Decoherence via Demyelination Hypothesis (DDH): A Mechanism of Cognitive Decline During Aging.

Progressive cognitive decline and loss of white matter integrity are observed during aging, but whether these two processes are connected remains unclear. We propose the Decoherence via Demyelination Hypothesis (DDH) as a mechanism linking non-uniform, tract-specific myelin loss during aging, which results in the degradation of conduction timing that is required to coordinate long-range neuronal assemblies. This impairs the dynamic assembly of task-dependent functional networks and promotes age-associated cognitive change. Axonal myelination determines conduction velocity as well as signal tra

Risk-benefit considerations for antirheumatic drug use in pregnancy, breastfeeding, and male preconception: expert perspectives.

OBJECTIVES: Managing antirheumatic drug therapy during family planning, pregnancy, and breastfeeding remains challenging in rheumatology. Since inactive rheumatic disease is an important prerequisite for favourable fertility and pregnancy outcomes, clinicians must weigh the risk of drug exposure against the risk of uncontrolled disease. This study explored how considerations of disease activity influence prescribing decisions. METHODS: We surveyed 20 experts from the EULAR (European Alliance of Associations for Rheumatology) task force on their use of antirheumatic drugs in reproduction, preg

Auditory-Somatosensory Influences on Vocal Motor Control in Primary Muscle Tension Dysphonia.

PURPOSE: Auditory and somatosensory systems jointly support vocal motor control, yet their relative contributions in primary muscle tension dysphonia (pMTD) remain poorly understood. This study examined how auditory and somatosensory feedback influence vocal motor control and adaptation in individuals with and without pMTD. METHOD: Fifty-eight participants (pMTD: n = 17; controls: n = 41) completed a 200-trial altered auditory feedback paradigm involving sustained vowel productions with fundamental frequency downward shifts at -100 cents. The task was repeated with nebulized lidocaine to tran

Rethinking injury in construction safety technology: A theory-informed analytical review of hazard detection and injury burden.

INTRODUCTION: The construction industry remains one of the most hazardous work environments despite advances in safety technologies. Construction safety technology (CST) research has increasingly focused on hazard detection, monitoring, and prediction; however, the conceptualization of injury within this literature remains unclear. METHOD: This study examines how injuries are represented in CST research through a theory-informed analytical review of 148 studies. The analysis is structured around three domains: safety risk context, safety management function, and injury outcome and burden. RE

Aerobic conditioning and ginsenoside modulation modify molecular resilience in middle-aged rats: Differential effects of individual and combined interventions.

Hippocampal aging is characterized by early cognitive decline, disrupted redox homeostasis, and shifts in amyloid processing. This study evaluated the modulatory effects of aerobic exercise and ginseng supplementation, both individually and concurrently, against these alterations. Middle-aged Wistar rats (n = 30) were allocated into young control, middle-aged control, exercise, ginseng supplementation, and combined intervention groups, and assessed through T-maze and Morris Water Maze tasks. Hippocampal integrity was examined with Cresyl violet staining and Nrf2 protein immunohistochemistry. G

Elucidating directed neural dynamics of scene construction across memory and imagination.

Autobiographical memory (AM) and imagination both rely on the brain's ability to construct vivid, coherent mental representations of past or imagined experiences. A central cognitive process underlying these functions is scene construction-the mental generation of spatially organized, imagery-rich representations of environments. Using ultra-high field 7T fMRI and Dynamic Causal Modeling (DCM), we examined directed effective connectivity among the ventromedial prefrontal cortex (vmPFC), hippocampus, and precuneus during object imagery, single-scene construction, scenario construction and AM re

Revisiting Face-to-Hand Area Remapping in the Human Primary Somatosensory Cortex after a Cervical Spinal Cord Injury.

Spinal cord injury (SCI) leads to profound disruptions in sensorimotor processing. Seminal research in nonhuman primates suggests this sensory deprivation causes functional remapping in the primary somatosensory cortex (S1), where somatotopic representations of deprived body parts, such as the hand in cervical SCI, become responsive to touch on intact body parts, such as the face. However, evidence for such remapping in humans remains inconclusive. We investigated face-to-hand remapping in 16 chronic cervical SCI patients (15 male, 1 female) and 21 able-bodied controls (19 male, 2 female) usin

A Contrastive Domain Adaptation Framework for Knee Osteoarthritis Severity Grading.

Kneeosteoarthritis (KOA) is a common degenerative joint disease that causes pain, stiffness, and impaired mobility. Automated Kellgren-Lawrence (KL) grading from knee X-ray images facilitates efficient screening and follow-up assessment. However, models trained on a single-source dataset frequently suffer performance degradation when applied to external cohorts, due to heterogeneities in image quality, acquisition protocols, class distributions, and annotation patterns. Furthermore, conventional domain adaptation approaches typically treat all source samples uniformly, making them vulnerable t

Parameter-Efficient LoRA-GRL Adaptation for Cross-Center Classification of Benign and Malignant Lung Nodules on Heterogeneous Standard-Dose Chest CT: A Multi-Institutional Study from Palestine.

