An unexpected challenge: intra-abdominal neurofibromatosis during biliary reconstruction: a case report.

INTRODUCTION AND IMPORTANCE: Neurofibromatosis type 1 (NF1) is a genetic disorder characterized by peripheral nerve sheath tumors in the skin or nervous system. Its prevalence is 1 in 2600-3000 individuals, but intra-abdominal manifestations appear in only 5-25% of these patients, which makes this presentation a rare medical condition. The presence of intra-abdominal lesions can significantly complicate surgical procedures and render biliary reconstruction technically challenging. CASE PRESENTATION: This case describes an uncommon presentation of visceral NF1 in a 75-year-old woman with cutan

Endoscopic retrograde cholangiopancreatography and primary sclerosing cholangitis: a retrospective study of a high-volume program.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is valuable in management of primary sclerosing cholangitis (PSC). Herein, we characterize individuals with PSC who underwent ERCP on-demand at a high-volume quaternary centre, with evaluation of pathology results and short-term postprocedural outcomes. METHODS: This retrospective cohort study evaluated patients with PSC who underwent ERCP at St Michael's Hospital between April 2011 and July 2021. Chart review was conducted to collect clinical, procedural, and pathology data, and reported post-ERCP complications within 90 days.

Portal vein hypoplasia is present in patients with biliary atresia at the time of diagnosis.

BACKGROUND: In patients with biliary atresia (BA), severe portal hypertension (HTN) develops even with successful bile flow restoration, suggesting an intrinsic factor driving portal HTN independent from bile obstruction. We hypothesize that patients with BA have abnormal portal vein (PV) development, leading to PV hypoplasia. METHODS: In this observational cohort study, we enrolled patients who were referred to a tertiary center from 2017 to 2021 to rule out BA. Newborns who underwent computed tomography angiogram as a clinical routine before intraoperative cholangiogram, and laparoscopic Ka

When It Doesn't Fit: Congenital Anomalies of the Choledochus.

Introduction  Congenital choledochal malformations (CCMs) are characterized by intra- and/or extrahepatic bile duct dilatation. Five basic types (1-5) are recognized in Todani's classification and its modifications, of which types 1 and 4 typically have an associated anomalous pancreatobiliary junction and common channel (CC). We describe two cases with previously undescribed features. Case Report  1 Antenatal detection of a cyst at porta hepatis was made in an otherwise normal girl of Iranian parentage. She was confirmed to be a CCM (20 mm diameter), postnatally, with no evidence of obstructi

Metal stent versus plastic stent in endoscopic ultrasound-guided hepaticogastrostomy for unresectable malignant biliary obstruction: Large single-center retrospective comparative study.

OBJECTIVE: Whether metal stents (MS) or plastic stents (PS) yield better outcomes for malignant biliary obstruction in endoscopic ultrasound-guided hepaticogastrostomy (EUS-HGS) is controversial. We aimed to compare outcomes of initial EUS-HGS performed with MS or PS. METHODS: In this single-center retrospective study, we included patients (MS/PS groups: n = 151/72) with unresectable malignant biliary obstruction and performed multivariable analysis. The landmark date was defined as day 100 and used to evaluate the time to recurrent biliary obstruction (TRBO). RESULTS: The clinical success r

Clinical Outcomes of Biliary Drainage in Patients with Malignant Biliary Obstruction Caused by Colorectal Cancer Metastases.

BACKGROUND AND AIM: Malignant biliary obstruction is an ominous complication of metastatic colorectal cancer (mCRC) that is challenging to solve. Biliary drainage can be performed to relieve symptoms of jaundice, treat cholangitis, or enable palliative systemic therapy. The aim of this study is to evaluate clinical outcomes of biliary drainage of malignant biliary obstruction in mCRC patients. METHODS: Consecutive patients with malignant biliary obstruction due to mCRC who underwent endoscopic retrograde cholangiopancreatography or percutaneous transhepatic cholangiography were included. Diagnosis and treatment of cystic fibrosis. An update.

Cystic fibrosis is the most common fatal inherited disease of Caucasians. At present, cystic fibrosis accounts for most cases of chronic progressive pulmonary disease and for many other clinical features in the first three decades of life. Thus, it is a challenge to both pediatricians and internists, particularly chest physicians. The diagnosis is based on the triad of chronic obstructive pulmonary disease, pancreatic insufficiency, and increased levels of electrolytes in the sweat. The cardinal test for confirmation of the diagnosis is the "sweat test," which is an excellent discriminant for

Extreme Intrahepatic Cholestasis of Pregnancy With Fetal Ascites and Neonatal Meconium Peritonitis.

