[Electromyographic analysis of the role of the otolith apparatus in regulation of postural disorders in humans with extrapyramidal pathology].

In 30 patients with cervical dystonia in the posture of orthostasis, an obvious change of the electromyographic pattern was shown both at rest and in different functional tests. The analysis has shown an enhanced excitability of motoneurons and a disorder of the cortical control, as well as enhancement of the irradiation process between paired structures controlling activity of opposite muscular groups. After treatment with intratympanal administration of an ototoxical preparation on the opposite side in respect to head turn direction, normalising of the cortical control and diminishing of the

Abnormalities of the vestibulo-ocular response in congenital esotropia.

In 32 infants with congenital esotropia, I measured the vestibuloocular response. The eight patients with nystagmus compensation syndrome exhibited brisk responses. Fourteen of the 24 who had estropia without nystagmus had severely diminished responses and the other ten had moderate responses. Vestibulo-ocular testing is useful in detecting subtle forms of nystagmus compensation syndrome, especially in young infants, in whom thorough examination of ductions and versions is difficult. Dysfunction of the vestibular system is not necessarily a primary cause of congenital estropia, but brain-stem

Hereditary neuronal intranuclear inclusion disease with autonomic failure and cerebellar degeneration.

BACKGROUND: Neuronal intranuclear inclusion disease (NIID), a multiple-system degeneration, occurs usually as a sporadic disorder with onset in childhood. The disease has been found in monozygotic twins and in siblings. In 2 previously described families, the disorder has affected 2 generations. OBJECTIVE: To investigate the clinical, anatomical, and electrophysiological characteristics of NIID that affect the central nervous system and the central and peripheral components of the autonomic nervous system in 2 successive generations of a family. DESIGN: Case report. SETTING: Tertiary care h

Case Report: Cognitive Impairment without Clinical Spinal Disease May Be the First Sign of HTLV-1 Neurological Alteration.

Human t-cell lymphotropic virus type 1 (HTLV-1)-associated myelopathy (HAM) is a progressive neurological disease whose diagnosis is defined by clinical manifestations and seropositivity for HTLV-1 infection. Cognitive impairment (CI) is considered to occur after spinal impairment. A 51-year-old HTLV-1-infected man classified as an asymptomatic carrier presented difficulties in listening comprehension and executive memory. He was assessed for central auditory processing (CAP), cognition (event-related auditory evoked potential [P300]), and otoneurological functions (galvanic vestibular-evoked

Familial lateral semicircular canal malformation with external and middle ear abnormalities.

We report a family with inner ear lateral semicircular canal (LSC) malformation and external and middle ear abnormalities. The family had no history of known syndromes or toxic exposures. Distinct phenotypic manifestations were found in three family members. A young girl exhibited bilateral LSC malformation with a right-sided preauricular tag, a mildly deformed auricle, a stenotic external auditory canal, and a constricted middle ear cavity. She had moderate conductive hearing loss in the right ear but normal hearing in the left ear. Her younger brother exhibited right-sided LSC malformation,

Dizziness predicts initial worsening in panic disorder: an otoneurological investigation.

Dizziness is a frequent complaint during the initial worsening of panic patients treated with clomipramine. In order to investigate the relationship between dizziness and initial worsening mechanisms, otoneurological tests were administered to 32 consecutive patients with panic disorder, with or without agoraphobia, before treatment, 36 h after the first dose of 25 mg clomipramine and after treatment and full remission of the panic and agoraphobic symptoms. A pretreatment complaint of 'dizziness during panic attacks' but not the occurrence of otoneurological abnormalities, predicted the occurr

Clinical course of persistent geotropic direction-changing positional nystagmus with neutral position-Light cupula.

CONCLUSION: A short clinical course and frequent recurrence are common features of persistent geotropic direction-changing positional nystagmus with neutral position (positional nystagmus of light cupula: PNLC) and cupulolithiasis of the lateral semicircular canal. It is suggested that PNLC is caused by light debris attached to the cupula of the lateral semicircular canal. OBJECTIVES: PNLC is a sub-type of direction-changing positional nystagmus. It is thought to be caused by anti-gravitational deviation of the cupula of the lateral semicircular canal (light cupula); however, the exact mechan

Differences in Postoperative Hearing Outcomes and Vertigo in Patients with Otosclerosis Treated with Laser-Assisted Stapedotomy versus Stapedectomy.

BACKGROUND: Otosclerosis is an abnormal bone growth in the otic capsule that can result in hearing loss. In this study, we compared postoperative hearing outcomes and vestibular symptoms between patients treated with laser-assisted stapedotomy versus stapedectomy. METHODS: The medical charts of 99 ears treated with stapes surgery were retrospectively reviewed. RESULTS: A stapedotomy, partial stapedectomy, or total stapedectomy was conducted in 77, 16, and 56 ears, respectively. The ears treated with partial- and total stapedectomies were unified into one stapedectomy group. The postoperative

Clinical hints and precipitating factors in patients suffering from Meniere's disease.

