INTRODUCTION: Results of the skull vibratory test (SVT) in partial unilateral vestibular peripheral lesions (PUVL) are different from the results in total vestibular lesions (TUVL).
AIM: To reveal a correlation between the results of the analysis of the skull vibratory nystagmus (SVN) horizontal component and the side of the lesion; to correlate these results with the stimulus frequency. To find out a predictive correlation between the SVN horizontal and vertical components and the topography of a vestibular lesion. To appreciate the degree of vestibular deafferentation (extended to high freq

A study of 197 epileptic outpatients consecutively observed by one Neurologist allowed a prospective study of the side effects of antiepileptic drugs. This study shows essentially that a clinical improvement comparable to that generally reported can be obtained with side effects limited in their number and intensity. The main principle is the routine search of these effects which are not always spontaneously mentioned by the patients, especially intellectual slowness and loss of sexual activity. The detection of the side effects through history and physical examination is far more fruitful tha

The vestibular system provides inputs to many neurons in the brain stem that participate in autonomic control. This multiplicity of vestibular-autonomic connections plays a variety of roles. Whereas it has been known for decades that unilateral vestibular lesions can result in motion sickness, recent data suggest that the vestibular system participates in making adjustments in blood pressure and respiration that are necessary to maintain homeostasis during movement and changes in posture. Animals with bilateral vestibular lesions are more susceptible to posturally related hypotension than vest

The term 'endolymphatic hydrops' is not a definitive diagnosis since the condition occurs in a variety of otological disorders. Endolymphatic hydrops was found in 57 ears (9 per cent) of the 703 temporal bones in the collection of the Ear Research Institute, Los Angeles. These 57 ears included normal ears of two newborns, post-stapedectomy ears, and ears with the primary diagnosis of Menière's disease. Paget's disease, congenital syphilis, and chronic otitis media. The typical clinical picture of fluctuant hearing loss, pressure, tinnitus, and episodic vertigo occurred in about one-third of th

A study was made of the anatomical structure and functional significance of the ultriculo-endolymphatic (UE) valve in man. The material consists of 170 human temporal bones of which 75 were chosen to show normal structure through nine decades of life, 29 with endolymphatic hydrops, 22 with developmental defects and 44 with pertinent staining characteristics of the endolymphatic fluid. The studies show that the UE valve is ideally suited to preserve the humoral and anatomical features of the pars superior (utricle and canals) from the developmental, disease, and traumatic susceptibilities of th

HYPOTHESIS: There is increased expression of adrenergic receptors in the vestibular end organs of Meniere disease.
BACKGROUND: Meniere disease (MD) is an inner ear condition characterized by episodic vertigo, hearing loss, tinnitus, and aural fullness. It is believed to be due to an imbalance in inner ear fluid, often leading to hydrops; however, the exact mechanism is unclear. Pupillometry data have suggested a role for autonomic dysfunction, specifically sympathetic hyperactivity, but few studies have investigated the molecular pathways involved in this.
METHODS: Vestibular end organs from

OBJECTIVE: Sleep dysfunction is common after traumatic brain injury (TBI) and can be difficult to manage due to medication side effects and complex neuropsychiatric comorbidities. Noninvasive electrical vestibular system stimulation (VSS) is an emerging neuromodulation therapy that has demonstrated benefit for primary chronic insomnia in adults without known brain injury, but has not been described for chronic insomnia in individuals with TBI.
RESULTS: We present a retrospective case series of 5 adult veterans with chronic TBI and moderate-to-severe insomnia (Insomnia Severity Index [ISI] ≥15

Alzheimer's disease (AD) is often mixed with cerebrovascular disease (AD-CVD). Heterogeneity of dementia etiology and the overlapping of neuropathological features of AD and AD-CVD make feature identification of the two challenging. Separation of AD from AD-CVD is important as the optimized treatment for each group may differ. Recent studies using vestibular responses recorded from electrovestibulography (EVestG™) have offered promising results for separating these two pathologies. An EVestG measurement records responses to several different physical stimuli (called tilts). In previous researc

BACKGROUND: Cervical and ocular vestibular-evoked myogenic potentials (VEMPs) have become common clinical vestibular assessments. However, VEMP testing requires high intensity stimuli, raising concerns regarding safety with children, where sound pressure levels may be higher due to their smaller ear canal volumes.
PURPOSE: The purpose of this study was to estimate the range of peak-to-peak equivalent sound pressure levels (peSPLs) in child and adult ears in response to high intensity stimuli (i.e., 100 dB normal hearing level [nHL]) commonly used for VEMP testing and make a determination of w

Out-of-body experiences (OBEs) are states during which people experience their centre of awareness as located outside of their physical body, along with the sensation of seeing the environment from an elevated viewpoint. OBE is encountered in epilepsy, migraine and depersonalization, and it is not an uncommon experience in the general population. Current neuroscientific models of bodily self-consciousness consider that OBE are related to a failure to integrate visual, somatosensory and vestibular signals. These models have highlighted the importance of visual-vestibular mismatch in OBE. Case r

BACKGROUND: Vestibular-evoked myogenic potentials (VEMPs) are commonly used clinical assessments for patients with complaints of dizziness. However, relatively high air-conducted stimuli are required to elicit the VEMP, and ultimately may compromise safe noise exposure limits. Recently, research has reported the potential for noise-induced hearing loss (NIHL) from VEMP stimulus exposure through studies of reduced otoacoustic emission levels after VEMP testing, as well as a recent case study showing permanent sensorineural hearing loss associated with VEMP exposure.
