sWe describe the management of a cystic duct remnant calculus in a 45-year-old male patient who had undergone a laparoscopic cholecystectomy and re-presented with abdominal pain and jaundice. Magnetic resonance cholangiopancreatography was utilized to confirm the diagnosis of an impacted calculus within the remnant cystic duct along with several small retained common bile duct stones. Four sequential endoscopic procedures successfully removed all retained common bile duct calculi to alleviate the biliary obstruction; however, we were unable to treat the cystic duct remnant calculus endoscopica

Alagille syndrome is an autosomal dominant disorder characterized by abnormalities in multiple organ systems, including the liver, and is caused by mutations in JAG1. Chronic cholestasis secondary to paucity of interlobular bile ducts is traditionally both a clinical and a pathologic hallmark of this disease at diagnosis. We describe the biliary changes on serial liver biopsies in a patient who presented with jaundice and extrahepatic stigmata of Alagille syndrome. Her initial specimens at 6 and 10 months of age demonstrated interlobular bile duct proliferation and cholestasis, suggestive of d

BACKGROUND: Veno-occlusive disease of the liver is a common cause of morbidity and mortality after chemotherapy and/or radiotherapy and bone marrow transplantation. Non thrombotic hepatic venous obstruction is characteristic, but pathogenesis of this disease remains unknown.
CASE REPORT: In june 1993, a 58-year-old man was treated by cyclophosphamide and steroid for a POEMS syndrome. In november 1996, he developed hepatalgia, edema, ascitis and jaundice. The diagnosis of hepatic veno-occlusive disease was obtained by hepatic biopsy. Refractory ascitis was treated by transjugular intrahepatic

In 178 patients undergoing elective cholecystectomy for biliary colic, the papilla was calibrated with a no. 10 French catheter through the cystic duct. The catheter passed through the papilla into the duodenum in 94 patients. Six (6 percent) had common duct stones. No postcholecystectomy colic occurred. In 37 patients the catheter was obstructed by papillary stenosis and sphincteroplasty was done. Common duct stones occurred in 17 patients (46 percent), jaundice in 7 and dilatation of the common duct in 6. Cholangiograms failed to detect small stones in seven patients and were of minimal valu

Operative cholangiography should be used routinely during operations on the gallbladder and biliary tree. The indications for operative cholangiography include demonstration of unsuspected stones, of abnormal biliary tree anatomy, and of tumour within intrahepatic branches of the biliary tree; determination of the nature of periampullary obstruction and of the best approach for removal of stones impacted low in the common bile duct; avoidance of unnecessary choledochotomy; assistance in diagnosis of stenosing cholangitis, cholangiolytic jaundice or choledochal cyst; and to ensure, postexplorat

Caroli-like disease is a rare congenital ductal plate malformation characterized by intrahepatic bile duct dilatation and has only rarely been reported in veterinary medicine. A 2-year-old castrated male small-breed dog (5.4 kg) was presented with jaundice, weight loss, and lethargy. Laboratory tests revealed elevated liver enzyme activities, hyperbilirubinemia, and coagulopathy. Radiography showed two mineralized opacities superimposed on the hepatic silhouette. Ultrasonography revealed cholecystolithiasis, gallbladder wall thickening, and biliary dilatation with choledocholithiasis, suggesti

Hepatic angiosarcoma (HAS) is an uncommon malignant tumor that is often misdiagnosed because of its nonspecific presentation and overlapping imaging findings with benign vascular tumors. Sinusoidal obstruction syndrome (SOS) is a disorder of hepatic microcirculation, usually associated with myeloablative chemotherapy and hematopoietic stem cell transplantation. The clinical and pathological overlap between SOS and hepatic angiosarcoma may complicate the diagnosis of hepatic angiosarcoma, as illustrated by this case. We report the case of a sixty-year-old man who presented to the emergency depa

BACKGROUND AND AIMS: Annular pancreas is a rare congenital condition where the second part of the duodenum is encircled by pancreatic tissue. There is a scarcity of data on its natural history therefore, we aimed to evaluate the epidemiology of annular pancreas and identify underlying associations using a large database.
METHODS: A multi-institutional database (Explorys) was surveyed. A cohort of patients with a Systematized Nomenclature of Medicine-Clinical Terms diagnosis of "MRI, CT, EUS and/or ERCP" between April 2015 and April 2020 was identified. Subsequently a cohort of patients with h

The simplest definition of Santoro's operation is a sleeve gastrectomy with transit bipartition. Santoro et al. reported long-term data regarding sleeve gastrectomy with transit bipartition, which is a similar operation to duodenal switch but without complete exclusion of the duodenum to minimize nutritional complications and to allow endoscopic management of obstructive jaundice. Afterward, several studies proved the efficacy and safety of transit bipartition; the real benefit of this operation is the reduction of side effects and protein malnutrition compared with the bilio-pancreatic divers

