Infection of disk space secondary to fistula from pelvic abscess.

Disk space sepsis is a recognized complication of spinal surgery, but infection by a penetrating fistula from a retroperitoneal or pelvic abscess is rare. The lack of localizing symptoms, the insidious disease process, and the clinical and myelographic findings simulating a herniated disk syndrome obscure the true diagnosis and make identification of the primary infection difficult. A good result was obtained in this case by laminectomy and drainage of the lumbosacral disk space, followed by a diagnostic fistulogram and evacuation of a large pelvic abscess.

Pyogenic vertebral osteomyelitis after acute bacterial prostatitis: a case report.

A case of pyogenic vertebral osteomyelitis after acute bacterial prostatitis in a 78-year-old man is reported. The rarity and subtle clinical presentation of this condition, and the delayed appearance of radiologic signs of progression to destructive osteomyelitis, contributed to a significant delay in diagnosis. An arterial blood culture positive for bacterial growth during the episode of acute prostatitis suggested that bacteremia might result from hematogenous spread of the infection to the vertebral column via the venous system. Since intensive antimicrobial therapy proved ineffective, deb

Anterior instrumentation in tuberculous spondylitis: is it effective and safe?

Radical anterior débridement and strut grafting is the gold standard in the surgical treatment of tuberculosis spondylitis. For many years anterior instrumentation was avoided due to concerns about infection until recently. We investigated the effectiveness and safety of anterior instrumentation for different sites of involvement, number of involved levels, and different age groups in 100 consecutive patients (mean age, 44.3 +/- 12.4). We measured preoperative, postoperative, and final kyphotic deformities radiographically and looked for the presence of fusion. Patients had a minimum followup

[First results of anterior versus posterior instrumentation-fusion in the treatment of spondylodiscitis].

AIM: The present study on spondylodiscitis patients was carried out to compare prospectively and randomized the results of clinical and radiological outcomes of anterior versus posterior instrumentation after debridement and grafting. We aimed to make a recommendation concerning the optimum treatment of spondylodiscitis. METHOD: A total of 22 patients with spondylodiscitis were enrolled in the randomized trial to undergo an operation with anterior or posterior instrumentation after debridement and fusion with autologous bone transplant. 12 patients (group 1) were assigned to posterior Univers

Atypical Pott's disease: localized infection of the thoracic spine due to Mycobacterium avium-intracellulare in a patient without human immunodeficiency virus infection.

Mycobacterium avium-intracellulare (MAI) rarely causes disease of the spine in healthy individuals. We describe an elderly woman who had isolated skeletal involvement with MAI, mimicking Pott's disease. She responded well to surgical excision of the inflamed tissue and antibiotic therapy. Osteomyelitis due to MAI must be differentiated from that due to Mycobacterium tuberculosis because the treatment regimens are different.

Liposomal multimodal theranostics for bone disorders: from rational responsive delivery and biomimetic strategies to clinical translation.

Bone and bone-related disorders remain difficult to diagnose and treat because of the dense mineralized matrix, limited vascular perfusion, complex pathological microenvironment, and insufficient site-specific accumulation of conventional agents. Liposomes, as biocompatible vesicular nanocarriers capable of loading therapeutic, genetic, and imaging cargos, have emerged as versatile platforms for bone-oriented theranostics. In contrast to previous reviews mainly focusing on general liposomal drug delivery or individual skeletal diseases, this review provides an integrated perspective on liposom

Immune checkpoint inhibitor use and tuberculosis: a systematic review of the literature.

BACKGROUND: An amassing body of evidence exists to support an association between the use of immune checkpoint inhibitors (ICIs) and the development of tuberculosis (TB). METHODS: We performed a systematic review of the literature to assess the nature of this relationship using PubMed, EMBASE and meeting proceedings. RESULTS: We have identified 16 patients who developed active TB during immunotherapy. Median age was 61 (range: 49-87). Twelve (75%) were male and 4 (25%) were female. Lung cancer was the most common type of cancer (n = 8), followed by melanoma (n = 3) and head and neck cancer (

Enhancing WSI image classification with graph convolutional neural networks and model uncertainty modeling.

BACKGROUND: The primary research question addresses whether integrating Graph Convolutional Neural Networks with model uncertainty modeling can improve the accuracy and robustness of Whole Slide Imaging (WSI) classifications in pathology. METHODS: This study employed a novel framework combining GCNs with uncertainty quantification techniques to classify WSI images of spinal infections. We constructed a graph from segmented regions of WSI, where nodes represented segmented pathological features and edges represented spatial relationships. The model was trained on a dataset of 422 cases from the

Biomechanical Evaluation of Cervical Interbody Fusion Cages for Anterior Cervical Discectomy and Fusion With Variations in Morphology: A Finite Element Analysis.

The spinal diseases commonly faced by people in the 19th century included intervertebral disc degeneration, tuberculosis and congenital defects that resulted in neurological impairment and global disability. To address these issues, cervical spine surgery was performed. Modern techniques currently used in spine surgery include interbody devices, pedicle screws, artificial discs and bone grafts. The postoperative complications clinically reported during follow-up include nonunion and implant subsidence, which remain significant drawbacks. The objective of this study is to develop a 3-dimensiona

Efficacy and Safety of Tofacitinib in the Management of Ankylosing Spondylitis: A Comprehensive Systematic Review.

Ankylosing spondylitis (AS) is a chronic, immune-mediated inflammatory arthritis that can severely impair the quality of life. Although biologic therapies have significantly advanced disease management, a substantial number of patients continue to experience suboptimal treatment outcomes. This systematic review evaluated the efficacy and safety of tofacitinib, an oral Janus kinase (JAK) inhibitor, in the management of AS. We conducted a comprehensive search across multiple databases for studies published between 2015 and 2025, ultimately including 11 relevant studies (two randomized controlled

Pearls & Oy-sters: Autoimmune Glial Fibrillary Acidic Protein Astrocytopathy Presenting as Encephalomyelitis With Leptomeningeal Enhancement.

Autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy is an uncommon diagnosis in the differential for leptomeningeal enhancement. This case highlights the presentation, imaging features, and investigations important for diagnosis of GFAP astrocytopathy to ensure timely treatment of this corticosteroid-responsive disease. A 50-year-old man from Hong Kong presented with 10 days of progressive urinary retention, dysarthria, diplopia, and gait ataxia after a viral illness. Initial nonenhanced MRI brain was negative. After he developed encephalopathy, repeat MRI with gadolinium on admis

Neuroimmune recognition and regulation in the respiratory system.

Neuroimmune recognition and regulation in the respiratory system is a complex and highly coordinated process involving interactions between the nervous and immune systems to detect and respond to pathogens, pollutants and other potential hazards in the respiratory tract. This interaction helps maintain the health and integrity of the respiratory system. Therefore, understanding the complex interactions between the respiratory nervous system and immune system is critical to maintaining lung health and developing treatments for respiratory diseases. In this review, we summarise the projection di

"Tuberculosis on the spot" - Discussion of a probable sternal tuberculosis from a late medieval cemetery from Hungary.

Investigations of non-adult remains are particularly suitable for finding epidemic periods in past populations. This study presents a probable unique example of osseous manifestation of tuberculosis on a child's skeletal remains from medieval Hungary. Between 2009 and 2011 the Field Service for Cultural Heritage excavated the exceptional cemetery of Perkáta - Nyúli-dűlő in Hungary, with around 5000+ graves. The analysed skeleton (SNR 948) was located in the medieval (10-16th century) part of the cemetery. Besides the standard macroscopic pathological observation, we also performed radiographic
