of broken nails have been previously described but due to variations in equipment and fracture configurations these methods are not always easily reproducible. We describe an open surgical technique using simple equipment that can be utilised when other methods of extraction have failed. CASE PRESENTATION AND SURGICAL TECHNIQUE: We present a case of a peri-implant fracture secondary to non-union in

Arthroscopic fixation with intra-articular button for tibial intercondylar eminence fractures in skeletally immature patients.

The aim of this study was to describe our experiences with arthroscopy-guided intra-articular button fixation in the treatment of displaced tibial eminence fractures in skeletally immature children. Eleven adolescent patients with an average age of 12.2 years were treated arthroscopically between January 2005 and February 2007. At follow-up evaluation at 69 months, we did not find any instability. Only minimal differences were found in the functional outcomes (Lysholm and International Knee Documentation Committee scores: 95.7 and 94.3, respectively). None of the patients had a leg-length disc