Intrahepatic cholestasis of pregnancy (ICP) is a rare but potentially severe condition with significant maternal and fetal risks. A 28-year-old gravida 2 para 1 woman presented at 21 weeks' gestation with pruritus and jaundice and was diagnosed with severe ICP (bile acids 297 μmol/L). Despite ursodeoxycholic acid (UDCA) treatment, escalation with rifampicin and antihistamines was required. At 23 weeks, fetal ultrasound revealed massive ascites and features suggestive of meconium peritonitis (MP). Intensive maternal-fetal monitoring allowed pregnancy prolongation to 36 weeks, when cesarean deli

Acute Liver Injury With Cholestatic Pattern: An Adverse Effect of Trimethoprim-Sulfamethoxazole.

Trimethoprim-sulfamethoxazole (TMP-SMX), commonly known by the brand name Bactrim, is a combination antibiotic medication that is used to treat a broad spectrum of infections. It is known to be associated with a wide array of adverse reactions, including drug-induced liver injury (DILI). Drug-induced liver injury associated with TMP-SMX more commonly presents with a hepatocellular or mixed liver injury pattern rather than a cholestatic pattern. Here, we present the case of a 52-year-old female patient who presented with jaundice and pruritus after a recent course of TMP-SMX. Labs and imaging r

When blood mimics bile: A case of haemorrhagic cholecystitis masquerading as biliary pancreatitis.

BACKGROUND: Cholecystitis and pancreatitis are frequent indications for hospitalization in gastroenterology and hepatology, predominantly due to cholelithiasis. Standard management strategies with imaging and cholecystectomy are common practice. This case report highlights the importance of considering atypical causes in patients presenting with symptoms suggestive of cholecystitis and pancreatitis. CASE REPORT: A female patient presented repeatedly to the emergency department with atypical ultrasound findings suggestive of cholecystitis with secondary pancreatitis. Following an episode of se

[A Case of Long-Term Response to Chemotherapy in Mucinous Carcinoma of the Duodenum with Multiple Distant Metastases].

Primary carcinoma of the duodenum, especially mucinous carcinoma of the duodenum, is extremely rare. We present a case of a long-term response to chemotherapy in mucinous carcinoma of the duodenum with multiple distant metastases. A 60-year-old man was admitted to our hospital with epigastric pain and jaundice. CT showed a thickening of the duodenal wall; extensive lymphadenopathy around the head of the pancreas, in the para-aortic region and the mediastinum; suspected peritoneal dissemination; lung metastases; and bone metastases. An upper gastrointestinal endoscopy revealed a duodenal stenos

Endoscopic ultrasound-guided biliary drainage: a literature review.

Neoplasms of the biliopancreatic confluence may present with obstruction of the bile tract, leading to jaundice, pruritus and cholangitis. In these cases drainage of the bile tract is imperative. Endoscopic retrograde cholangiopancreatography (ERCP) with placement of a choledochal prosthesis is an effective treatment in about 90% of cases, even in experienced hands. In cases of ERCP failure, therapeutic options traditionally include surgical bypass by hepaticojejunostomy (HJ) or percutaneous transparietohepatic drainage (DPTH). In recent years, endoscopic ultrasound-guided biliary drainage tec

Modified hepaticocholecystoenterostomy technique for certain hepatobiliary and pancreatic diseases: Case series report by a single surgeon.

Prior to this research, very few reports of hepaticocholecystoenterostomy (HCE) and its modification have been published. Two anastomoses were used to establish a biliary bypass using the gallbladder as a conduit by a senior hepato-biliary surgeon. Between 2013 and 2019, 11 patients (five men and six women) with a mean age of 61.7±15.7 (range, 31-85) years were seen. Disease indications included periampullary malignant tumours of Vater (7 cases), chronic pancreatitis (1 patient), cystic pancreatic head tumours (2 patients), and choledochal cysts (1 patient). Rapunzel syndrome in adult with mysterious presentation: a rare case report with literature review.

Bezoars are indigestible foreign material in the gastrointestinal tract (GIT). A phytobezoar is a mass of plant material, while a trichobezoar consists of matted hair. This is found most commonly in the stomach in children who chew their long hair. When a gastric trichobezoar extends to the duodenum or jejunum, this is known as Rapunzel syndrome (RS), after a famous Brothers' Grimm fairy tale. Our patient presented with haematemesis, localised peritonitis, a palpable epigastric mass, coagulopathy, severe anaemia and deranged liver function tests. An abdominal computed tomography scan was sugge

Acute hepatitis induced by Epstein-Barr virus infection: a case report.

Epstein-Barr virus is a causative agent of infectious mononucleosis syndrome, which is commonly seen in young adults and characterized by fever, sore throat and lymphadenopathy. In adults, Epstein-Barr virus infection can cause liver function test abnormalities without pharyngitis or lymphadenopathy. Liver involvement usually causes mild elevation of transaminases and this abnormality resolves spontaneously. Jaundice might develop rarely during the clinical course of Epstein-Barr virus infection. It reflects either more severe hepatitis or Epstein-Barr virus infection-associated hemolytic anem
