Impact of Right Superior Pulmonary Vein Anatomy and 31-mm Cryoballoon Use on the Risk of Phrenic Nerve Injury During Cryoablation for Atrial Fibrillation.

BACKGROUND: Phrenic nerve injury (PNI) is a common complication of cryoballoon pulmonary vein isolation, especially during right superior pulmonary vein (RSPV) ablation. The impact of a newly introduced size-adjustable 31-mm cryoballoon on PNI and its anatomical predictors remain unclear. METHODS AND RESULTS: This retrospective study included 249 patients who underwent first-time cryoballoon ablation with successful RSPV isolation: 153 with a conventional 28-mm balloon and 96 with a size-adjustable balloon in the 31-mm configuration. Preprocedural computed tomography assessed RSPV anatomical

Robotic-assisted bronchoscopy-guided cryobiopsy for the diagnosis of ground-glass opacity-predominant peripheral pulmonary nodules.

BACKGROUND: The combination of shape-sensing robotic-assisted bronchoscopy (ssRAB) and cryobiopsy may improve the diagnostic yield of ground-glass opacities (GGOs). However, the literature on this approach is limited. This study aims to assess the diagnostic yield and safety of ssRAB integrated with mobile cone-beam computed tomography (CBCT)-guided cryobiopsy for GGO-predominant peripheral pulmonary nodules (PPNs). METHODS: This is a single-center retrospective study of patients suspected to have malignant GGO-predominant PPNs (consolidation-to-tumor ratio ≤ 0.5) who underwent ssRAB integrat

Cryoanalgesia Increases Neuropathic Pain in Adults Undergoing Minimally Invasive Surgery: A 1-Year Follow-up of a Randomized Study.

OBJECTIVE: We previously reported results of a randomized trial comparing standard intercostal nerve block (SOC) with SOC plus cryoanalgesia (CRYO) in minimally invasive lung surgery. While no benefit was seen in early postoperative pain or opioid use, CRYO patients had significantly higher Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) scores at 2 weeks. We now report 1-year follow-up outcomes using LANSS and Visual Analog Scale (VAS). METHODS: This randomized trial (NCT05348447) enrolled adults undergoing minimally invasive thoracic surgery. SOC patients received intercostal ner

Concomitant cryoballoon pulmonary vein and posterior wall isolation in persistent atrial fibrillation: Results from the Left Atrial Posterior Wall Isolation in Conjunction With Pulmonary Vein Isolation Using the Cryoballoon for Treatment of Persistent Atrial Fibrillation (PIVoTAL) IDE trial.

BACKGROUND: Pulmonary vein isolation (PVI) remains the cornerstone of atrial fibrillation (AF) ablation, but its efficacy in patients with persistent AF (PersAF) remains suboptimal compared with those with paroxysmal AF. OBJECTIVE: This study investigated the outcomes of cryoballoon PVI with concomitant "direct" posterior wall isolation (PWI) vs PVI alone in patients with symptomatic PersAF. METHODS: The PIVoTAL IDE was a prospective, multicenter trial (NCT04505163) that randomized patients with symptomatic, drug-refractory PersAF, refractory/intolerant to ≥1 class I-IV antiarrhythmic drug,

Effect of early cryoballoon ablation on atrial fibrillation recurrence-A Danish nationwide register study.

INTRODUCTION: Cryoballoon ablation is a safe and efficient rhythm control strategy in atrial fibrillation (AF) patients. The impact of time from diagnosis to ablation is unclear. The aim of this study was to examine the impact of timing of first-time cryoballoon ablation on AF recurrence in a nationwide cohort of AF patients. METHODS AND RESULTS: From nationwide registers, all AF patients ≥18 years of age who underwent first-time AF cryoballoon ablation in Denmark from 2012 to 2018 were included. The AF patients were stratified by ablation timing: Early group (≤1 year after AF diagnosis), int

Impact of shortening the blanking period to 8 weeks after PVI: Insights from COMPARE-CRYO using continuous rhythm monitoring.

