Indications and Limitations of Bilayer Wound Matrix-Based Lower Extremity Reconstruction: A Multidisciplinary Case-Control Study of 191 Wounds.

BACKGROUND: Little is known about the efficacy of newer skin substitute scaffolds to reconstruct complex lower extremity wounds. The investigators present a multihospital experience of reconstructive surgeons utilizing collagen-GAG bilayer wound matrix in lower extremity soft-tissue reconstruction with the goals to (1) characterize a suitable patient population, (2) categorize failures to optimize patient selection, and (3) determine wound factors affecting success. METHODS: Subjects underwent collagen-GAG-based lower extremity wound reconstruction from May of 2010 to June of 2017. The primar

The abdominal flap using scarred skin in the treatment of postburn hand deformities of severe burn patients.

PURPOSE: To present a random-pattern flap technique using scarred skin from the abdominal wall to cover the defect after burn scar was released from the hand and the scarred skin raised from the hand was transferred to the abdominal wall to cover the flap-donor area in patients who suffered from severe burns involving the entire body and had no available healthy skin to be used for coverage. METHODS: Ten hands of 7 patients (5 males, 2 females) were treated in our department between April 1994 and February 2001. The total body surface area involved with second-or third-degree burns was 85% to

Surgical Techniques for Repair of Atraumatic Tibialis Anterior Tendon Ruptures: A Report of Two Cases.

Tendon ruptures of the foot and ankle are overwhelmingly due to direct or blunt force trauma; however, spontaneous tendon ruptures have been less commonly documented in the published data. Surgical techniques for the repair of atraumatic ruptures differ from those for acute traumatic ruptures owing to the delayed patient presentation. Spontaneous tendon ruptures usually result from predisposing factors that have compromised the structural integrity of the tendon before the rupture occurs. We present 2 cases of atraumatic rupture of the tibialis anterior tendon, each repaired using a different

Management of Heel Pad Degloving Injury After Severe Foot Crush Injury: A Case Report Study.

Crush injuries of the foot and ankle are uncommon and they have a poor prognosis leading to some form of disability. Degloving injuries of the heel and foot after crush injuries are rare and very challenging to manage due to the need for reconstruction of both osseous and soft tissue architecture. We present a salvage strategy for an open injury to the foot with extensive soft tissue detachment from the plantar and dorsal surface. A 30-year-old man was transferred to the Emergency Department from a neighboring hospital with a crush injury to the foot that had resulted in a degloving injury of

Easy Technique for Radiographic Evaluation of Stump-socket Fit in Below-knee Amputees.

BACKGROUND: The alignment of a prosthesis affects the way load is transferred to the stump through the socket; this is critically important for its comfort and function. The aim of the study was to assess the value of radiopaque materials in evaluation of stump-socket fit in people with below-knee amputation. MATERIAL AND METHODS: Thirty-two amputees (skin breakdown group) had cutaneous lesions in the stump, and another 26 amputees (control group) had no lesion in the stump after training with their prosthesis. Metallic markers were attached to weight-bearing points in the internal socket. X-

Anatomical Study of the Compositions and Internal Connections of the Chiasma Plantare (Master Knot of Henry): Exploring Its Possible Clinical Impact.

The purposes of this study were to integrate the types of interconnecting fibers among components of the chiasma plantare and to deduce thei … ses, and the fifth toe in 11 (22%) cases. These data suggest that such variations might result from tendon transfer. In conclusion, we considered the FDL to be more advanced for the recovery of both the ankle and the forefoot based on this study.

Versatility of the pedicled peroneal artery perforator flaps for soft-tissue coverage of the lower leg and foot defects.

Even a small defect in the lower leg and foot with exposure of bones or tendons can result in an intractable wound, which may require a microsurgical tissue transfer. With the concept of the perforator flap, a pedicled peroneal artery perforator flap can be used for coverage of this difficult region. Between August 2001 and August 2008, 18 pedicled peroneal artery perforator flaps were performed in 18 patients. The fasciocutaneous flaps were employed to cover defects in the pretibial area (n=6), Achilles' tendon and/or hindfoot (n=7) and lateral malleolar area (n=5). The pedicled peroneal arte

The modified Kidner-Cobb procedure for symptomatic flexible pes planovalgus and posterior tibial tendon dysfunction stage II: review of 50 feet in 39 patients.

Symptomatic flatfoot is a prevalent disorder. We undertook a review of 50 feet in 39 patients with flexible flatfoot treated between August 2000 and January 2008 in order to evaluate the modified Kidner-Cobb procedure. Overall clinical results were rated as good in 48 (96%) feet and fair in 2 (4%) feet, and there were no poor results. Average follow-up was 4.6 years, and total recovery time was 5.7 months in older patients and 3.7 months in children. Manual muscle-strength testing revealed no difference in tibialis anterior strength between the operated and contralateral extremity. All patient

Brachialis muscle transfer for reconstructing digital flexion after brachial plexus injury or forearm injury.

UNLABELLED: Restoration of digital flexion after brachial plexus injury or forearm injury has been a great challenge for hand surgeons. Nerve transfer and forearm donor muscle transfer surgeries are not always feasible. The present study aimed at evaluating the effectiveness of restoring digital flexion by brachialis muscle transfer. Ten lower brachial plexus- or forearm-injured patients were enrolled. After at least 12 months following surgery, the middle-finger-to-palm distance was less than 2.5 cm in six patients. In the other four patients with less satisfactory results, secondary tenolysi

[Surgical treatment of sternoclavicular joint instability with tenodesis].

BACKGROUND: Instability of the sternoclavicular joint is a very uncommon disorder of the shoulder girdle. Acute traumatic dislocations are commonly treated nonoperatively. But severe displacement or chronic instability with recurrent symptomatic subluxation may require surgical intervention. We present our results with open reduction and internal fixation through an autologous gracilis tendon transplant or fiber tape in 8 patients treated surgically. The operative stabilisation of the sternoclavicular joint reduces pain level and improves function of the shoulder. This technique provides an ef

Posterior elbow coverage using whole and split flexor carpi ulnaris flaps: a cadaveric study.

PURPOSE: The purpose of this study is to evaluate the coverage patterns of whole and split flexor carpi ulnaris (FCU) pedicle muscle flaps for posterior elbow soft-tissue defects. METHODS: Seventeen fresh-frozen cadaveric upper extremities were used. The whole FCU was raised to the dominant vascular pedicle and transposed proximally over the olecranon. The widths of coverage at 2-cm distances about the posterior elbow were measured. Widths were also measured after making 3 longitudinal cuts in the fascia and after suturing the muscle to adjacent soft tissue under tension. The FCU was also spl

Augmentation of tibial fixation of soft-tissue grafts in anterior cruciate ligament reconstruction.

PURPOSE: Fixation of the tibial aspect of soft-tissue grafts in anterior cruciate ligament (ACL) reconstruction is problematic. Augmentation of the interference fit in screw application is seen as a means of improving fixation. This study aims to determine the effect of mesh augmentation of interference screw application in ACL reconstruction. METHODS: Forty cadaveric ovine hind limbs were harvested at necropsy. A quadrupled hamstring tendon graft (QHT), used in human ACL reconstruction, was simulated. Metal and bioresorbable interference screws were used for tibial fixation in the presence o
