[Glomerulonephritis secondary to ventriculo-atrial shunts].

The authors report the case of a 45 year old man who had undergone a ventriculo-atrial CSF shunt procedure 5 years previously for normal pressure hydrocephalus and who had several unexplained episodes of infection over a 12 months period and has now developed a mixed nephrotic syndrome associated with a septicaemia. Corynebacterium commensale and Staphylococcus epidermis were isolated from the valve culture. Ablation of the valve resulted in clinical cure with minimal functional renal sequellae. The initial renal biopsy showed type I proliferative glomerulonephritis with subendothelial deposit

Defining a Taxonomy of Intracranial Hypertension: Is ICP More Than Just a Number?

Intracranial pressure (ICP) monitoring and control is a cornerstone of neuroanesthesia and neurocritical care. However, because elevated ICP can be due to multiple pathophysiological processes, its interpretation is not straightforward. We propose a formal taxonomy of intracranial hypertension, which defines ICP elevations into 3 major pathophysiological subsets: increased cerebral blood volume, masses and edema, and hydrocephalus. (1) Increased cerebral blood volume increases ICP and arises secondary to arterial or venous hypervolemia. Arterial hypervolemia is produced by autoregulated or dys

Ventricular Anatomical Differences-A Different Perspective on Endoscopic Treatment of Pediatric Hydrocephalus: Anomalies, Variations, and Deformations.

OBJECTIVE: Endoscopic management of pediatric hydrocephalus is always challenging because of the different anatomical structure of the ventricles. The aim of this study is to document the endoscopic anatomy of the ventricular system and to show the variations and deformations. METHODS: The study included 84 children who underwent endoscopic surgery for the treatment of hydrocephalus in the department of neurosurgery between 2017 and 2022. All preoperative and postoperative radiological images and intraoperative video recordings were retrospectively analyzed. Anatomy of the lateral ventricles

Cerebrospinal fluid pathways from cisterns to ventricles in N-butyl cyanoacrylate-induced hydrocephalic rats.

OBJECT: Cerebrospinal fluid typically enters the subarachnoid space from the ventricles via the fourth ventricular foramina. However, there is clinical evidence that CSF also flows in the opposite direction. Ventricular reflux of CSF from a cistern is a well-known phenomenon in radioisotope studies in patients with normal-pressure hydrocephalus. Additionally, the presence of ventricular blood in acute subarachnoid hemorrhage is frequently observed. The goal of this investigation was to examine the potential CSF pathways from cisterns to ventricles. The authors examined pathways in rat models i

Tuberculous hydrocephalus: comparison of different treatments with regard to ICP, ventricular size and clinical outcome.

The effect of different treatment regimes on intracranial pressure (ICP), degree of hydrocephalus and clinical outcome was evaluated in 81 children with tuberculous meningitis. 24 children underwent CSF shunting, while 57 with communicating hydrocephalus were randomly assigned to three treatment groups: antituberculous drugs only; or additional intrathecal hyaluronidase or oral acetazolamide and furosemide in addition to antituberculous treatment. The addition of acetazolamide and furosemide was significantly more effective in achieving normal ICP than antituberculous drugs alone. No differenc

Local cerebral glucose utilisation in experimental chronic hydrocephalus in the rat.

Many of the clinical features of normal pressure hydrocephalus have been ascribed to stretching of the periventricular nerve fibres. However, many patients may be asymptomatic despite considerable ventriculomegaly. To define the periventricular changes in cerebral energy metabolism we have measured local cerebral glucose utilisation in hydrocephalic and control Sprague-Dawley rats. Experimental hydrocephalus was induced by injection of kaolin into the cisterna magna of 3-week-old rats. After 3 months, the animals fed and walked normally. LCGU was measured using the 14C-deoxyglucose method of S

Reverse brain herniation during posterior fossa surgery.

