Congenital hallux varus with ventricular septal defect: a unique case report.

INTRODUCTION AND IMPORTANCE: Congenital hallux varus (CHV) is a very rare foot deformity, defined by the medial deviation of the great toe at the metatarsophalangeal joint. It can appear in isolation or with other anomalies, such as polydactyly, metatarsus adductus, or a longitudinal epiphyseal bracket. However, an association with a ventricular septal defect (VSD) has not been described before. Early surgical correction is essential to restore function, improve cosmetic appearance, and support normal motor development. PRESENTATION OF CASE: A 13-month-old male presented with congenital right

Evaluating the Biomechanical Effects and Real-World Usability of a Novel Ankle Exo for Runners.

Achilles tendon overuse injuries are common for long-distance runners. Ankle exos (exoskeletons and exosuits) are wearable devices that can reduce Achilles tendon loading and could potentially aid in the rehabilitation or prevention of these injuries by helping to mitigate and control tissue loading. However, most ankle exos are confined to controlled lab testing and are not practical to use in real-world running. Here, we present the design of an unpowered ankle exo aimed at reducing the load on the Achilles tendon during running while also overcoming key usability challenges for runners outs

Dancers With Chronic Ankle Instability Have Different Regional Plantar Force During Single-Leg Jump Landing.

Background: It is already known that the prevalence of ankle sprain is high in dancers, leading to chronic ankle instability (CAI) and, consequently, affecting jump landing. Therefore, methods that allow the early identification of jump landing alterations related to CAI are necessary. Research Question: This study aimed to investigate if regional plantar force estimated from plantar pressure distribution is different between dancers with and without ankle instability during jump landing. Methods: Female dancers with (n = 12) and without (n = 14) CAI performed single-leg jumps on a force plate

Scoliosis, leg length discrepancy, or both?

PURPOSE OF REVIEW: Scoliosis and limb length discrepancy (LLD) are frequent findings in pediatric orthopedic practice. Because both conditions can alter posture and spinal alignment, distinguishing between a compensatory spinal curvature due to LLD and a true structural scoliosis is crucial. This review explores the relationship between scoliosis and LLD in children, emphasizing recent evidence and clinical strategies for accurate diagnosis and management. RECENT FINDINGS: Minor limb length discrepancies are common in the general population. However, significant LLD (defined as >2 cm) can pro

Nonunion of Isolated Medial Cuneiform Fracture Fixed With a Compression Screw and Compression Staple: A Case Report.

Isolated cuneiform fractures are rare and account for only 1.7% of all midfoot fractures. Medial cuneiform fractures can be treated conservatively or surgically, with good clinical outcomes. However, nonunion is a rare complication of medial cuneiform fractures, and only a few cases have been reported in the literature. We report a case of a medial cuneiform fracture requiring surgical treatment that had a good clinical outcome. A 15-year-old boy presented to an orthopedic clinic with a complaint of pain in his right foot. The patient had landed on the foot during a handball game and was treat

Intelligent plantar pressure offloading for the prevention of diabetic foot ulcers and amputations.

The high prevalence of lower extremity ulceration and amputation in people with diabetes is strongly linked to difficulties in achieving and maintaining a reduction of high plantar pressures (PPs) which remains an important risk factor. The effectiveness of current offloading footwear is opposed in part by poor patient adherence to these interventions which have an impact on everyday living activities of patients. Moreover, the offloading devices currently available utilize primarily passive techniques, whereas PP distribution is a dynamically changing process with frequent shifts of high PP a

What Do Patients Report Regarding Their Real-World Function Following Triplane Metatarsophalangeal Joint Arthrodesis for Hallux Valgus?

First metatarsal phalangeal (MTP) joint arthrodesis has been employed for decades for pain related to arthritis and other associated abnormalities. Despite the commonality of the procedure there continues to be questions regarding functional expectations following the procedure especially when employed for correction of hallux valgus deformity. We surveyed 60 patients who had a tri plane MTP joint arthrodesis at mean 28.4 months (median 27.8) regarding their activities of daily living and sports activity through a direct conversation. Secondary endpoints assessed were return to activity, defor

Decision-Making and Management of Tarsal Coalition in the Young Adult Patient: A Critical Analysis Review.

» Tarsal coalitions most commonly affect the calcaneonavicular and talocalcaneal joints in up to 13% of the general population. They alter the mechanics of the subtalar joint, limiting inversion and eversion, and place excessive stress on neighboring joints causing pain, recurrent ankle sprains, and/or progressive pes planus during the adolescent growth spurt.» While many coalitions are identified on radiographs, advanced imaging with computed tomography or magnetic resonance imaging is sometimes required. These advanced imaging modalities also serve an essential role for surgical planning to

What Do Patients Report Regarding Their Real-World Function Following Concurrent Arthrodesis of the First Tarsometatarsal and First Metatarsophalangeal Joints.

First metatarsal phalangeal joint and the first Tarsometatarsal arthrodesis have both been used for deformity correction and treatment of pain and arthritis. Concomitant arthrodesis of the first metatarsophalangeal and first tarsometatarsal joints (Double First Ray Arthrodesis) has rarely been described. We surveyed 29 patients who underwent double first-ray arthrodesis for primary or revision correction of hallux valgus. A custom questionnaire was used to assess their activities of daily living and sports activity at a mean follow-up of 19.6 months (median 18.4 months). Secondary endpoints we

A wearable gait analysis protocol to support the choice of the appropriate ankle-foot orthosis: A comparative assessment in children with Cerebral Palsy.

BACKGROUND: Cerebral Palsy is, nowadays, the most common cause of pediatric disabilities, particularly debilitating for daily living activities. While the adoption of ankle-foot orthoses is very well established as gait treatment, the choice of the most appropriate orthotic configuration is not strongly supported by scientific evidence. The aim of this study was to develop an instrumented assessment protocol based on wearable gait analysis to support clinicians in ankle-foot orthoses configuration selection. METHODS: Ten children with spastic diplegic Cerebral Palsy were assessed (7 males, ag

An experimental study of die filling of pharmaceutical powders using a rotary die filling system.

Die filling is a critical process step in pharmaceutical tablet manufacturing. Mass and content uniformity of the tablets as well as production throughput depend upon the die filling performance of the formulations. The efficiency of the die filling process is influenced by powder properties, such as flowability, cohesion, particle size and morphology, as well as the process conditions. It is hence important to understand the influence of powder properties on the die filling performance. The purpose of the present study is to identify the critical material attributes that determine the efficie
