Coinfection of Schistosoma mansoni and Strongyloides stercoralis in a patient with variceal bleeding.

We report a case of hepatosplenic schistosomiasis with portal hypertension and variceal bleeding in an immigrant patient from Egypt, coinfected with Strongyloides stercoralis. The diagnosis was based on the following: (a) identification of Schistosoma mansoni ova in the stools and colonic biopsy specimens, (b) portal hypertension and esophageal varices with normal liver function and the absence of hepatic cirrhosis stigmata, (c) history of migration from an endemic area and (d) ultrasonographic findings of spleen and liver enlargement, fibrosed portal tracts, and normal lobular architecture of

Infection of naïve, free-living brushtail possums (Trichosurus vulpecula) with the nematode parasite Parastrongyloides trichosuri and its subsequent spread.

Despite the importance of spatial processes in host-parasite interactions, parasite dispersal has been the subject of few experimental studies. Introduced marsupial common brushtail possums (Trichosurus vulpecula) are a major environmental and agricultural pest in New Zealand. Parastrongyloides trichosuri, an intestinal rhabdiasoid nematode parasite specific to possums, is being evaluated as a self-disseminating delivery system for engineered fertility control vaccines. This study addressed whether an artificial infection could be established in a naïve, free-living possum population, by measu

Strongyloides robustus and the northern sympatric populations of northern (Glaucomys sabrinus) and southern (G. volans) flying squirrels.

Within North America, northern (Glaucomys sabrinus) and southern (Glaucomys volans) flying squirrels occupy distinct ranges with limited overlap. Sympatry in northern latitudes coincides with northern hardwood vegetation from Minnesota to New England. Strongyloides robustus is an intestinal parasite that infects both species but appears to be deleterious only to northern flying squirrels. As a result, S. robustus could be a critical determinant of flying squirrel population characteristics in at least some areas of sympatry. However, cold weather could potentially limit the distribution of S.

Changing paradigms in parasitic infections: common dermatological helminthic infections and cutaneous myiasis.

Parasitic diseases caused by helminths, or worms, account for billions of human infections worldwide. Although most human pathogens caused by these organisms are infrequent in the United States, skin manifestations of parasitic diseases are being seen much more frequently in dermatologists' offices due to the increase in immigration and travel. Helminths are notable for their complicated lifecycles often including consecutive developmental phases, in separate hosts or in a free-living state. These parasitic organisms are usually macroscopic, multicellular organisms, and do not breed within the

Strongyloides stercoralis infestation associated with septicemia due to intestinal transmural migration of bacteria.

BACKGROUND: The majority of patients with uncontrolled severe CRSwNP, asthma and atopic dermatitis share a similar T helper 2 type inflammation linked to their underlying phenotype. This discovery has triggered new research around treatments targeting specific cytokines driving inflammation in CRSwNP like IL-4, IL-13, IL-5 and IgE. Biologicals are increasingly tested as additional tre- atment for patients suffering from severe chronic rhinosinusitis with nasal polyps (CRSwNP). Their efficacy has been demonstrated in multiple studies. All studies differ in terms of baseline characteristics of i

Overwhelming strongyloidosis in a foal.

A 6-month-old foal was evaluated because of weakness, weight loss, and inappetence of 3 weeks' duration. On initial examination, the foal was weak, poorly responsive, and emaciated. Clinicopathologic abnormalities included anemia and hypoproteinemia. Because of its severe debilitation, the foal was euthantized. Necropsy revealed marked infection of the small intestine with Strongyloides westeri and severe edema of the entire gastrointestinal tract. The foal had been orphaned when it was 6 hours old and raised in isolation from other horses. We believe that this foal developed overwhelming stro

[Strongyloides stercoralis infection in a cortico-dependent patient with chronic airflow obstruction].

We report a case of Strongyloides stercoralis infection in a male agricultural worker with corticoid dependent chronic obstructive air flow disease and bronchial hyperreactivity. The clinical picture initially involved digestive symptoms and bronchospasm resistant to steroid treatment. Diagnosis was established by demonstrating the larvae of S. stercoralis in stool. We emphasize that this infection should be suspected in patients living in endemic areas such as Spain, and describe S. stercoralis-caused respiratory manifestations that can be masked by bronchial disease.

Selective effector mechanisms for the expulsion of intestinal helminths.

In the middle of the era of molecular biology, much less attention is paid to in vivo phenomena. However, carefully designed experimental systems in vivo still can provide valuable information as to the mechanisms underlying the establishment and maintenance of host-parasite relationships. In this review we describe the advantage of using concurrent infections with appropriately chosen combinations of different genera or different maturation stages of parasites to segregate the cellular responses of the host. By means of simple experimental approaches we have found that mucosal mast cells and

L-dopa malabsorption in a parkinsonian patient with Strongyloides stercoralis duodenitis.

We report a parkinsonian patient initially responding to L-dopa who developed a severe loss of drug efficacy due to Strongyloides stercoralis duodenitis. The patient was put on mebendazole and metronidazole, and the parasitosis abated, allowing L-dopa reduction by 33%. Our patient illustrates the advisability of searching for Strongyloides stercoralis when L-dopa malabsorption is suspected in Parkinson's disease.