BACKGROUND: Acute cholestatic hepatitis without features of infectious mononucleosis is a rare presentation of primary Epstein-Barr infection, with only several cases previously reported in the medical literature. Early investigation for Epstein-Barr virus in febrile patients with deranged liver function tests and no demonstrable biliary obstruction on imaging can expedite both diagnosis and treatment, thereby avoiding costly or invasive procedures such as liver biopsy.
CASE PRESENTATION: A 59-year-old white woman of Anglo-Saxon descent presenting with a febrile illness was noted to have a ch

PURPOSE: The major complication occurring with biliary stents is stent occlusion, frequently seen because of tumour in-growth, epithelial hyperplasia, and sludge deposits, resulting in recurrent jaundice and cholangitis. We report a prospective study with the results of first in man percutaneous intraductal radiofrequency (RF) ablation to clear the blocked metal stents in patients with malignant biliary obstruction using a novel bipolar RF catheter.
METHODS: Nine patients with malignant biliary obstruction and blocked metal stents were included. These patients underwent intraductal biliary RF

INTRODUCTION: Invasion of the portal and hepatic veins by hepatocellular carcinoma (HCC) is common, but macroscopic bile duct invasion is rare. Once a tumor thrombus completely obstructs the main bile duct, it causes obstructive jaundice. This type of HCC, known as icteric-type HCC (IHCC), has a poor prognosis.
PRESENTATION OF CASE: A 72-year-old woman had been treated for chronic hepatitis C since 1997. In 2002, percutaneous ethanol injection therapy was performed for HCC in segment 8. HCC recurrence occurred in 2004, and she underwent transarterial embolization (TAE) and radiofrequency abla

CONTEXT: Intraductal papillary mucinous neoplasm (IPMN) of the pancreas occasionally penetrates to others organs. We present a case of IPMN penetrating to the stomach and the common bile duct.
CASE REPORT: A 75-year-old man was admitted to the hospital because of epigastric pain. Computed tomography (CT) showed a papillary tumor protruding into the markedly dilated main pancreatic duct and splenic vein obstruction. The tumor was diagnosed as IPMN arising in the main duct, but he rejected surgery and he was followed without treatment. One year later, gastroduodenoscopy revealed gastropancreati

Duodenal intussusception is a rare entity. To date, only a few cases have been reported in the literature. In this report, a case of duodenal intussusception due to an unusual tumor was presented and the clinical features of this entity were discussed. A 42-year-old man with Peutz-Jeghers syndrome presented with epigastric pain, vomiting, and severe anemia. Computed tomography scan revealed synchronous duodenojejunal and jejunojejunal intussusceptions. An emergency laparotomy revealed a polypoid mass originating from the lateral wall of the descending duodenum with intussusception of the dista

BACKGROUND/AIMS: Distal bile duct stenosis is relatively rare in patients with non-alcoholic chronic pancreatitis.
METHODOLOGY: The clinical features of eight patients who had chronic pancreatitis complicated by bile duct strictures who underwent surgical treatments were reviewed.
RESULTS: Ages ranged from 38 to 80 years, with a mean of 53.4 years. All but one patient were male. Six patients had moderate or slight epigastric pain. Five patients had obstructive jaundice and underwent biliary drainage. All patients had liver dysfunction due to biliary obstruction. Although four of the eight pa

Pablo Luis Mirizzi was the first to describe an obstructive jaundice caused by compression of the common hepatic duct by the stone in the cystic duct or the neck of the gallbladder in 1948. McSherry et al in 1982. described a new type of Mirizzi's syndrome calling it type II. Csendes et al in 1989. gave a new classification in four types. According to it, type II of Mirizzi's syndrome was devided in three types depending on the size of the destruction of the common hepatic duct. We previously described a subtype of Mirizzi's syndrome in which besides very wide cholecystohepatic fistula, a comb

During a 48 month period to December 1990, 367 patients, median age 75 years, with obstructive jaundice caused by common bile duct stones (201), malignant biliary obstruction (148), and benign biliary strictures (18), underwent therapeutic endoscopic retrograde cholangiopancreatography. Endoscopic biliary stenting and drainage was achieved in 343 of 367 patients attempted (93%), seven patients requiring a combined percutaneous endoscopic approach. Endoscopic stenting failed in 24 patients because of malignant duodenal infiltration (10), Billroth 2 gastrectomy (6), tight and extensive biliary s

To obtain a histopathologic diagnosis at the site of a biliary obstruction, we recently have performed 24 cases of biliary biopsy using gastrofiberscopic biopsy forceps (Olympus, Tokyo, Japan) via transhepatic tracts provided in the course of the procedure of percutaneous biliary drainage. Histopathologic diagnosis was successfully made at the first attempt of biopsy procedure but a second trial was made a week later in 6 cases who were negative for malignant cells on the first attempt. The histological results from the biopsy specimens were 18 adenocarcinomas, 5 chronic inflammations and one
