In an area in which brugian filariasis is endemic, when cytokine levels were analyzed as a function of clinical status comparing those who were asymptomatic and amicrofilaremic with those who were microfilaremic, it was found that both interferon (IFN)-gamma and interleukin (IL)-5 were suppressed in microfilariae carriers (P < .01 and P < .001, respectively), but IL-4 was unabated. Age had a significant effect on cytokine production in both groups. In asymptomatic amicrofilaremic subjects, IL-4 production was high in young persons and decreased with age, whereas in microfilaremic subjects, IL-

The role of endothelial vascular cell adhesion molecule-1 (VCAM-1) in the trafficking of lymphocytes from the vascular circulation through endothelium and into sites of filarial inflammation was investigated in asymptomatic microfilaremic (MF; n = 16) and in patients with filaria-associated lymphatic pathology (LP; n = 23). When compared, by human umbilical vein endothelial cell ELISA, with PBMC supernatants generated from MF patients, filarial Ag (Brugia malayi adult Ag (BmA))-stimulated supernatants from LP patients stimulated over 50% more VCAM-1 (lymphatic pathology (LP) = 0.377 relative E

Bancroftian filariasis is a major public health problem in the city of Recife in north-eastern Brazil. In some of its urban areas microfilaraemia prevalence reaches 14%. This study describes epidemiological characteristics, infection and disease, in 2 urban areas, Coque and Mustardinha, before control measures were applied. The parasitological survey was performed by a 'door-to-door' census covering 5563 subjects, aged between 5 and 65 years. Microfilaraemia was detected by the thick drop technique, using 45 microL of peripheral blood collected between 20:00 and 24:00. In both areas the preval

This paper reports the results of 10 years of observations on the clinical manifestations, pathology and immunity to filariasis and aetiological biology of filariae in three volunteers (first author and his family members) who were inoculated experimentally with infective larvae of periodic Brugia malayi in 1981. The changes in clinical symptoms and signs were recorded systematically. Microfilariae were first detected at 41 and 46 weeks after inoculation in two subjects and remained detectable in small numbers until 8-8.5 years after infection. The microfilarial density fluctuated at 1-2 mf 12

To compare methods for diagnosing Bancroftian filariasis, six hundred and seventy four people from rural areas of Papua New Guinea were screened using microfilaraemia, the Og4C3 antigen capture ELISA, an ELISA to detect IgG4 specific antibodies and clinical examination. Both ELISA tests detected around twice the number of positive cases than those detected by the presence of microfilariae alone. No correlation was found with clinical signs and laboratory tests. This study raises concern over the underestimation of the prevalence of filariasis due to ineffective diagnostic criteria. The two ELI

BACKGROUND: Mass Drug Administration (MDA) with triple-drug regimen (Ivermectin, Diethylcarbamazine, and Albendazole- IDA), recommended by the World Health Organization (WHO) was introduced in India in 2018, as an alternate strategy to two drug regimens (Diethylcarbamazine, and Albendazole- DA), for accelerating lymphatic filariasis (LF) elimination. By December 2023, IDA-MDA has been implemented in 63 LF endemic districts in India. The currently followed monitoring and evaluation (M&E) guidelines for DA may not be suitable to this new strategy. The WHO is developing a new M&E guideline for ID

BACKGROUND OBJECTIVES: Mass Drug Administration (MDA) of Di-Ethyl Carbamazine and Albendazole is an effective strategy recommended by the World Health Organization to prevent transmission and eliminate lymphatic filariasis. Under the National Vector Borne Disease Control Programme (NVBDCP) in India, MDA is carried out as a yearly activity in endemic areas to accelerate the progress towards elimination of lymphatic filariasis. The study was carried with the objective to determine the coverage of MDA against lymphatic filariasis among the eligible population in Kalaburagi district, Karnataka and

BACKGROUND: Papua New Guinea (PNG) has a high burden of lymphatic filariasis (LF) caused by Wuchereria bancrofti, with an estimated 4.2 million people at risk of infection. A single co-administered dose of ivermectin, diethylcarbamazine and albendazole (IDA) has been shown to have superior efficacy in sustained clearance of microfilariae compared to diethylcarbamazine and albendazole (DA) in small clinical trials. A community-based cluster-randomised trial of DA versus IDA was conducted to compare the safety and efficacy of IDA and DA for LF in a moderately endemic, treatment-naive area in PNG

Podoconiosis is a type of tropical lymphedema that is clinically distinguished from lymphatic filariasis (LF) because it is ascending and commonly bilateral but asymmetric. The disease is a result of a genetically determined inflammatory reaction to long-term exposure to mineral particles in irritant red clay soils derived mainly from volcanic soils. We conducted the first nationwide mapping of the prevalence and risk factors of podoconiosis in Kenya. We performed a population-based cross-sectional survey to determine the national prevalence of podoconiosis and included 6,228 individuals from

The persistence of residual infection is one of the major factors in failure of the Global Programme to Eliminate Lymphatic Filariasis (GPELF). The present study aims to explore the status of sheath antibody and regulatory T cells (Tregs) known to play key roles in clearance of parasite and patent filarial infection, in individuals with residual infection after MDA. A total of 61 microfilaremic (Mf) individuals were followed up after at least 6 rounds of MDA. Infection status of subjects was assessed through the detection of Mf and circulating filarial antigen (CFA). Antibodies to Mf sheath we