Automated CT lung nodule classification suffers from domain shift across centers. We propose LoRA-GRL, combining Low-Rank Adaptation (LoRA) with adversarial domain harmonization via a Gradient Reversal Layer. Rank-8 LoRA modules were inserted into all attention and feed-forward linear projections (48 sites) of a frozen ViT-S backbone; a domain discriminator encouraged center-invariant features across three centers. Normal vs. Benign and Normal vs. Malignant tasks used 264 patients with patient-level five-fold stratified cross-validation. LoRA-GRL achieved AUCs of 0.9735 and 0.9869, close to fu

From Allergy to Tumor Immunology: The Dual Role of Antigen-Specific IgE-Mechanisms, Challenges, and Prospects for Clinical Translation.

This review aims to systematically explore the dual role of antigen-specific IgE (AS-IgE) in allergic reactions and tumor immunity, focusing on the possibility, mechanism and current obstacles of its clinical transformation. Recent studies have revealed that AS-IgE is not only a single function of promoting allergies. It can activate antibody-dependent cytotoxicity (ADCC) and phagocytosis (ADCP) by binding to FcεRI receptors on the surface of immune cells (common in monocytes and macrophages), thereby reshaping the surrounding environment of tumors (TME) and enhancing the anticancer activity o

When algorithms don't care: physiotherapy and the post-professional turn.

Physiotherapy has long resisted the forces that restructured manufacturing, journalism, and finance. This resistance is ending. Drawing on Gilles Deleuze's prescient analysis of societies of control, this paper argues that physiotherapy - along with the orthodox health professions more broadly - is entering a post-professional era characterized by the progressive marginalization of practitioners in the coordination and delivery of care. Three concurrent forces drive this transformation: the atomization of bodies and health into commodifiable fragments under late-stage capitalism; the systemati

Seeing ourselves in anatomy: Embedding equity, diversity, and inclusivity to transform learning in a UK BSc curriculum.

Embedding intersectional equity, diversity, and inclusion (EDI) in anatomy education is essential to reflect the growing diversity of student populations, support all learners, and prepare graduates for a more inclusive, socially responsive global workforce. Anatomical textbooks and models have typically focused on white, cis-gender, able-bodied individuals who perpetuates biases and underrepresentation, impacting learners' professional development. We embedded five workshops in the junior honors BSc anatomy curriculum, which all adopted an intersectional EDI approach. These were strongly anch

miRSiC: a regulatory-aware machine learning framework for microRNA expression inference across bulk and single-cell transcriptomes.

MicroRNAs (miRNAs) are key post-transcriptional regulators embedded in gene regulatory networks between upstream transcription factors (TFs) and downstream target genes (TGs), yet most computational approaches infer miRNA expression using unstructured transcriptomic features without explicitly modeling their regulatory architecture. In this study, we develop miRSiC, an interpretable machine learning framework that integrates TFs and experimentally validated TGs for regulatory-aware miRNA expression inference. miRSiC formulates prediction as miRNA-specific regression tasks and evaluates three r

Inducing Long-Term Plastic Changes and Visual Attention Enhancement Via One-Week Cerebellar Crus II Intermittent Theta Burst Stimulation (iTBS): An EEG Study.

Intermittent theta burst stimulation (iTBS) is a non-invasive technique frequently employed to induce neural plastic changes and enhance visual attention. Currently, most studies utilized a single iTBS session on healthy subjects to induce short-term neural plastic changes within tens of minutes post-stimulation and investigate its single-session effect on attention performance. Few studies have conducted multiple iTBS sessions on the cerebellum to explore long-term effects on the cerebral cortex and daily effects on visual attention performance. In this study, 18 healthy subjects were involve

In-orbit experiment of 48-hour error-free FSO communication enabled by an effective DFS compensation in a cross-orbital satellite link.

Inter-satellite optical wireless communications (ISOWC) has emerged as an enabling technology for next-generation satellite internet constellations and so has attracted a great of deal interest from the research community. However, establishing stable free-space optical (FSO) links over several thousand kilometers entails challenges due to instabilities and uncertainties in the optical beam pointing, acquisition, and tracking (PAT). Furthermore, compared to co-orbital communications, cross-orbital satellites experience greater relative motion, requiring inter-satellite FSO systems to overcome

Assessing agreement and usability of kinematic measures between markerless and marker-based motion capture systems during jumping activities.

Markerless motion capture technology can improve efficiency and accessibility in human biomechanics research, yet its accuracy and precision are not fully vetted. As markerless technology relies on the field-of-view from 2D video-based images from cameras, the effect of camera positioning on reliability of resulting kinematics and usability of such technology are not well understood. Various jumping tasks were concurrently recorded from 11 participants using three independent camera systems: two identical markerless camera systems positioned 1.4 and 3 m high, respectively, to assess precision
