Utilization of BRCA1/2 genetic testing in the clinical setting: report from a single institution.

BACKGROUND: Clinical testing for BRCA1/2 has been available since 1996. Interest in testing in the research and hypothetical situations has been consistently high, but there have been limited reports on its clinical utilization. METHODS: This is a retrospective study of BRCA1/2 test utilization by high-risk patients who were seen at the Johns Hopkins Breast and Ovarian Surveillance Service. RESULTS: Between February 1996 and December 1999, 258 families who had at least a 10% chance of carrying a BRCA1/2 mutation were offered genetic testing. Of these, 26 families seen between February 1996 a

Temporal profiles of physical health in family members of heart transplant recipients: predictors of health change during caregiving.

This study examined patterns of change in the physical health and well-being of 133 family caregivers to heart transplant recipients during the 1st year after transplant. Caregivers were assessed at 2, 7, and 12 months after transplant. Cluster analysis was used to identify temporal profiles reflecting unique patterns of change in the direction and nature of caregivers' physical health; their temporal profiles showed either (a) a worsening of general medical condition, with weight gain (14% of the sample); (b) worsening medical condition with weight loss (15%); (c) weight gain with stable medi

Behavioral effects of nonbenzodiazepine anxiolytic drugs: a comparison of CGS 9896 and zopiclone with chlordiazepoxide.

Zopiclone and CGS 9896 are two nonbenzodiazepine compounds which have been shown to displace benzodiazepines from their binding sites. The present study compared the behavioral effects of these two compounds in rats with those of chlordiazepoxide. The three drugs produced dose-related increases in punished drinking as did pentobarbital and meprobamate but not PK 9084, which also acts at benzodiazepine binding sites, or buspirone. Rates of lever pressing suppressed by punishment were also increased by chlordiazepoxide and zopiclone. CGS 9896 exerted a similar although less marked effect. Lever

Recovery of sensibility after suture of digital nerves.

A group of 49 patients with 74 injured digital nerves was studied more than 5 years following epineurial nerve suture. Three tests of return of sensibility were used: von Frey plastic filaments, a modified Weber two-point discrimination device, and a dynamic test--the plastic ridge. The correlation between two-point discrimination and tactilegnosis in the range of 8 to 12 mm was poor. The plastic ridge device detects the presence or absence of tactilegnosis in patients in the intermediate range of two-point discrimination between 8 and 12 mm. Of the factors studied, the patient's age at the ti

Explainable Machine Learning for Risk Prediction of Reduced Quality of Life in Hypertension.

BACKGROUND: Individuals with hypertension are at risk to reduced quality of life (QoL). Explainable machine learning (ML) can be used for domain-specific risk stratification and prioritization of modifiable determinants of low QoL. OBJECTIVE: To train ML classifier algorithms for QoL risk stratification in hypertension, where meaningful determinants were explored through Shapley additive explanations (SHAP). METHODS: Data from hypertensive individuals (n = 534) completed WHOQOL BREF, Quick Physical Activity Rating, Morisky Medication Adherence Scale 8, and standardized questionnaires for acc

A prospective study of the psychobehavioral factors responsible for a change from non-patient irritable bowel syndrome to IBS patient status.

BACKGROUND: To investigate non-patient irritable bowel syndrome (IBS) change to IBS and to determine factors predictive of the onset of IBS, individual biological factors, psychological factors, behavioral factors, and environmental factors were examined. METHODS: The subjects were 105 non-patient IBS (male = 59, female = 46, average age:21.49 +/- 2.37), including 68 of the diarrhea-predominant type and 37 of the constipation-predominant type selected from 1,409 university and technical college students by use of a questionnaire based on the Rome II diagnostic criteria. The effect of dietary omega-6 fatty acid enrichment in rodent models of military-relevant acute traumatic psychological stress and traumatic brain injury.

INTRODUCTION: Sequelae from traumatic brain injuries (TBIs) and post-traumatic stress disorder (PTSD) are major career-limiting factors for combat soldiers. Overlap between TBI and PTSD symptoms alongside other common comorbidities complicate the diagnosis and treatment. Systems-level and high-throughput approaches are key in understanding the underlying biomolecular mechanisms and differentiating these conditions. METHODS: The present study identifies dietary factors and proposes mechanisms behind psychological stress and TBI, using established preclinical animal models and a multi-omics app

Brain states analysis of EEG predicts multiple sclerosis and mirrors disease duration and burden.

BACKGROUND: Any treatment of multiple sclerosis should preserve mental function, considering how cognitive deterioration interferes with quality of life. However, mental assessment is still realized with neuro-psychological tests without monitoring cognition on neurobiological grounds whereas the ongoing neural activity is readily observable and readable. OBJECTIVE: The proposed method deciphers electrical brain states which as multi-dimensional cognetoms quantitatively discriminate normal from pathological patterns in an EEG. METHOD: Baseline recordings from a prior EEG study of 88 subjects

Tinnitus and its management in patients with vestibular schwannoma.