Meniere disease is one of the most fascinating and most vexing of all clinical conditions encountered by the otolaryngologist. Operationally speaking, a Meniere ear is a fragile ear. In fact, Meniere disease can and should be redefined as a degenerating inner ear that has impairment of one or more homeostatic systems, resulting in instability of hearing and balance function. This updated definition is a valuable guide to the clinical epidemiology and presentation of Meniere disease and to understanding the effects of conservative treatments. In the absence of a definitive test for Meniere dise

Is balance normal in migraineurs without history of vertigo?

OBJECTIVE: To investigate by static posturography the occurrence of balance disorder in migraineurs without a history of vertigo during the interictal period. BACKGROUND: The link between migraine and balance disorders has long been known but postural balance in migraineurs without manifest vestibulopathy has been rarely studied. METHODS: We studied 25 migraineurs and age- and gender-matched controls. With static posturography we measured: (1) postural sway with eyes open or closed on a platform or on foam with 4 different head positions; (2) limits of stability as patients change their cent

Superficial siderosis causing retrolabyrinthine involvement in both cochlear and vestibular branches of the eighth cranial nerve.

Although superficial siderosis (SS) has been clinically characterized as a combination of sensorineural hearing impairment, cerebellar ataxia and pyramidal signs, precise evaluation of the function of the eighth cranial nerve has rarely been reported. The purpose of this study was to evaluate the audiological and vestibular function. We present a patient with complaints of progressive bilateral hearing loss and gait difficulty. We evaluated the audiological and vestibular functions with auditory brainstem responses and vestibular evoked myogenic potentials (VEMPs) by clicks and galvanic stimul

Compensatory changes in static and dynamic subjective visual vertical in patients following vestibular schwanoma surgery.

OBJECTIVE: Among patients with vestibular schwanoma (VS), vestibular function is nonhomogeneous, both before and after surgical removal of the VS. This paper reports investigations of neural changes, especially changes in the contribution of visual input to vestibular system integration, after VS surgery. METHODS: We examined 33 patients who underwent VS surgery via a middle fossa approach. Static and dynamic subjective visual vertical (SVV) was measured once after surgery and compared to those measured in control subjects. SVVs were assessed using a paradigm requiring the subject to manually

Therapeutic efficacy of the Epley canalith repositioning maneuver.

OBJECTIVES/HYPOTHESES: The hypotheses of the current study are as follows: 1) That if the Epley canalith repositioning maneuver is an effective treatment for benign positional vertigo (BPV), relief from the vertigo should occur virtually immediately after the performance of the maneuver; 2) that the Epley canalith repositioning maneuver does provide almost immediate relief in BPV and should be the established treatment of choice for this disorder in both primary and tertiary care settings; and 3) that residual symptoms of lightheadedness and imbalance do persist after the resolution of the ver

Phase difference of vestibulo-ocular reflex in head autorotation test.

In the head autorotation test (HART) gain is commonly used to determine the vestibulo-ocular reflex (VOR). The purpose of this work was to evaluate the relevance of phase (the phase difference between the eye and corresponding head movement) in describing the VOR. The phase was determined in 5 frequency bands in both degrees and milliseconds (ms) in 4 groups: 30 healthy adults, 15 elderly subjects, 37 vestibular schwannoma (VS) patients, and 35 Meniere's disease (MD) patients. In healthy adults and in MD patients the phase was close to nil in the frequency bands of 3 and 4 Hz. In elderly subje

Clinical survey of Meniére's disease: 574 cases.

Only 27% out of 574 patients suffering from Meniére's disease initially started with the typical Meniére's triad. A great advantage in order to diagnose Meniére's disease can be established with the telemetric-ENG. With this equipment it is possible to verify a nystagmus outside the clinic (at home, at work) at any time in patients suffering from vertigo attacks. Most of our patients showed an omnifrequent sensorineural hearing loss on the affected side. Many of our patients with Meniére's disease suffered from internal diseases (e.g. hypotonia, hyperlipidemia, diabetes mellitus), allergy or a

[Disability: a proposal for an anamnestic questionnaire in the evaluation of the degree of disability caused by disorders of equilibrium].

Correct identification and evaluation of balance disorders and an assessment of their repercussions on a patient's life are common problems faced by the otoneurologist. These symptoms may not infrequently have a chronic or recurrent course and can lead to in restrictions in the patient's everyday life and even have consequences for the patient' mental health. In the doctor-patient relationship anamnesis is of crucial importance in diagnosis as well as in establishing a relationship of professional confidence with the patient which may influence the outcome of the treatment. Relationship between GBST and equilibrium examination in vestibular neuronitis.

Eleven patients with vestibular neuronitis were followed up from 1988 to 1990. The relationship between the GBST and other vestibular examinations was investigated. Vertiginous symptoms showed relative improvement, but in the last medical examination 4 out of 11 cases (36.4%) complained of unsteadiness when their head was rotated and fatigued. Seven patients (63.6%) presented spontaneous and positional nystagmus at the first consultation, but only one patient (9.1%) at the last one. Seven patients (63.6%) showed canal paresis to caloric stimulation, while 3 (27.3%) showed normal responses. In