Posterior fossa tumors are commonly associated with obstructive hydrocephalus. Relieving the raised intracranial pressure by draining the cerebrospinal fluid presents the possibility of reverse herniation of the brain. A 5-year-old male child with a diagnosis of posterior fossa space-occupying lesion and hydrocephalus was scheduled for craniectomy in the prone position. After craniectomy, the surgeons placed an intraventricular shunt catheter to drain out cerebrospinal fluid in an attempt to reduce the tense brain so as to facilitate easy dissection of the tumor. The patient had sudden and sev

The perihippocampal fissures: normal anatomy and disease states.

Understanding the three-dimensional anatomy of the perihippocampal fissures (PHFs) can be helpful in making the correct diagnosis of diseases of the mesial temporal lobe. Disorders of the parenchyma and cerebrospinal fluid spaces are reflected by specific changes in the PHFs. A marker for Alzheimer disease and mesial temporal sclerosis is atrophy of the hippocampus and associated dilatation of the PHFs. This finding is best visualized on coronal magnetic resonance images but can and should be appreciated on routine computed tomographic scans. Hydrocephalus is characterized by dilatation of the

Hydrocephalus as a cause of disturbances of gait in the elderly.

To investigate the possible role of symptomatic normal-pressure hydrocephalus (NPH) in unexplained walking difficulties of the elderly, we used CT to measure lateral ventricular size. We compared data in 50 patients over the age of 60 with a disturbance of gait and 80 control subjects of similar age who walked well and were mentally sound. Forty-six of the 50 gait patients (92%) had a ventricular span of more than 12 mm, whereas only 5 of the 80 controls (6.3%) had ventricles of that size. There was a narrow zone of overlap. Seventeen of the gait patients later improved after surgical shunting

The role of mesencephalic aqueduct obstruction in hydrocephalus development: a case report.

We report on three patients with mesencephalic aqueduct obstruction, which completely blocked the cerebrospinal fluid communication between the third and fourth cerebral ventricle, demonstrated by standard and high-resolution magnetic resonance sequences. Only one patient developed radiological and clinical presentation of hydrocephalus, without radiological signs of increased intraventricular pressure. The remaining two patients did not show clinical signs of hydrocephalus and had a normal radiological presentation of the ventricular system. These findings contradict the classical concept of

Long-term outcome in 109 adult patients operated on for hydrocephalus.

To examine the long-term effects of surgery in adult hydrocephalus we conducted a cross-sectional questionnaire study assessing both the patients' sense of well-being, and changes in gait, living conditions, daily need of sleep and bladder function. One-hundred-and-nine consecutive patients operated for non-communicating hydrocephalus (N-CH) (22) and communicating normal pressure hydrocephalus (NPH), both idiopathic (38) and secondary (49) were included. For survival analyses, three reference groups were selected from the general population and from the Northern Sweden MONICA Project. At long-

[Dementia: role of imaging].

This article summarizes the role of imaging in the evaluation of cognitive disorders of the elderly. The primary goal of imaging is to exclude potentially reversible dementia due to intracranial lesions such as tumor, subdural hematoma or normal pressure hydrocephalus. The value of neuroimaging in the diagnosis of degenerative disorders is then discussed: Alzheimer's disease, frontotemporal dementias, dementia with Lewy bodies, dementia associated with parkinsonian syndromes, vascular dementias and mild cognitive impairment.

Low-Pressure Hydrocephalus After Traumatic Brain Injury: A Case Series and Scoping Review.

Background/Objectives: Low-pressure hydrocephalus (LPH) is an underappreciated pathology in which patients present with ventriculomegaly and intracranial pressures (ICPs) below normal, making traditional pressure-driven cerebrospinal fluid diversion treatment strategies challenging. LPH has been observed in patients following traumatic brain injury (TBI), often within the neurocritical care unit (NCCU). However, its presentation and associated patient characteristics remain unclear. Thus, we sought to compile patients from our institution and the literature to characterize LPH after TBI. Metho

Communicating Hydrocephalus After Radiotherapy for a Vestibular Schwannoma.