Strongyloides stercoralis: an initial autoinfective burst amplifies primary infection.

Compartmental analysis of Strongloides stercoralis burdens in experimentally infected, serially necropsied dogs was used to test an autoinfective burst hypothesis. The hypothesis states that in well-established, active infections and in chronic infections as well, the rate of larval development is down-regulated so that most larvae do not attain infectivity internally. The majority pass in the feces as preinfective, rhabditiform larvae, but a few (those with the most rapid developmental rate) attain infectivity internally, and therefore are positioned for autoinfectivity. In contrast, in immun

A basis to extend the proof of migration routes of immature parasites inside hosts: estimated time of arrival of Nippostrongylus brasiliensis and Strongyloides ratti in the gut of the rat.

The time taken for larvae of Nippostrongylus brasiliensis and a homogonic strain of Strongyloides ratti to complete their migration from the skin surface to the small intestine of the rat was estimated by a new method in which a mirror image of arrival times was created by counting mature parasites at day 8 (N. brasiliensis) or day 5 (S. ratti) in the intestines of rats that had received a single pulse of morantel tartrate by stomach tube at different times starting 10 h after skin application of exact doses of infective larvae. It was confirmed that the effect of the drug was confined to para

Possible effects of Strongyloides fuelleborni-like infections on children in the Karimui area of Simbu Province.

A parasitological survey of 75 children less than 5 years of age from the village of Yuro, 5km south-east of Karimui, revealed that 47 (63%) were infected with the Strongyloides species resembling S. fuelleborni; 42 (56%) were infected with hookworm. Poor compliance with an intervention study of thiabendazole therapy resulted in the abandonment of the proposed follow-up survey. However, with the small number of children who did comply, we found evidence to suggest a link between high Strongyloides egg count and poor nutritional status.

Transmammary transmission of Strongyloides ratti.

The rate of transmammary transmission of Stronglyloides ratti was examined in albino rats in terms of the route of subcutaneous (s.c.) migration from the infection site (the skin) to the cranium. Inoculation sites nearer the cranium resulted in less frequent transmammary infection. The maximum number of adult worms was recovered from the sucklings when the mother was inoculated in her hindquarter and sucklings were allowed to feed for 30-36 h after inoculation (AI). Few worms were recovered from sucklings when they were allowed to nurse during periods of less than 24 h AI or greater than 42 h

Cultural ecology of prehistoric parasitism on the Colorado Plateau as evidenced by coprology.

The study of coprolites (desiccated feces) is recognized as a viable method for analyzing parasitism of prehistoric peoples. Eight species of helminth parasites, including nematodes, cestodes, and acanthcephalans, have been recovered from archaeological sites on the Colorado Plateau. The comparative analysis of parasitological findings illustrates the effects of changing subsistence strategies and varying life-style on prehistoric human parasitism. This comparative study is based on the analysis of coprolites recovered from one Archaic hunter-gatherer site and two Anasazi agricultural villages

Localization of mucosal mast cells in W/Wv mice after reconstitution with bone marrow cells or cultured mast cells, and its relation to the protective capacity to Strongyloides ratti infection.

Localization of mast cells in the intestinal epithelium, villous lamina propria and basal lamina propria of mast cell-deficient WBB6F1 (W/Wv) mice reconstituted with either bone marrow cells or with cultured mast cells (BMMC) was compared to that of mast cell-sufficient C57BL/6 or C57BL/6-bgj/bgj (beige) mice after infection with Strongyloides ratti. In mast cell-sufficient C57BL/6 or beige mice, the maximum number of intestinal mucosal mast cells (MMC) was more than 160 MMC/10 villus crypt units (VCU) and more than 90% of MMC were located in the intestinal epithelium. When W/Wv mice were reco

Fever, petechiae, and pulmonary infiltrates in an immunocompromised Peruvian man.

The diagnostic considerations raised by immunocompromised patients with opportunistic infection continue to expand. When such patients harbor latent or persistent infection acquired in a tropical environment, the diagnostic challenge is even greater. The Infectious Disease Service at Yale-New Haven Hospital was asked to see a middle-aged man from Peru with known T-cell lymphoma who had recently completed a course of chemotherapy. He presented to the hospital with fever, petechial skin rash, pulmonary infiltrates, and neutropenia. Ultimately this case illustrated the necessity for careful evalu

Unsuspected strongyloides coexisting with adenocarcinoma of the lung.

The findings in an unsuspected case of Strongyloides stercoralis infestation coexisting with adenocarcinoma of the lung are presented. The diagnosis of adenocarcinoma was confirmed histologically by lung biopsies; the parasitic infection was initially diagnosed by sputum cytology and then by bronchial washings. A stool specimen submitted to the microbiology laboratory confirmed the diagnosis of S. stercoralis infection. This case again suggests the possibility that neoplastic disease may play a role similar to that of immunosuppression in creating an immune deficiency, thus increasing the susc
