Complications of standardized endonasal dacryocystorhinostomy with unciformectomy.

PURPOSE: To report the nature and frequency of complications occurring with endonasal dacryocystorhinostomy (DCR) based on routine unciformectomy. DESIGN: Prospective, noncomparative interventional consecutive case series study. PARTICIPANTS: Endonasal DCR was performed on 300 patients with complete nasolacrimal stenosis. METHODS: The uncinate process was resected (unciformian endonasal DCR), except when the lacrimal fossa was anterior to its insertion (23 cases [7.7%]). Preoperative clinical assessment and computed tomography scans, intraoperative endoscopic video recording, and postoperat

The treatment of acromegaly with special reference to trans-sphenoidal hypophysectomy.

Experience in the management of 100 cases of acromegaly is described. Three quarters of these had been referred directly to the endocrine clinic at the Middlesex Hospital. The remainder were referred from the Royal Post-graduate Hospital because they were thought unsuitable for yttrium implantation. The patients were studied by clinical assessment of severity, by measurement of basal growth hormone levels on three separate mornings, and by a review of possible complications. Particular attention was paid to diabetes, hypertension, cardiomegaly, respiratory, vascular and skeletal changes as wel

The role of frontal sinus drillouts in nasal polyposis.

PURPOSE OF REVIEW: Historically, the common frontal sinusotomy has been reserved only for salvage frontal sinus surgery in patients with severely recalcitrant inflammatory sinus disease. By understanding the pathophysiology of inflammatory sinus disease and the long-term role of topical medication in their treatment, the indications for Draf3 have expanded. RECENT FINDINGS: A major therapeutic impact from endoscopic sinus surgery for inflammatory sinus disease is imparted by allowing sinus access to topical therapies to work. Although traditionally, the Draf3 frontal sinus procedure has been

Patient-reported olfaction improves following outside-in Draf III frontal sinus surgery for chronic rhinosinusitis.

OBJECTIVES: The Draf III frontal sinusotomy is an established surgical procedure with an important role in the surgical management of recalcitrant chronic rhinosinusitis (CRS). In 2012, the outside-in approach to the Draf III was described as a safe and efficient procedure. Smell recovery in inflammatory CRS is challenging, and to date there is limited evidence suggesting that Draf III improves patient-reported olfactory dysfunction from CRS. METHODS: A consecutive series of patients who underwent an outside-in Draf III for inflammatory CRS by a single, tertiary rhinologist were reviewed. Pat

Preoperative computed tomography evaluation in sinus surgery: a template-driven approach.

Anatomic abnormalities in the paranasal sinuses and skull base are not uncommon. Awareness of these abnormalities may be of assistance in pre-operative planning. This content presents a template-driven approach to the analysis of computed tomography scans in preparation for endoscopic sinus surgery.

[Frontal sinus mucocele].

Between 1995 and 2000 twenty-two patients were treated for frontal sinus mucocele in the Department of Otorhinolaryngology of Medical University of Warsaw. History and physical examination revealed several-year-old chronic rhinosinusitis (CRS) in all but one cases. Thirteen patients had been previously operated on for this reason at least once. Conservative management, mainly in periods of symptom deterioration, was applied in other patients. The only patient with negative history of CRS had undergone in the past head injury with fracture and dislocation of the anterior wall of the frontal sin

Ictal speech arrest and parasagittal lesions.

Lesions in the parasagittal region may cause seizures with speech arrest. To correlate lesion localization within the parasagittal region with ictal speech function we studied 11 patients with parasagittal lesions. The exact lesion localization was assessed with magnetic resonance imaging. In all patients with speech arrest the lesion involved the left superior frontal gyrus (n = 6), in 3 patients the lesion was confined to this gyrus. Speech arrest did not occur in patients with right-sided lesions (n = 4) or left-sided lesions outside the superior frontal gyrus (n = 1). Our data suggest that

Chronic post-traumatic erosion of the skull base.

Delayed post-traumatic erosion of the skull base is reported in three patients who presented as adults with cerebrospinal fluid fistulae and a history of recurrent meningitis. These skull defects were associated with herniation of the subarachnoid space into the diploe of the skull base, the paranasal sinuses and the orbit. This rare complication of head injury is assumed to have occurred as the result of a dural tear at the time of trauma. Its site probably determines whether a resulting meningocele widens the intradiploic space or broaches the cranial floor.

