Combined fractures of the distal radius and scaphoid.

Combined fractures of the distal radius and scaphoid are uncommon, are usually the result of a high-energy trauma and there is no consensus regarding their optimal management. We present a retrospective study of ten patients, out of whom nine underwent internal fixation of their fractures. Open reduction and internal fixation were performed in six of the eight intraarticular fractures of the distal radius. After a mean follow-up of 40 months, eight patients reported no pain and the mean range of wrist motion was 55 degrees flexion and 71 degrees extension. Our current management protocol is ou

[Distal radius fractures and concomitant lesions. Experimental studies concerning the pathomechanism].

Since Colles (1814) first described a distal radius fracture and the publication by Dupuytren (1834), the frequency of and problems surrounding these fractures were increasingly recognised. Not only the treatment but also the formation and type of fracture were repeatedly analysed. The various theories concerning the pathomechanism led to names such as: "tear fracture", "bend fracture", "thrust fracture", "compression fracture", "snap fracture" and others. In our experimental studies we attempted to simulate the pathomechanism of distal radius fractures and present them pictorially. With the h

Perilunate dislocations and transscaphoid perilunate fracture-dislocations: a retrospective study with minimum ten-year follow-up.

PURPOSE: Perilunate dislocations and transscaphoid perilunate fracture dislocations are associated with a high incidence of posttraumatic arthritis. According to the current literature, at medium-term follow-up, radiological signs of arthritis do not correlate with functional scores. The aim of this study was to evaluate patient hand function and the development of posttraumatic arthritis after perilunate dislocations (11 cases) and transscaphoid perilunate fracture dislocations (7 cases) at a minimum 10 years of follow-up. METHODS: A retrospective review was performed including 18 patients w

Scaphoid fractures with scapholunate ligament involvement: Instability or ligamentous laxity? Role of arthroscopy and pinning.

PURPOSE: Percutaneous fixation of scaphoid fractures often does not allow the evaluation of potential ligamentous lesions. Arthroscopy is an useful tool in the management of scaphoid fractures to visualize potentially associated lesions. With arthroscopic assistance, we often found scapholunate ligament lesions. Our study's aim was to evaluate the role of arthroscopy in the treatment of scaphoid fracture and in particular if the alteration found was a real lesion causing instability or just a paraphysiologic laxity. Furthermore, we evaluated whether the scapholunate joint pinning alters the ou

Scaphotrapeziotrapezoid Fusion for Chemotherapy-induced Capitate Avascular Necrosis.

The capitate's retrograde vascular supply and limited intraosseous anastomoses predispose its proximal pole to avascular necrosis, although this remains rare. Although typically linked to trauma, connective tissue disorders, or corticosteroid use, avascular necrosis may also arise secondary to chemotherapy. We present the case of an 18-year-old woman with a history of childhood leukemia who developed capitate avascular necrosis. This case emphasizes the importance of considering avascular necrosis in chemotherapy-exposed patients presenting with atraumatic wrist pain and evaluates outcomes of

Comparison of the Diagnostic Performance of Strain Elastography and Shear Wave Elastography for the Diagnosis of Carpal Tunnel Syndrome.

OBJECTIVES: To compare the diagnostic performance between strain elastography and shear wave elastography (SWE) for the diagnosis of carpal tunnel syndrome (CTS). METHODS: Between July 2018 and June 2019, 66 consecutive patients with 95 imaged wrists underwent wrist ultrasound, including grayscale ultrasound, strain elastography, and SWE, because of the suspicion of CTS. During wrist ultrasound, the cross-sectional area (CSA), strain ratio, elasticity, and shear wave velocity of each median nerve were measured at the proximal carpal bone level (scaphoid to pisiform). The variables were compar

Personal technique for wrist dorsal approach.

In hand disorders surgical procedures are more and more widely used and often it's necessary to approach the wrist by the dorsal way. Beneath anatomy of this region is well known, there is still room enough to develop new surgical exposure techniques mostly related to physiology and biomechanics. Our goals are to present an innovative surgical dorsal exposure of the wrist, to show its use for different problems solving, and to evaluate its mini-invasive and functional outcome. Our inedited surgical technique is presented. Since November 1999 to February 2008, this technique has been used by th

The use of temporary screw augmentation for the treatment of scapholunate injuries.