Hydrocephalus is a known tumour complication associated with vestibular schwannomas, which can be either the obstructive or communicating (non-obstructive) type. The latter represents a rare and controversial complication following radiotherapy, typically resulting from tumour necrosis. Management generally involves cerebrospinal fluid diversion. Here, we present a case of normal-pressure and communicating hydrocephalus that developed after radiotherapy for a vestibular schwannoma. The patient was a 72-year-old woman who initially presented with imbalance and right sensorineural hearing loss.

Cerebral autoregulation among patients with symptoms of hydrocephalus.

OBJECTIVE: To study the relationship between the resistance to cerebrospinal fluid (CSF) outflow and cerebral autoregulation. METHODS: We examined 35 patients who presented with ventricular dilation and clinical symptoms of communicating hydrocephalus. For all of these patients, CSF compensatory reserve was investigated by using a computerized infusion test, with simultaneous recording of blood flow velocity wave forms (by using transcranial Doppler ultrasonography) and arterial blood pressure (with a Finapress finger cuff). The resistance to CSF outflow was calculated as the absolute increas

Creatine kinase BB release into cerebrospinal fluid after lateral ventricle cannulation.

Creatine kinase (CK) and creatine kinase BB isoenzyme (CK-BB) activities were measured in ventricular CSF obtained by lateral ventricle cannulation in patients suspected of normal pressure hydrocephalus. Lateral ventricle cannulation resulted in highly and variably elevated CK and CK-BB levels. The results emphasise the interference of these CK and CK-BB elevations in studies on the prognostic value of CSF CK and CK-BB levels in head trauma patients with respect to outcome.

Behavior of an extradural pressure monitor in clinical use. Comparison of extradural with intraventricular pressure in patients with acute and chronically raised intracranial pressure.

A comparison of intraventricular pressure (IVP) and extradural pressure (EDP) was carried out in 17 patients being investigated for normal-pressure hydrocephalus, and in six patients with acutely raised intracranial pressure following events such as head injury or intracerebral hematoma. Extradural pressure was measured using the CardioSearch monitor. There was a reasonably good correlation between EDP and IVP in the chronic stable group with pressures up to 25 mm Hg. In the acute group there was no predictable relationship between EDP and IVP, and during a 24-hour period the pressures could v

Classification of slit-ventricle syndromes using intracranial pressure monitoring.

The term slit-ventricle syndrome appears to have different meanings to different authors. This study proposes a sublcassification of headaches in shunted children based on data obtained from chronic monitoring of intracranial pressure in 7 symptomatic children. Five distinct syndromes are identified: (1) intermittent, extremely low pressure headaches that are analogous to spinal headaches, (2) intermittent proximal obstruction, (3) shunt failure with small ventricles ('normal volume hydrocephalus'), (4) intracranial hypertension with working shunts (hydrocephalic pseudotumor), and (5) headache

Hypodipsic hypernatremia in a dog with defective osmoregulation of antidiuretic hormone.

Hypernatremia was detected in a dog that was evaluated because of seizures. During hospitalization, the dog was fully conscious and remained hypernatremic when drinking voluntarily and when water was added to the food. Urine volume increased and urine osmolality decreased during an infusion of hypertonic saline (2.5% NaCl) solution, despite development of progressive hyperosmolality. There was no correlation between plasma antidiuretic hormone concentration and osmolality during the infusion study. The dog released antidiuretic hormone normally after nonosmotic stimulation (ie, apomorphine adm

Bilateral Lateral Ventricle Cerebrospinal Fluid Diversion in a Rare Case of Foramen of Monro Occlusion: A Case Report.

Obstructive hydrocephalus in young adults is a rare and often misdiagnosed condition. Normal pressure hydrocephalus (NPH), typically affecting older adults, may resemble obstructive forms when bilateral foramen of Monro occlusion is present. Traditional cerebrospinal fluid (CSF) diversion may be insufficient in cases with isolated ventricles. We present a unique case of a 37-year-old patient with presumed NPH, a prior endoscopic third ventriculostomy (ETV), and persistent symptoms due to undiagnosed foraminal obstruction. The patient presented with progressive gait instability and memory impai