[1-stage procedure in the reconstruction of frontobasal middle face fractures. Modifications and treatment modalities].

This paper deals with the management of 269 patients with frontobasal fractures of which 143 were treated operatively. The close interrelation between midface and base of skull and our operative modifications are discussed. The advantages and importance of the transethmoidal approach to the base of the skull is most adventageous for reconstruction of the midface pillars. Specially for comminuted fractures, avoiding complications such as pseudohypertelorism, telecanthus and frontobasal disturbances. This also enables an accurate reposition to be made with minimal use of foreign materials such a

Bilateral eyelid edema and orbital cellulitis associated with Pott's puffy tumor.

Pott's puffy tumor is rarely associated with orbital cellulitis. We describe a pediatric patient with Pott's puffy tumor and right-sided orbital cellulitis. He underwent urgent surgical drainage of a glabellar subperiosteal abscess and completed a six week course of intravenous antibiotics with complete resolution of infection and no long-term sequelae. Close monitoring with imaging for intracranial spread and multidisciplinary surgical intervention are commonly recommended.

Outcomes of endoscopic sinus surgery (ESS) on sino-orbital osteomas: ophthalmic manifestations and visual outcomes from a 12-year multicenter experience and literature review.

BACKGROUND: Osteomas are the most common benign craniofacial bone tumors that may extend into the orbital cavity. While the majority of them remain asymptomatic, orbital involvement can lead to clinically significant ophthalmologic manifestations. Although endoscopic sinus surgery (ESS) has been increasingly utilized, data focusing specifically on ophthalmologic outcomes following ESS in sino-orbital osteomas are limited. METHODS: This 12-year multicenter descriptive cohort evaluated 12 patients with symptomatic sino-orbital osteomas managed with ESS, including advanced approaches such as Dra

Craniofacial Ossifying Fibromas in Children: Clinical Variability and Surgical Outcomes in a Case Series.

Juvenile ossifying fibroma (JOF) is a rare benign fibro-osseous tumor affecting the craniofacial skeleton in children and adolescents. Although nonmalignant, it may behave aggressively and extend into adjacent structures such as the orbit or anterior skull base. Early symptoms are often subtle, delaying diagnosis. Two histologic variants exist - trabecular and psammomatoid - each with distinct clinical behavior and recurrence potential. We describe a case series of three pediatric patients (an 8-year-old male, a 13-year-old female, and a 16-year-old male) diagnosed with craniofacial JOF betwee

Early Endoscopic Repair of Traumatic Skull Base Dural Injuries: Challenging the Dogma of Conservative Management.

INTRODUCTION: Traditional management of traumatic skull base dural injury has often favored observation and conservative therapy, despite the risk of delayed intracranial complications. This paradigm originated when operative intervention required craniotomy and carried significant morbidity. The objectives of this study were to evaluate outcomes of early endoscopic repair for traumatic skull base dural injury and to assess whether radiographic evidence of dural violation, including pneumocephalus, may support consideration of early intervention. METHODS: A retrospective review was performed

Radiographic detection of retained neuropatties in the paranasal sinuses: a cadaveric diagnostic accuracy study.

BACKGROUND: Retained surgical materials are a preventable source of postoperative complications. This study evaluates the radiographic detectability of retained neuropatties in the paranasal sinuses to support safer postoperative imaging and surgical practice. METHODS: Sixteen previously dissected cadaveric heads were labeled and assigned to predefined sinonasal or nasopharyngeal site using an allocation table (e.g., frontal sinus anterior table, maxillary sinus, sphenoid sinus floor, nasal cavity, nasopharynx). For each head–site combination, a large neuropatty (1 × 3 in) was first inserted a

Modelling the effects of post-FESS middle turbinate synechiae on sinonasal physiology: A computational fluid dynamics study.

OBJECTIVE(S): Chronic rhinosinusitis (CRS) is common and often requires surgical intervention. Surgical failure may lead to persistent symptoms and recalcitrant disease, often secondary to synechiae between the middle turbinate (MT) and lateral nasal wall. Synechiae prevention techniques have been extensively investigated, however evidence for the effect of synechiae on sinonasal physiology is lacking. We aimed to model the effects of MT synechiae on a post-functional endoscopic sinus surgery (FESS) sinonasal cavity using computational fluid dynamics (CFD). METHODS: DICOM data from a CT-sinus
