[Vocal cord dysfunction. An important differential diagnosis to bronchial asthma].

Vocal cord dysfunction (VCD) is described as a functional disorder of the vocal folds which leads to an intermittent, inspiratory 'paradoxical' glottal closure. We report on three women with frequent repetitive shortness of breath attacks caused by VCD. This was diagnosed by transnasal videofiberendoscopy, with glottal closure being seen during inspiration. Because of the different etiologies, one of the patients was treated with breathing and speech therapy, another received Omeprazol for laryngopharyngeal reflux, and the third was treated by intralaryngeal botulinum toxin injections. All thr

Cytomegalovirus and the otolaryngologist.

Human cytomegalovirus (CMV) is ubiquitous and has been documented as the etiologic factor in several diseases ranging from respiratory tract infection to congenital deafness. Special clinical manifestations of CMV of interest to the otolaryngologist are presented as related to the temporal bone and audiovestibular system, the facial nerve, the aerodigestive tract, the thyroid, the hematopoietic-lymphoreticular system, and salivary gland. In addition, current concepts of CMV oncogenic potential are discussed as they relate to head and neck neoplasms. The results of a study performed on surgical

Evaluation of the implementation of the EWARS Mobile epidemiological surveillance tool in Sudanese refugee camps in Eastern Chad: a retrospective and population-based surveillance study.

BACKGROUND: The escalation of the conflict in Sudan has created a major humanitarian challenge for neighboring countries, especially in the Eastern regions of Chad. This humanitarian setting's health needs are unique in that they are more vulnerable to both outbreak-prone disease and a lack of essential services. To address these challenges, the World Health Organization has supported implementing the Early Warning Alert and Response System (EWARS) Mobile. The purpose of this study was to evaluate the application of the EWARS Mobile epidemiological surveillance tool in Sudanese children's refu

Clinical characteristics, temporal patterns, and outcomes of Guillain-Barré syndrome: A 7-year cross-sectional study in a low-resource setting.

Guillain-Barré syndrome (GBS) is a rare but serious neurological disorder that often leads to significant morbidity and mortality. This study aimed to investigate the clinical characteristics, epidemiological trends, and outcomes of GBS patients admitted to Jimma Medical Center (JMC). This was a mixed retrospective and prospective study conducted at JMC, a tertiary teaching hospital in southwest Ethiopia. Medical records of patients admitted with GBS from October 2017 to August 2021 were retrospectively reviewed, while patients admitted between September 2021 and October 2024 were prospectivel

Characteristics of delirium and its association with sedation and in-hospital mortality in patients with COVID-19 on veno-venous extracorporeal membrane oxygenation.

BACKGROUND: Veno-venous extracorporeal membrane oxygenation (VV-ECMO) has been used in patients with COVID-19 acute respiratory distress syndrome (ARDS). We aim to assess the characteristics of delirium and describe its association with sedation and in-hospital mortality. METHODS: We retrospectively reviewed adult patients on VV-ECMO for severe COVID-19 ARDS in the Johns Hopkins Hospital ECMO registry in 2020-2021. Delirium was assessed by the Confusion Assessment Method for the ICU (CAM-ICU) when patients scored-3 or above on the Richmond Agitation-Sedation Scale (RASS). Primary outcomes wer

The recurrent laryngeal nerve related to thyroid surgery.

BACKGROUND: Iatrogenic injury of inferior laryngeal nerve is one of the most serious concerns in thyroid surgery. Paralysis of vocal cords is a common sequela of thyroidectomy. It represents a serious complication inducing, when bilateral, serious functional sequelae such as phonatory, respiratory and psychological problems that limit working capacities and social relationships of patients. We carried out an intraoperative study aimed to define anatomical relationships between the recurrent laryngeal nerve and the adjacent structures (the inferior thyroid artery in particular), intraoperative

Clinical spectrum of botulism.

Botulism is a paralyzing disease caused by the toxin of Clostridium botulinum. The toxin produces skeletal muscle paralysis by producing a presynaptic blockade to the release of acetylcholine. Recent studies have pinpointed the site of action of the several types of botulinum neurotoxin at the nerve terminal. Since the discovery of the toxin about 100 years ago, five clinical forms of botulism have been described: 1) classic or foodborne botulism; 2) wound botulism; 3) infant botulism; 4) hidden botulism; 5) inadvertent botulism. A clinical pattern of descending weakness is characteristic of a

Twenty-nine Cases of Enterovirus-D68-associated Acute Flaccid Myelitis in Europe 2016: A Case Series and Epidemiologic Overview.

BACKGROUND: Enterovirus-D68 (EV-D68) is a respiratory virus within the genus Enterovirus and the family of Picornaviridae. Genetically, it is closely related to rhinovirus that replicates in the respiratory tract and causes respiratory disease. Since 2014, EV-D68 has been associated with the neurologic syndrome of acute flaccid myelitis (AFM). METHODS: In October 2016, questionnaires were sent out to a European network including 66 virologists and clinicians, to develop an inventory of EV-D68-associated AFM cases in Europe. Clinical and virologic information of case patients was requested. In

A study of an electrodiagnostic technique for the evaluation of equine recurrent laryngeal neuropathy.

The electrodiagnostic measurement of the thoracolaryngeal reflex (TLR) ('slap test') latency was compared to 5 other diagnostic techniques used for evaluation of laryngeal function, namely laryngeal muscle palpation, resting and immediately postexercise endoscopic examinations, and palpable and endoscopic responses to the TLR. Compared to resting endoscopy, the electrodiagnostic measurement of TLR latency was not found to be an accurate test for the evaluation of recurrent laryngeal neuropathy (RLN), nor was laryngeal muscle palpation or the endoscopic response to the TLR. Twenty-five (71%) of

Ductal arteriosus aneurysm, right aortic arch, and isolated left subclavian artery in a neonate.

Ductal arteriosus aneurysm (DAA) is a well-recognized condition, especially in infancy, and is usually asymptomatic. We report the first case of a newborn who presented with significant inspiratory stridor and, using multiple imaging investigations, was subsequently diagnosed with the rare constellation of a congenital DAA, a right aortic arch and an isolated left subclavian artery with normal intra-cardiac anatomy. The patient underwent surgical resection of the DAA with significant improvement in symptoms.

Intraperitoneally administered IgG from patients with amyotrophic lateral sclerosis or from an immune-mediated goat model increase the levels of TNF-α, IL-6, and IL-10 in the spinal cord and serum of mice.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease that involves the selective loss of the upper and lower motor neurons (MNs). Neuroinflammation has been implicated in the pathogenesis of the sporadic form of the disease. We earlier developed immune-mediated animal models of ALS and demonstrated humoral and cellular immune reactions in the nervous system and in the sera of patients and animals. The accumulation of immunoglobulin G (IgG), an elevated intracellular level of calcium, ultrastructural alterations in the MNs, and activation of the microglia were noted in

Thoracic epidural anesthesia (TEA) with 0.2% ropivacaine in combination with ipsilateral brachial plexus block (BPB) for modified radical mastectomy (MRM).

BACKGROUND: Breast cancer is the 2nd most common tumors in Thai women. Until now, oncologic breast surgeries are typically performed by general anesthesia (GA). However, GA cannot provide adequate postoperative pain control and routine use of parenteral opioids aggravate postoperative sedation, nausea, emesis, impaired oxygenation and depressed ventilation. Thoracic epidural anesthesia (TEA) is one of the regional anesthetic techniques that can be done by using a low dose of local anesthetic in combination with ipsilateral brachial plexus block (BPB) for axillary node dissection. TEA can provi
