Trauma affects up to 8% of pregnancies and is the leading cause of death among pregnant women in the United States. A pregnancy test is mandated for all females of childbearing age who are involved in trauma. Orthopaedic trauma in the pregnant patient is managed similarly to that for all trauma patients. Initial resuscitation efforts should focus on the pregnant patient because stable patient vital signs provide the best chance for fetal survival. In the stable patient, fetal assessment and a pelvic examination are mandatory. Radiographs as well as abdominal ultrasound of the patient and fetal

After studying this article, the participant should be able to: 1. Describe the anatomic differences in the subciliary versus the subtarsal approach. 2. Discuss the difference between the "skin-only" and the "skin-muscle flap" variations of the subciliary approach. 3. Discuss the potential complications of both approaches. 4. Discuss the advantages of the subtarsal approach versus the subciliary approach. Many incisions have been described for approaches to orbitozygomatic fractures, the most frequently used being the subciliary incision with its modifications, the subtarsal incision, and the

BACKGROUND: Classifications of nasal fracture are based on clinical findings or radiological findings. The classification systems of nasal fracture usually determine the type of nasal fracture. It is important that a classification gives information about treatment modality and prognosis rather than determining the type of fracture. The objective of this study was to show the effect of the new topographic classification on determining the parameters of prognosis and deciding on treatment modality of the nasal fracture.
METHODS: We reviewed patients with nasal fracture that was referred from e

OBJECTIVES: Atypical femoral fractures (AFFs) are stress or insufficiency fractures occurring in the femoral shaft associated with long-term osteoporosis treatment with antiresorptives. Expeditious diagnosis of AFF is important, as ongoing treatment with antiresorptives is then contraindicated. AFFs can occur bilaterally, either simultaneously or sequentially; prompt diagnosis is pivotal to ensuring appropriate surgical and medical management. The American Society for Bone and Mineral Research (ASBMR) has devised diagnostic criteria based on history and distinctive radiographic features that a

Introduction Pediatric fractures account for one-fourth of all pediatric injuries. Stabilizing the fracture, regulating the length and alignment, encouraging bone healing, and minimizing morbidity and problems for the child and family are the objectives of treatment for diaphyseal fractures of long bones in children. Our goal is to investigate how pediatric diaphyseal long bone fractures are treated with a titanium elastic nailing system (TENS). Methods A prospective interventional study was conducted on 24 children who had displaced diaphyseal fractures of major long bones, involving 31 diaph

We present a case report of a patient who had delayed small bowel obstruction secondary to an incarcerated loop of small bowel within an acute lumbar spine fracture. The patient was involved in a rollover motor vehicle accident, resulting in lumbar spine fractures at L2-4. A comminuted fracture of the L3 vertebral body with likely disruption of the anterior longitudinal ligament (ALL) was noted. The patient underwent L1-4 posterior spinal fusion with the introduction of mild lumbar lordosis to prevent future complications of flatback syndrome. On postoperative Day 4, the patient was noted to h

INTRODUCTION: Impalement injury is a rare type of mechanical injury following forceful insertion of projecting object into the body. Careful planning for removal of the impaling object is essential to decrease the blood loss and preserve the function of the injured organ.
PRESENTATION OF CASE: A 27 year-old male fell from 4m height over a U shaped projecting up metallic bar. The bar penetrated the left side of the pelvis and traversed through the left iliac bone causing a comminuted fracture in the supra-acetabular region extending to the left psoas muscle, injuring the viscera and causing fr

It has been proposed that patients with talocalcaneal and talonavicular coalitions have decreased ankle joint range of motion. It has also been reported that rotational forces regularly absorbed by the talocalcaneal joint are transferred to the ankle joint in patients with coalitions, increasing the stress on the ankle joint after trauma. To the best of our knowledge, only 1 reported study has detailed the increased stress placed on the ankle joint secondary to a coalition. We present a case study of a 53-year-old female who experienced a traumatic fall and subsequent right ankle fracture. Adv