Lymphatic filariasis (LF) affects 120 million people around the world and another 856 million people are at risk of acquiring the infection. Mass Drug Administration (MDA) spearheaded by the World Health Organization is the only current strategy to control this infection. Recent reports suggest that despite several rounds of MDA, elimination has not been achieved and there is a need for more stringent control strategies for control of LF. An effective prophylactic vaccine combined with MDA has significant potential. Initial trials using a prophylactic trivalent recombinant Brugia malayi heat s

BACKGROUND: Onchocerciasis control in Côte d'Ivoire started with aerial insecticide spraying in 1974 and continued with community directed treatment with ivermectin (CDTi) from 1992 to the present. Onchocerciasis and lymphatic filariasis (LF) are co-endemic in 46 of the 81 health districts in the country. Fourteen and 12 districts are endemic for only LF or onchocerciasis, respectively. This paper aims to review the impact of past interventions on onchocerciasis in Côte d'Ivoire between 1975 and 2013, and review plans for disease elimination.
METHODS: We reviewed microfilaria (MF, skin snip)

BACKGROUND: During the past 20 years, enormous efforts have been expended globally to eliminate lymphatic filariasis (LF) through mass drug administration (MDA). However, small endemic foci (microfoci) of LF may threaten the presumed inevitable decline of infections after MDA cessation. We conducted microsimulation modeling to assess the ability of different types of surveillance to identify microfoci in these settings.
METHODS: Five or ten microfoci of radius 1, 2, or 3 km with infection marker prevalence (intensity) of 3, 6, or 10 times background prevalence were placed in spatial simulatio

A high proportion of the human population in lymphatic filariasis (LF) endemic areas is positive for filarial specific IgG4 antibodies, including many individuals without microfilariae (mf; circulating larvae in the human blood) or circulating filarial antigens (CFA; marker of adult worm infection). The antibodies are commonly regarded as markers of infection and/or exposure to filarial larvae, but a direct association between the antibodies and these indices has not been well documented. The present study assessed the role and relative effect of potential drivers of the human IgG4 antibody re

Lymphatic filariasis is the leading cause of secondary lymphedema wherein lymph transport is impaired due to lymphatic damage. FLT4 signaling and transcription factors such as FOXC2 play an important role in this type of lymphangiogenesis process induced by filarial parasites. The present study aims to assess the association of FLT4 and FOXC2 genes in lymphatic development/remodeling in lymphatic filariasis. A total of 118 lymphatic filariasis patients and 100 non-endemic and 50 endemic healthy subjects were enrolled for the present study. Allele-specific PCR and PCR-RFLP were adopted for the

BACKGROUND: Lymphatic filariasis is a neglected tropical disease leading to profound disfiguring causing socio economic burden in the tropics. Current diagnosis strategies available during field surveys and epidemics are based on traditional microscopic detections and a few antigen/antibody assays. We have compared different sampling methodologies and standardized the highly sensitive and reliable rWbSXP-1 antigen detection assay to our new sampling methodology.
METHODOLOGY: Samples collected as serum, whole blood, whole blood on filter paper and whole blood on microscopic slides from patient

BACKGROUND: Monitoring and evaluation guidelines of the programme to eliminate lymphatic filariasis require impact assessments in at least one sentinel and one spot-check site in each implementation unit (IU). Transmission assessment surveys (TAS) that assess antigenaemia (Ag) in children in IUs that have completed at least five rounds of mass drug administration (MDA) each with >65% coverage and with microfilaria (Mf) levels <1% in the monitored sites form the basis for stopping the MDA. Despite its rigour, this multi-step process is likely to miss sites with transmission potential ('hotspots

The ribosomal deoxyribonucleic acid (DNA) internal transcribed spacer region (ITS1) of two filarial nematodes, Loa loa and Mansonella perstans, was amplified and further sequenced to develop an species-specific polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) protocol for the differentiation of both species from Wuchereria bancrofti, three filarial nematodes with blood circulating microfilariae. The ITS1-PCR product digested with the restriction endonuclease Ase I generated an specific diagnostic pattern for each of the three species. Moreover, three new specific n

BACKGROUND: Mansonella perstans infection is common in areas of Africa where Wuchereria bancrofti, a causative agent of lymphatic filariasis, is endemic. M. perstans is refractory to standard antifilarial therapies. The recent discovery of bacterial endosymbionts (e.g., wolbachia) in most filarial species, including M. perstans, provides new therapeutic options for reducing microfilaremia.
METHODS: In an open-label, randomized trial, we recruited subjects with M. perstans microfilaremia, with or without concomitant W. bancrofti infection, from four villages in Mali and randomly assigned them

The association between HIV and Wuchereria bancrofti, and the role of malaria and hookworms, were analysed by comparing three groups of individuals with: (1) HIV (HIV+; n=16); (2) W. bancrofti (circulating filarial antigen (CFA)+; n=25); and (3) HIV and W. bancrofti (HIV+/CFA+; n=18). A slightly higher HIV load and lower CD4% was observed in the HIV+/CFA+ group compared with the HIV+ group, and a slightly higher W. bancrofti CFA intensity was observed in the CFA+ group compared with the HIV+/CFA+ group, but none of these differences were statistically significant.