Tinnitus is the second most common presenting symptom of vestibular schwannoma (VS) after hearing loss. There is conflicting evidence regarding the impact of tinnitus and its contribution to the overall quality of life in VS, and there are many theories regarding the pathogenesis of tinnitus in VS, including cochlear, neural, and central mechanisms. Assessment should include speech audiometry in addition to pure-tone audiometry, as VS patients often have worse speech discrimination than would be expected from their pure-tone thresholds. There is no objective measure of tinnitus. Questionnaires

FACE-Q eye module for measuring patient-reported outcomes in blepharoplasty surgery: A validation study.

This study aimed to conduct a linguistic validation of all FACE-Q eye module scales in German, and to evaluate the reliability and validity of the scales in patients with blepharoplasty. According to international recommendations, all FACE-Q scales and checklists were translated from English to German. Psychometric testing of blepharoplasty surgery-related scales of translated versions were administered to patients with blepharochalasis or dermatochalasis and a history of blepharoplasty surgery (n = 64; 11 scales) or no history of blepharoplasty surgery (n = 65; seven scales), and to a group o

Validation of the Moral Injury Outcome Scale in acute care nurses.

INTRODUCTION: Moral injury, predominantly studied in military populations, has garnered increased attention in the healthcare setting, in large part due to the psychological and emotional consequences of the COVID-19 pandemic. The measurement of moral injury with instrumentation adapted from military settings and validated by frontline healthcare personnel is essential to assess prevalence and guide intervention. This study aimed to validate the Moral Injury Outcome Scale (MIOS) in the population of acute care. METHODS: A sample of 309 acute care nurses completed surveys regarding moral injur

Obsessive-compulsive and perseverative behaviors in Huntington's disease.

Obsessive-compulsive and perseverative behaviors (OCBs/PBs) are characteristic features of Huntington's Disease (HD). Although a few recent research have attempted to discriminate between OCBs and PBs, most of the available evidence on OCBs does not consistently make this distinction. In this article, we aimed to explore the current inconsistencies in assessing and reporting OCBs/PBs and map the body of existing evidence. Up to half of the patients with motor manifest HD can experience OCBs. Separate reporting of PBs in HD patients has been uncommon among the studies and was frequently reporte

Clinical applications of EEG as an excellent tool for event related potentials in psychiatric and neurotic disorders.

Electroencephalography is an excellent tool for assessing neurobiological and neurotransmission dysfunction. Event-related potentials (ERPs) are a simple, non-invasive method of studying neurophysiology. ERPs reflect complex activity of neural networks to blame for discriminative behavior of people and recognition of novel stimuli. They are defined as brain voltage fluctuations linked in time with some physical or mental event. EEG is used to assess ERPs, and its use has grown in popularity since the 1960s. This article provides an overview of the ERP methods as well as the properties of the v

Adolescent Drug Use, Relational Variables and Personality Factors.

OBJECTIVE: An ongoing issue in the study of adolescent drug use is the impact of family and the peer group on the problem of adolescent substance use. The present study has examined relative effects of these contexts as well as personality variables on drug use outcomes. METHOD: A test battery measuring various psychological variables was administered to a representative sample of 1652 secondary school students (grades 9 and 11), 876 male (mean age=17,61, SD=0.99) and 789 female (mean age=16.73, SD=1.31). Psychometric properties of the Generalized Anxiety Disorder 7-item scale in youth: Screening in a primary care sample.

BACKGROUND: Anxiety disorders in youth are frequently underdiagnosed and untreated, partly due to a lack of screening in primary care. The Generalized Anxiety Disorder 7-item (GAD-7) scale is a brief self-report measure designed to screen for anxiety in primary care settings. However, little is known about the psychometrics of this scale with adolescents. METHODS: Participants included 579 youth age 11 to 17 years who received screening for depression in a primary care setting through a web-based application, VitalSign6, over a 4-year period. Psychometric analyses were completed based on clas

Initial validation of the general attitudes towards Artificial Intelligence Scale.

A new General Attitudes towards Artificial Intelligence Scale (GAAIS) was developed. The scale underwent initial statistical validation via Exploratory Factor Analysis, which identified positive and negative subscales. Both subscales captured emotions in line with their valence. In addition, the positive subscale reflected societal and personal utility, whereas the negative subscale reflected concerns. The scale showed good psychometric indices and convergent and discriminant validity against existing measures. To cross-validate general attitudes with attitudes towards specific instances of AI