Open reduction and internal fixation was performed on 50 displaced acetabular fractures in 49 patients by nine different attending surgeons over a 10-year period. At an average follow-up of 38 months, poor results were noted clinically in 38% and radiographically in 40%. The incidence of short- and long-term complications was greater than in other studies. Particularly distressing was the 58% incidence of heterotopic ossification (HO). Twenty-four percent had grade III or IV; five hips were autofused and the remainder had 40-60% loss of motion. There was no correlation of HO with age, sex, fra

Scaphoid fractures are the most common carpal bone injuries and remain clinically challenging due to retrograde vascular supply and risks of nonunion, malunion, and avascular necrosis. Surgical fixation is often preferred over conservative management for unstable, displaced, or proximal fractures, offering faster union and earlier functional recovery. This review outlines current fixation techniques, including Herbert and headless compression screws, plate fixation, and newer minimally invasive or hybrid methods. Headless compression screws provide interfragmentary compression, while plate fix

BACKGROUND: Radial head excision has historically been a common surgical procedure for the operative management of radial head fractures and post-traumatic conditions. With recent advances in other surgical techniques, controversy exists regarding its indications. This review evaluates the indications and outcomes of radial head excision in traumatic and non-traumatic elbow pathology.
METHODS: Multiple databases were searched for studies involving radial head excision. Screening and data abstraction were conducted in duplicate. Only studies reporting outcomes for radial head excision were inc

Successful treatment of elbow instability is aided by fundamental biomechanical understanding. This allows for clinical instability to be traced back to structural injuries, which can be addressed with targeted therapy. Acute instability of the elbow joint after frequent posterolateral dislocations tends to affect the radial ligamentous complex. In addition to simple ligamentous injuries, fractures can also contribute to elbow instability; comminuted fractures of the radial head often correlate with a rupture of the ulnar collateral ligament. Improperly healed acute injuries can result in chro

INTRODUCTION: Bosworth fracture-dislocations are rare ankle injuries characterized by posterior entrapment of the fibula behind the tibia, making closed reduction inherently difficult. Their subtle radiographic appearance often delays diagnosis. This case highlights the diagnostic challenges and operative considerations associated with this uncommon and frequently irreducible injury pattern.
CASE REPORT: A 34-year-old man presented after a fall with a posterior malleolus fracture, distal fibular fracture, and tibiotalar dislocation. Closed reduction improved alignment, but persistent mortise

The incidence of periprosthetic and native distal femoral fractures is rising with the increasing utilization of total knee arthroplasty and an aging global population. These injuries present significant clinical challenges, most commonly affecting elderly patients. While fixation-based strategies remain widely employed, distal femoral replacement has emerged as an increasingly utilized alternative. The purpose of this narrative review is to examine the current indications, implant considerations, and clinical evidence supporting the use of distal femoral replacement in the management of both

Aim The Epibloc elastic-dynamic fixation has been applied for many years at several hand surgery centres in Italy. This technique has been considered safe and reliable in the treatment of distal meta-epiphyseal fractures of the radius with dorsal displacement. The aim of this study was to evaluate an alternative use of this method in the treatment of volar displaced wrist fractures in cases where an internal fixation could not be recommended. Methods The procedure consisted of two flexible pins with a trocar tip and an external plate for locking fixation with compression. The surgical techniqu

BACKGROUND: The primary aim was to compare the outcomes of locked plating of closed distal femur periprosthetic and non-periprosthetic fractures by testing the hypothesis that outcomes would be worse in the periprosthetic group. The secondary aim of this study was to identify risk factors for non-union.
METHODS: A single-center study over an 8-year period utilizing a retrospective cohort design was performed. Sixty-eight patients with periprosthetic fractures and 57 patients with non-periprosthetic fractures met inclusion criteria for the study. There was a significant difference between grou