INTRODUCTION: Injuries to the scapholunate (SL) interosseous ligament can have devastating consequences to carpal stability. The purpose of this study is to provide a technique for screw augmentation to SL reconstruction, while comparing radiographic outcomes to a traditional Kirschner wire fixation cohort. METHODS: A retrospective review of all patients treated by screw fixation for SL ligament injuries was cross-matched to a cohort of patients treated with pin fixation for age and length of time from the time of injury to surgical fixation. Outcomes were the values of SL gap and SL angle on

[Functional results two years after the treatment of a delayed diagnosed scapho-capitate fracture syndrome].

The scapho-capitate syndrome (Fenton syndrome) is so rare that diagnosing is a real challenge to every clinician. Therefore, its primary treatment is not always possible. In this case we are reporting about a delayed open treatment of a Fenton syndrome ten days after the original injury. Open reduction was performed via a dorsal approach and the capitate fracture was stabilized with K-wires and the scaphoid fracture with screw fixation. After postoperative immobilization of the wrist using an external fixator for six weeks the patient was already able to work again only twelve weeks after surg

Proximal ROw carpectOmy versus four-corner Fusion (PROOF-trial) for osteoarthritis of the wrist: study protocol for multi-institutional double-blinded randomized controlled trial.

BACKGROUND: Scapholunate advanced collapse (SLAC) and scaphoid non-union advanced collapse (SNAC) are common types of wrist osteoarthritis (OA). Non-operative treatment consists of pain medication, splinting, and avoiding activities that induce pain. However, in case a course of conservative treatment is unsuccessful, operative treatment is needed. The two most conventional operative approaches for SLAC/SNAC OA are four-corner arthrodesis (FCA) and proximal row carpectomy (PRC). Although FCA is the gold-standard operative technique and may lead to superior grip strength, the evident benefit of

Carpal ligament injuries with acute scaphoid fractures - a combined wrist injury.

Fifty-two consecutive patients with acute scaphoid fractures were treated by closed reduction and percutaneous screw fixation. In addition, all underwent a wrist arthrogram and 22 patients also underwent a wrist arthroscopy. Eighteen patients (34%) had an associated carpal ligament injury (four scapholunate ligament, eight lunotriquetral ligament, two combined, three TFCC and one minor leak from the distal carpal row). These patients had worse Mayo wrist scores (no ligament injury median score=95: ligament injury median score=85) at a mean final follow-up of 61 weeks. We advocate that scaphoid

Mid-term outcomes of scapholunate ligament reconstruction with internal brace augmentation.

INTRODUCTION: Scapholunate ligament injury may cause carpal instability leading to pain, dysfunction and risk of scapholunate advanced collapse (SLAC) if untreated. Chronic dissociation is challenging to manage, with no clear reference standard reconstruction technique. This study aimed to evaluate the outcomes of scapholunate ligament reconstruction using an extensor carpi radialis brevis autograft and internal brace augmentation. METHODS: We prospectively enrolled 12 patients with symptomatic chronic scapholunate (SL) dissociation treated between 2020 and 2024. Clinical outcomes included a

Arterial Perfusion of the Medial Femoral Trochlea Flap Examined by Micro-Computed Tomography Angiography.

PURPOSE: The medial femoral trochlea (MFT) flap is a source of vascularized articular cartilage that has most commonly been used for reconstruction of proximal pole scaphoid nonunions and lunate osteonecrosis in Kienbock disease. This study aimed to describe the arterial perfusion of the MFT. METHODS: Selective injection of the descending genicular artery (DGA) was performed with radiopaque dye in eight cadaveric lower extremities. Micro-computed tomography was performed on the medial femoral condyle including the MFT segment, with digital analysis quantifying vessel location and internal dia

Adverse tissue reactions to bioabsorbable fixation devices.

Among 2528 patients operated on using pins, rods, bolts, and screws made of polyglycolic acid or polylactic acid, 108 (4.3%) were affected by a clinically significant local inflammatory, sterile tissue reaction. The three most common indications for the use of these fixation devices were a displaced malleolar fracture, a chevron osteotomy for hallux valgus, and a displaced fracture of the radial head. In 107 patients, the reaction was elicited by a polyglycolic acid implant, and in one patient by a polylactic acid implant. The incidences were 5.3% (107 of 2037) and 0.2% (one of 491), respectiv