BACKGROUND: Early recurrences of atrial tachyarrhythmias (ERATs) after pulmonary vein isolation (PVI) are common. While many correlate to late recurrences (LRs), some do not. The impact of ERATs timing is difficult to assess with noninvasive Holter monitoring because of undersampling. The recommended duration of the blanking period (BP) was recently shortened from 90 days to 8 weeks. OBJECTIVE: The purpose of this study was to evaluate the impact of ERATs timing after PVI and the effect of a shorter BP by using implantable cardiac monitors for continuous rhythm monitoring. METHODS: This is a

Intercostal nerve cryoablation versus thoracic epidural analgesia for minimal invasive Nuss repair of pectus excavatum: a protocol for a randomised clinical trial (ICE trial).

INTRODUCTION: Epidural analgesia is currently considered the gold standard in postoperative pain management for the minimally invasive Nuss procedure for pectus excavatum. Alternative analgesic strategies (eg, patient-controlled analgesia and paravertebral nerve block) fail in accomplishing adequate prolonged pain management. Furthermore, the continuous use of opioids, often prescribed in addition to all pain management strategies, comes with side effects. Intercostal nerve cryoablation seems a promising novel technique. Hence, the primary objective of this study is to determine the impact of

Comparison of diagnostic yield and safety of endobronchial ultrasound-guided mediastinal lymph nodal cryobiopsy and endobronchial ultrasound-guided Franseen tip needle biopsy.

In this prospective study, we evaluated the diagnostic yield and safety of two endobronchial ultrasound (EBUS) biopsy techniques - mediastinal cryobiopsy (EBUS-MCB) and Franseen tip needle biopsy (EBUS-ANB) - in patients with undiagnosed mediastinal lymphadenopathy. The study included 30 patients who underwent both EBUS-MCB and EBUS-ANB, with four biopsies taken from each patient using both methods. The results demonstrated that EBUS-MCB provided a higher diagnostic yield (96.4%) compared to EBUS-ANB (73.3%). Specimens from EBUS-MCB showed fewer artifacts and a higher density of granulomas and

Cryoballoon versus radiofrequency ablation for paroxysmal atrial fibrillation in hemodialysis patients.

INTRODUCTION: Little evidence exists regarding cryoballoon ablation (CBA) of paroxysmal atrial fibrillation (PAF) in hemodialysis (HD) patients. We compared CBA and radiofrequency ablation (RFA) of PAF in HD patients, referring to CBA of PAF in non-HD patients. METHODS AND RESULTS: This historical cohort study examined 88 patients who underwent catheter ablation of PAF, including 21 HD patients with a second-generation 28-mm cryoballoon (CB-HD group), 17 HD patients with a non-force-sensing radiofrequency catheter (RF-HD group), and 50 non-HD patients with a cryoballoon (CB-non-HD group). Cryoballoon Ablation as Initial Therapy for Atrial Fibrillation.

BACKGROUND: In patients with symptomatic paroxysmal atrial fibrillation that has not responded to medication, catheter ablation is more effective than antiarrhythmic drug therapy for maintaining sinus rhythm. However, the safety and efficacy of cryoballoon ablation as initial first-line therapy have not been established. METHODS: We performed a multicenter trial in which patients 18 to 80 years of age who had paroxysmal atrial fibrillation for which they had not previously received rhythm-control therapy were randomly assigned (1:1) to receive treatment with antiarrhythmic drugs (class I or I

Radial Endobronchial Ultrasound-guided Transbronchial Cryobiopsy.

BACKGROUND: Transbronchial lung cryobiopsy (TBLC) is a novel technique that has proved its diagnostic value in various diffuse parenchymal lung diseases (DPLD). However, there is substantial variability among interventional pulmonologists in procedural technique, diagnostic yield, and complication rate. Radial endobronchial ultrasound (R-EBUS) is useful for identification of ground-glass opacity lesions and can help identify target lung parenchyma. We aim to evaluate R-EBUS in TBLC histopathologic diagnosis for patients with DPLD. METHODS: This was a prospective observational study of patient
