The authors have reviewed their experience in the management of 55 patients admitted to Henry Ford Hospital with symptoms of vertebrobasilar insufficiency and associated proximal vertebral artery stenosis or occlusion. In 48 patients, the symptoms occurred as multiple repeated events, five of which resulted in permanent deficits. The remaining seven patients had single events, four of which caused permanent deficit. These patients had been treated unsuccessfully with antiplatelet agents (37 cases) and with anticoagulant drugs (15 cases) before surgery. Most patients had multiple angiographic a

UNLABELLED: Invasive mold diseases (IMDs) such as mucormycosis and aspergillosis carry high mortality despite optimal antifungal therapy. Adjunctive immunomodulation, including anti-PD-1 antibodies and interferon-γ, may help restore antifungal immunity in severely ill patients. We implemented multidisciplinary team meetings in France to assess and validate the use of adjunctive anti-PD-1 monoclonal antibodies and interferon-γ in combination with antifungal therapy for patients with life-threatening IMDs. Here, we report the characteristics and outcomes of the treated patients. Twelve cases wer

BACKGROUND: In amyotrophic lateral sclerosis (ALS), heterogeneity of motor phenotypes is a fundamental hallmark of the disease. Distinct ALS phenotypes were associated with a different progression and survival. Despite its relevance for clinical practice and research, there is no broader consensus on the classification of ALS phenotypes.
METHODS: An expert consensus process for the classification of ALS motor phenotypes was performed from May 2023 to December 2024. A three-determinant anatomical classification was proposed which is based on the (1) region of onset (O), (2) the propagation of

INTRODUCTION: Although COVID-19 is mainly a respiratory disease, recent evidence has emerged of vascular and procoagulant pathologies even in young and otherwise healthy individuals. Ophthalmic manifestations include, among others, visual impairment due to arteritic and venous retinal obstructions, which at times precedes other aspects of the disease. We present two atypical cases of internal carotid dissection (ICAD) and review the different ocular symptoms of ICAD and its association with the COVID-19 pandemic.
BACKGROUND: A 43-year-old otherwise healthy man was referred to the Emergency De

BACKGROUND: Women in sub-Saharan Africa have the highest rates of morbidity and mortality during childbirth globally. Despite increases in facility-based childbirth, gaps in quality of care at facilities have limited reductions in maternal deaths. Infrequent physiologic monitoring of women around childbirth is a major gap in care that leads to delays in life-saving interventions for women experiencing complications.
METHODS: We will conduct a type-2 hybrid effectiveness-implementation study over 12 months to evaluate using a wireless physiologic monitoring system to detect and alert clinician

BACKGROUND: The transition to the new ICD-10 (International Classification of Diseases, Tenth Revision) coding system in the U.S. poses challenges to the ability to consistently and accurately measure trends in comorbidities and complications. We examined the prevalence of comorbidities and postoperative medical complications before and after the transition from ICD-9 to ICD-10 among patients who underwent primary total hip or knee arthroplasty (THA or TKA). We hypothesized that the transition to ICD-10 codes was associated with discontinuity and slope change in comorbidities and medical compl

OBJECTIVE: Extrapleural pneumonectomy for therapy of mesothelioma has been associated with significant perioperative mortality and morbidity. Postoperative complications of this procedure require a unique management approach. We developed treatment algorithms for most of the common complications of extrapleural pneumonectomy resulting in reduced mortality and hospital stay. Complications after extrapleural pneumonectomy were further analyzed to elucidate means of prevention, early detection, and treatment.
METHODS: A total of 496 patients undergoing extrapleural pneumonectomy were reviewed fo

BACKGROUND/PURPOSE: Identifying major trauma patients in the prehospital setting is essential in determining management, destination, and best utilization of emergency department resources. Few methods of trauma triage have been accepted unanimously. This study prospectively evaluates the efficacy of comprehensive field triage using 12 criteria (simplified version of the American College of Surgeon's guidelines) in 1,285 pediatric trauma patients.
METHODS: Major trauma was defined as occurring in those who died in the emergency room, had major surgery (penetrating injury involving surgery of

INTRODUCTION: Schizophrenia is a severe psychiatric disorder. Catatonia is relatively common in schizophrenia; its main manifestations include catatonic stupor, mutism, negativism, and other psychomotor symptom clusters. Patients with schizophrenia are at increased risk of infections, which are also recognized triggers for Guillain-Barré syndrome (GBS). GBS typically presents with limb weakness, paresthesia, facial weakness, respiratory muscle paralysis, and autonomic symptoms, and may similarly result in severe immobility and impaired communication. Consequently, when schizophrenia (especiall

INTRODUCTION: The COVID-19 pandemic caused by SARS-CoV-2 has profound implications for global public health. It significantly affects the nervous system and cognitive function. The emergence of long COVID has raised concerns regarding long-term cognitive impairment, particularly among patients who experienced neurological symptoms during the acute phase of infection.
AIM: This study investigated the impact of neurological symptoms during acute SARS-CoV-2 infection on subsequent cognitive change and neurological sequelae over a 2-year period.
METHODS: We enrolled 3,419 hospitalized patients w

PURPOSE: To explore the clinical efficacy of unilateral modified arytenoidectomy (UMA) in the treatment of bilateral vocal fold paralysis(BVFP) based on animal models.
METHODS: The UMA which had been tested in excised canine larynges with simulated BVFP was adapted into a clinical technique. The characteristics of this surgical technique involve contouring of the arytenoid cartilage. The procedure expands the vocal fold respiratory region and subglottic space of the arytenoid cartilage while preserving the muscular process to maintain partial vocal fold muscle function. A retrospective analys

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disease with progressive muscle wasting, paralysis, and respiratory failure. Whereas approximately 10-15 % of ALS cases are familial, the etiology of the remaining, sporadic ALS cases remains largely unknown. Environmental exposures have been suggested as causative factors for decades, and previous studies have found elevated concentrations of metals in ALS patients.
PURPOSE: This meta-analysis aims to assess metal concentrations in body fluids and tissues of ALS patients.
METHODS: We searched the MEDLINE and EMBASE

BACKGROUND: Patients undergoing thoracoscopic procedures may be at high-risk for incomplete neuromuscular recovery and associated complications. The aim of this clinical investigation was to assess the incidence of postoperative residual neuromuscular blockade in adult thoracic surgical patients administered neostigmine or sugammadex when optimal dosing and reversal strategies for these agents were used. The effect of choice of reversal agent on hypoxemic events and signs and symptoms of muscle weakness were also determined. Additionally, operative conditions in each group were graded by surge

PURPOSE: To validate the effectiveness of percutaneous pedicle screw (PPS) fixation for spinal fractures associated with diffuse idiopathic skeletal hyperostosis (DISH) by comparing surgical outcomes for PPS fixation and conventional open posterior fixation. Patients with DISH are vulnerable to unstable spinal fractures caused by trivial trauma, and these fractures have high rates of delayed paralysis, postoperative complications, and mortality.
METHODS: This retrospective study assessed surgical outcomes for 16 patients with DISH (12 men; mean age 76.1 ± 9.4 years) who underwent PPS fixation
